THE STUDENT AGREES TO. 3.1 Comply with the Experience Provider’s policies and procedures.
3.2 Comply with the applicable educational institution’s dress and grooming standards and honor code.
3.3 Enroll as an academic internship student and perform the duties indicated unless released by the applicable educational institution and the Experience Provider.
3.4 Report serious problems, including safety and personnel problems, to the Internship Coordinator of the applicable educational institution and the Experience Provider.
3.5 Maintain personal health insurance or student health insurance.
3.6 Execute a separate Student Internship Agreement with BYU agreeing to abide by the terms of this Agreement and to perform additional duties and responsibilities as outlined in the Student Internship Agreement.
THE STUDENT AGREES TO. 4.1. to study full-time and to complete the EACH programme in regular studies;
4.2. pay the participation cost for each semester by due date in accordance with Conditions of Compensation of Study Expenses. The tuition fee is paid on the basis of an invoice that the university issues to the person paying the tuition fee electronically at least 14 days before the due date. The faculty submits to the Finance Office a directive of the vice xxxx for academic affairs or an invoice proposal on the basis of a contract for reimbursement of study costs or an application of (an international) visiting student at least 21 days before the due date. In case of two payments, the due dates for 2018/2019 are the following: for the autumn semester October 20th and for the spring semester March 20th. In case of paying the tuition fee in a lump sum for the entire academic year, the due date is October 20th.
4.3. in the event of failure to pay the tuition by the due date, pay late interest at the rate of 0,06% of the overdue amount per day of delay;
THE STUDENT AGREES TO. Be supervised by and meet with SVUSD teacher and administrator at their scheduled times.
THE STUDENT AGREES TO. 1. Enroll in the appropriate Internship Education course, for at least one semester, upon accepting employment in a college-approved Internship Education placement.
2. Attend the initial internship orientation meeting.
3. Continue enrollment in an approved certificate or degree program at the College while participating in a college-approved internship work experience placement.
4. Maintain a minimum Grade Point Average of 2.0 while participating in an internship work experience placement.
5. Identify and write specific work performance and learning objectives relating the internship work experience to his/her academic studies, for each semester enrolled in an internship education course.
6. Keep a Weekly Log of work activities and submit it to the faculty supervisor at mid-term and prior to finals.
7. Participate in activities designed to provide an overview of all applicable aspects of the industry.
8. Prepare a final five to eight page paper summarizing the work experience, or an alternative project approved by your Faculty Internship Advisor.
9. Participate in the student/employer evaluation process.
10. Turn in workbook and all other assignments to the faculty supervisor.
11. Approach responsibility to the employer and College with dedication, enthusiasm, and maturity.

THE STUDENT AGREES TO. To identify a social, civic issue, or a professional area of interest Develop a plan for personal involvement in addressing the issue or learning about the professional area, as well as evaluating and reflecting on such experience through a paper/reflection Serve the assigned service agency in order to receive training and volunteer experience Adhere to all rules and regulations of the organization and always act in ethical manner Participate in any required training sessions Develop knowledge and skills necessary to provide effective service to the organization and its clients Be responsible for providing own transportation to and from the site, as necessary Be punctual and attend all scheduled hours In the event of illness or emergency, notify the organization as soon as possible
THE STUDENT AGREES TO a. read and comply with the information provided in this Memorandum of Understanding by signing and returning this contract to the University PRIOR to the commencement of the LiWC experience, which will activate coverage by the University’s Insurance Policy;
b. behave in a professional manner, including being punctual, attending on all agreed days for the duration of the contract, behaving professionally, not disclosing any confidential information of the Host Organisation, dressing appropriately and performing tasks satisfactorily;
c. work in accordance with the policies, procedures, directions and requirements of the Host Organisation (including, without limitation, those relating to occupational health and safety, equal opportunity, confidentiality and information privacy);
d. advise the Host Organisation if the Student suffers from any medical condition or disability that may affect his/her work performance;
e. comply with the statutes, policies and procedures of the University (including, without limitation, the Learning in the Workplace Policy & Procedures and the procedures relating to occupational health and safety, equal opportunity);
f. maintain communication with the University and the workplace supervisor(s);
g. be available and prepared to discuss relevant issues when visited or contacted by the University, complete relevant LiWC evaluation documentation and submit to the University at the completion of the LiWC experience;
h. address any issues/concerns that arise with the workplace supervisor in order to resolve them, referring any unresolvable issues on to the University;
i. immediately advise the Host Organisation and the University of any accident or incident in the workplace; and
j. at the completion of the activity return to the Host Organisation all its property or equipment including security cards, computer disks, documents and records and all copies of such material in the possession or control of the Student.
THE STUDENT AGREES TO. 1) Perform assigned duties to the best of his/her ability. The student is considered a member of the staff and, as such, should aim at being a productive member of the staff.
2) Keep the agency/institution/organization’s best interests in mind at all times and be punctual, dependable, and loyal to them.
3) Follow directions, avoid unsafe acts, and be honest in all dealings with the supervisor and the agency/institution/organization’s customers/clients.
4) Complete required forms before beginning placement.
5) Register with Minnesota West for academic credit (45 hours/1credit required).
6) Keep the supervisor and faculty internship/field experience supervisor informed about any changes in his/her program or intentions. The student must gain permission from the internship/field experience supervisor if he/she considers dropping internship/field experience credit.
