Substantiation. An employee shall substantiate the use of sick leave by such reasonable means as the Police Chief may require. Falsification of any sick leave affidavit or fraudulent use of sick leave shall be grounds for disciplinary action up to and including discharge.
Substantiation. The Executive shall, as a condition of any such payment or reimbursement, submit verification, substantiation and documentation of the nature and amount of such expenses in accordance with the policies of Company from time to time.
Substantiation. It is understood that any time off charged to disability leave pursuant to Section 3 and Section 4 of this Article may require substantiation to the satisfaction of the COUNTY prior to compensation. Failure to provide satisfactory substantiation will result in denying compensation and may result in disciplinary action pursuant to Article 5, DISCIPLINE AND DISCHARGE, of this Agreement.
Substantiation. The following procedures shall be applied for purposes of substantiating claimed Eligible Medical Expenses after the use of a Debit Card to pay the claimed Eligible Medical Expense:
(i) If the dollar amount of the transaction at a health care provider equals the dollar amount of the co-payment for that service under the Employer’s major medical plan of the specific employee-cardholder, the charge is fully substantiated without the need for submission of a receipt or further review.
(ii) If the merchant, service provider, or other independent third-party (e.g., pharmacy benefit manager), at the time and point of sale, provides information to verify to the Recordkeeper (including electronically by e-mail, the internet, intranet, or telephone) that the charge is for a medical expense, the charge is fully substantiated without the need for submission of a receipt or further review.
Substantiation. Employees shall substantiate any claim for repair or replacement with evidence clearly indicating that damage was caused by circumstances beyond the control of the employee. Receipts for payment of repair or replacement costs will be required. Request for substantiation by the District shall not be unreasonable and shall be submitted on agreed upon forms.
Substantiation. The Employee’s Department Head will verify and substantiate the reasons whenever possible, and, after die consideration of the requirements of the Department together with the length of service, attendance, work performance and attitude of the Employee, will forward the request with his recommendations to the Town Administrator for transmittal to the Board of Selectmen for their consideration and approval or denial.
Substantiation. A. An employee shall substantiate the use of sick leave to his/her department head.
B. No sick leave allowance will be made for the day before or after a holiday, or scheduled days off, without presenting a doctor's certificate of illness; but, a department head, at his/her discretion, can waive the need for a doctors certificate of illness.
Substantiation. The Executive must:
(a) provide receipts or other documentary evidence of payment satisfactory to the Employer and the purpose of each expense in a form reasonably required by the Employer to support each claim for reimbursement; and
(b) keep those records of expenses reasonably required by the Employer to meet income tax, fringe benefits tax and other statutory requirements.
Substantiation. A taxpayer claim- ing a reduction of taxable income under this section must maintain records sufficient to show entitlement to the reduction as provided in forms, instructions, publications or other guidance published by the IRS.
Substantiation. The Contractor shall provide Company with documentation supporting all expenses.
