Case Records. Contractor shall maintain current information, including case log notes, on individual contract cases. To the extent ethically possible, records shall be kept in a manner to be available on request for inspection by PDSC, or PDSC's designee or agent.

Case Records. Contractor shall preserve all case documents, notes, files, physical evidence or any other items created or received in the course of the representation of a client in an orderly and organized manner such that it can readily be made available to successor counsel, if one is appointed or retained. Case Records. 9 5.1 CONTRACTOR shall maintain a current and complete electronic case 10 record on COUNTY’s computer information system for each participant referred. 11 ADMINISTRATOR will provide sufficient training regarding use and maintenance 12 of electronic case records on the computer information system, track cases, 13 generate reports, etc., to CONTRACTOR’s staff. CONTRACTOR shall conduct 14 future training for its staff and also provide this training to any partner 15 agencies that will have on-line access.
16 5.2 CONTRACTOR may also be required to maintain a physical case record 17 (hard copy), as required by ADMINISTRATOR. The content of the physical case 18 records shall be in a format approved by ADMINISTRATOR and shall be uniform 19 for each subcontractor. The physical case record shall contain any 20 documentation not included in the electronic case record, as requested by
Case Records. 5 CONTRACTOR shall maintain case records on each referral which shall 6 include, but not be limited to the following:
8 6.2 9 CONTRACTOR. Psychosocial Assessments and any related documentation; Authorization to release information between ADMINISTRATOR, and
Case Records. Counsel shall compile and maintain appropriate case records for each person whom Counsel is appointed to represent. Counsel shall maintain such case records in their entirety for a period of no less than seven (7) years from the date on which the case or matter is fully and finally concluded or for any other time period specified under applicable court rule or statute, whichever date/event occurs last.
Case Records. You agree that none of the case records (tests, interview notes, observations notes, and notes from others) will be given to either parent (party) until the evaluation is finished. Then, the lawyers of the parents (parties) may look through and ask for copies of these records. The tests, though, cannot be copied. The tests can only be given to another psychologist. Copying fees for all case records are 96 cents per page for the first 30 pages and 73 cents per page after that. You also agree to pay $25 for a secretary to copy the records. In addition, you will pay a $40 per hour case review fee. This covers the cost of the time Xx. Xxxx spends with you while you review the file. Xx. Xxxx will not discuss your case with you during your case review appointment. At the end of the evaluation, your lawyer and Guardian ad litem will be given a written report with recommendations. Xx. Xxxx’x findings and opinions are important to you. Please do not ask Xx. Xxxx to give you any information until Xx. Xxxx has completed the evaluation process. Xx. Xxxx must look at the tests and all the information necessary to complete a thorough evaluation. You will get to hear and respond to the results of the evaluation about you before the report is finished. Then, your lawyer and the Guardian ad litem will receive the report. Xx. Xxxx’x statements, report, and other actions might harm your court case.

ADMINISTRATOR will 11 provide CONTRACTOR staff with initial training in use of CalWIN or other 12 COUNTY data systems regarding use and maintenance of electronic case records 13 on CalWIN.
