Value Added Benefits. Any information provided by You shall be kept confidentialFor services which are provided through empanelled medical experts/ centres/ service providers, We are only acting as a facilitator, hence We would not be liable for any incremental cost of the services. • All medical services are being provided by empanelled medical experts/ centres/ service providers who are empanelled after full due diligence. Nonetheless, Insured Person may consult their personal doctor before availing the medical services. The decisions to utilise the services will solely be at the Insured Person’s discretion. • We/Company/Us or its group entities, affiliates, officers, employees, agents, are not responsible for or liable for any actions, claims, demands, losses, damages, costs, charges and expenses which an Insured Person/ You may claim to have suffered or sustained or incurred by way of or on account of utilization of any benefits specified herein. • This shall not be deemed to substitute the Insured Person’s visit or consultation to an independent Medical Practitioner. The Insured person is free to choose whether or not to undergo the same and if done whether or not to act on it. • We do not assume any liability towards any loss or damage arising out of or in relation to any opinion, advice, prescription, actual or alleged errors, omissions and representations made by the Medical Practitioner.

(d) Online Pharmacy, Diagnostic tests and other Health/ Wellness Offering We will facilitate the Insured Person for various offerings on health and wellness services like Diagnostic Centers, Pharmacy, Gymnasiums, Yoga, etc. through the Network Providers/ vendor tie- ups. (e) Health tips/ articles We will provide You information on various health related applications, wellness training, maintaining fitness and good health, information on various diseases, dietary plans, etc. through periodic communications and through online platform. (f) Home Health We will provide through vendor tie-ups, Home Health services like physiotherapy, nursing care, trained attendants and medical equipments, for the Insured Person. (g) E-consultations We will provide with or arrange for an online platform through vendor tie-ups for providing with E-consultations to the Insured Person. Value Added Benefits. Improving may provide additional benefits and services in addition to Vendor Products as part of this Agreement. Improving reserves the right to modify or change the definition, description and scope of these benefits and services at any time. Customer will be notified of any changes through the support services provided within this Agreement as defined under the Improving CSP Support Agreement or by email or regular mail. Benefits and service provided and performed by Improving are non-transferrable.
Value Added Benefits. The Contractor may offer Value-Added Benefits and Services in addition to the capitated benefit package to support the health, wellness, and independence of Dental Health Plan Enrollees and to advance the State’s objectives for the SoonerSelect Dental program. Value Added Benefits. 2.5.5.1 As permitted under 42 C.F.R. §438.3(e)(1), the Contractor may offer value-added benefits (VAB) which are not Medicaid covered services or prohibited services. Value- added benefits are provided at the Contractor’s expense, are not included in the capitation rate, and shall be identified as value-added benefits in encounter data in accordance with the MCO Manual. 2.5.5.2 At a minimum, the Contractor shall offer the VAB proposed in its response to the RFP and agreed upon by LDH, consistent with this Section. Additional VAB may be offered, at the Contractor’s option, and shall be reported in accordance with the MCO Manual. 2.5.5.3 At the Contractor’s discretion, it may provide or assist enrollees with transportation to access a VAB. Encounters for transportation related to VAB shall be identified as such. 2.5.5.4 The Contractor may propose to change the VAB proposed in the Contractor’s RFP response on an annual basis as pre-approved in writing by LDH. Additions, deletions or modifications to these VAB shall be submitted to LDH for approval at least six (6) months in advance of the effective date for open enrollment. 2.5.5.5 The VAB proposed in the Contractor’s RFP response, and as amended annually, shall be listed in Attachment C, Value-Added Benefits. 2.5.5.6 Annually, for the VAB proposed in the Contractor’s RFP response, and as amended, the Contractor shall: 2.5.5.6.1 Indicate the PMPM actuarial value of VAB based on enrollment projections for the Contractor’s plan, accompanied by a statement from the preparing/consulting actuary who is a member of the American Academy of Actuaries certifying the accuracy of the information; and 2.5.5.6.2 Include a statement of commitment to provide the VAB for the year. 2.5.5.7 The Contractor shall be directed to revise its proposed PMPM based on any feedback from LDH, following an independent review of any statements of actuarial value provided by the Contractor. 2.5.5.8 The proposed monetary value of value-added benefits shall be considered a binding Contract deliverable. If for any reason, including but not limited to lack of enrollee participation, the aggregated annual PMPM proposed is not expended by the Contractor, LDH reserves the right to require the Contractor to provide an alternate benefit of equal value and/or may conduct a reconciliation for the amount unexpended. 2.5.5.9 Value-added benefits are not subject to appeal and state fair hearing rights.
