PROGRAM PROCEDURES. NGB uses a CA as the means of providing financial assistance and other support to recipients for the operation of the NGB program in the recipient’s jurisdiction, except for financial assistance and support provided under separate authority (e.g., military and technician pay and the military supply system). Recipients enter into a Master Cooperative Agreement (MCA) with the NGB. Generally, an MCA consists of two parts: (1) the Agreement and (2) the Appendices. The Agreement includes the standard terms and conditions applicable to all Appendices. The Appendices contain the terms and conditions, policy, administrative procedures, scope of work, authorized and unauthorized activities/charges, budget information, funding limitations, and Agreement particulars applicable to that functional area (e.g., Real Property Operations and Maintenance, Security Guard activities, etc.). Funding for the CA is identified in each of the Appendices to the MCA. The total sum of Federal reimbursements to the recipient for an MCA Appendix may not exceed the approved funding limits identified in the Funding Limitation section of the Appendix. . NGB, a joint Bureau of the Department of the Army (DA) and the Department of the Air Force (AF), enters into a CA with a State when NGB transfers something of value, through funding or otherwise, to the State to support the State ARNG FP and ANG FOMA instead of acquiring (by purchase, lease, or barter) property or services for the direct benefit or use of the ARNG or ANG and substantial involvement is expected between the NGB and the State when carrying out the activity contemplated in the agreement (NGR 5-1/XXXX 63-101, chapter 1-1). Generally, a CA consists of two parts, the agreement and appendices (Master Cooperative Agreement (MCA) and as many Appendices as apply to that State). Policies and procedures to be followed for cooperative agreements with States are contained in National Guard Grants and Cooperative Agreements NGR 5-1/XXXX 63-101. PROGRAM PROCEDURES. The TB Program supports county tuberculosis control endeavors by: • funding health departments partially to support staff and/or operating costs; • supplying pharmaceuticals to health departments for TB testing and for TB treatment at no cost to people being treated curatively or preventively; • providing medical and nursing consultation and case management assistance; • providing technical assistance and training for the surveillance system; • offering health education programs and training; and, • performing program evaluation, and data quality assurance. All health departments provide direct TB services involving: • case management including provision and monitoring of medication; • epidemiologic evaluations; measures to interrupt and prevent transmission, including provision and monitoring of preventive treatment; and, • communication, coordination, education and promoting TB awareness among all health care providers in their jurisdiction. Federal funds provided to local health departments are obtained through a competitive Federal cooperative agreement administered by CDC. The number of counties funded and the amount of funding are approved annually by CDC. In regard to State funds, the funds are allocated to local health departments based upon a needs-based formula. Additionally, non-formula funds are utilized to support one-time costs such as operating expenses, equipment, etc. The TB Program is administered by the TB Program, Communicable Disease Branch, Epidemiology Section, Division of Public Health, Department of Health and Human Services, 0000 Xxxxx Xxxxxx, Xxxxx 000, Xxxxxxx, XX 00000, (000) 000-0000. In addition to federal statutory requirements, the State has the authority to issue rules consistent with State statutes. These rules are incorporated by reference into this compliance supplement. Provider agrees to participate and cooperate in any internal and external quality assessment review, utilization management and grievance procedures established by Company, DHH and/or its designee. Provider further agrees to comply with all limitations on Provider marketing and any corrective action plan initiated by Company and/or required by DHH. Provider shall monitor and report the quality of services provided under this Agreement and, if necessary, initiate a plan of correction to improve the quality of care in accordance with that level of care which is recognized as acceptable professional practice in Provider’s community and/or the standards established by Company, DHH and/or its designee.
