Accident/Incident Reports. The provisions of Part I, 6.15 shall apply to part-time operators.
Accident/Incident Reports. An operator involved in an accident or incident shall provide a written accident/incident report of the event within 24 hours of the time the event took place, or was brought to the operator’s attention. Operators preparing the statement outside their normal working hours shall be paid ½ hour at the regular rate of pay. Operators requested to attend further investigation of the matter after submitting a complete and properly prepared Accident or Incident report shall be paid one hour at the regular rate of pay.

Accident/Incident Reports. Children sometimes receive bumps and bruises as part of their daily routines. An accident report will be completed by a staff person when a child is injured and filed with the Program Director by the end of the program day. When an injury is caused by another child, an incident report will be completed, and parents will be notified by the staff. All reports are filed in the child’s file. The staff will use basic First Aid procedures for all bumps and bruises. If more care is needed, the parent will be notified and asked to seek additional care with the child’s physician. If a child requires immediate attention, 911 will be called and the parent / guardian will be contacted. The child will be transported by ambulance to the most appropriate medical facility. If the parent is unable to accompany the child to the hospital, the Director or a staff member will remain with the child until a parent/guardian arrives.

(a) A payment of 50% of one-hour straight time shall be made when an employee is required to complete any report requested by management outside their regular working hours. Time for completion of such reports is to be entered on time cards and paid at the time the report is completed to the satisfaction of Transit Management.
(b) If an employee is called in by the Department or Municipality outside their normal working hours, for an interview for any purpose other than completing an Accident/Incident Report, the employee shall be paid at straight time rates for the time of such interview. A minimum payment of one (1) hour shall apply.

A written report of the accident/incident involving injuries and/or potential injuries will be presented to POINT ARENA within 10 business days, and shall include, but not be limited to, all reports which are required by other regulatory and law enforcement agencies. TRIBE covenants and agrees, and shall, cooperate with and timely provide POINT ARENA such further and additional information as POINT ARENA may require relative to such accidents and/or incidents. A written report of the accident/incident involving injuries and/or potential injuries will be presented to the CHARTER SCHOOL within 10 business days, and shall include, but not be limited to, all reports which are available by other regulatory and law enforcement agencies. DISTRICT covenants and agrees, and shall, cooperate with and timely provide the CHARTER SCHOOL such further and additional information as CHARTER SCHOOL may require and request relative to such accidents and/or incidents.
Accident/Incident Reports. The Union will appoint a member from the MFC Unit as an observer to the Accident or Incident Investigation Committee (for Type A or B incidents) when a represented employee is involved. Upon request and subject to the review and editing of the Company's Legal Counsel, the appointed Union representative and the affected employee may review the final investigation report. If the Company decides to take disciplinary action against the Union member involved, a copy of the report will be given to the Union, and the employee at that time.
