Check-Off Form. During the life of this Agreement and in accordance with the terms of the form of authorization of check-off of dues or service fee, as provided by the Union, subject to advance approval of the Board, the Board agrees to deduct Union membership dues or service fees levied in accordance with the constitution and by-laws of the Union from the pay of each employee who executes or has executed an authorization for check-off of dues or service fees form.
Check-Off Form. 26.01 The form of check-off authorization to be supplied by the Company as follows: I, an employee of Le Meridien Xxxx Xxxxxx Hotel, individually and voluntarily hereby authorize and direct the aforesaid Company to deduct my Union dues, as presently approved in the Union by-laws, from my earnings accumulated to my credit on the first pay period of each calendar month, and remit direct to the UNITE HERE Local 75. It is understood that the amount of dues is determined by the Local Union, or by Union International Convention and can be changed by the Local Union or by Union International Convention at any time to comply with such Local or Convention decision regarding same, and this authorization check-off will hold harmless both the Company and the Local Union if so directed. I agree that Le Meridien Xxxx Xxxxxx Hotel shall be save harmless for all deduction and payment so made. I also authorize the Employer to deduct from second pay period, after employment, my initiation fee. The Union will supply the Employer with a letter from their Executive Board advising them of the amounts to be deducted for the initiation fee. The Union agrees to defend and hold the Employer completely harmless against all claims and demands, should any person at any time contend or claim that the Employer has acted wrongfully or illegally in making the aforementioned deduction for Union dues.
Check-Off Form. Employees must submit the following form of authorization thirty (30) days prior to deduction from a payroll check. During the life of this Agreement, the Employer agrees to deduct Union membership dues or service fee from the pay of each employee who executes or who has executed the Authorization for Check-off form and filed the same with the Employer. The Employer shall be entitled to rely solely on the written notice of the Executive Director of the Capitol City Lodge No. 141, Fraternal Order of Police as to the amount to be deducted from the employee's wages and the employees from whom such deductions are to be made, provided that authorization shall be given to the payroll department at least three (3) weeks prior to the pay day of which deductions are to be made and provided deductions for check-off shall not supersede any legally required deductions or deductions authorized prior to April 18, 1974, and the Employer shall not be required to make any check-off if the employee's pay is not sufficient to cover the dues in any pay period.

During the life of the Agreement and in accordance with the terms of the Form of Authorization of Check-Off of dues hereinafter set forth, and to the extent the Laws of the State of Michigan permit, FSU agrees to deduct Union membership dues levied in accordance with the constitution and by-laws of the Union from the pay of each employee who executes or has executed an authorization for Check-Off of Dues form which is consistent with the terms of the Agreement and does not impose restrictions of free choice upon employees.
Check-Off Form. During the life of this Agreement and in accordance with the terms of the form of Authorization of Check-Off of Dues hereinafter set forth and to the extent the laws of the State of Michigan permit, the Employer agrees to deduct Union membership dues levied in accordance with the Constitution and By-Laws of the Union from the pay of each employee who individually executes or has executed an authorization form authorizing such deductions.

During the life of this Agreement and in accordance with the terms I of the Form of Authorization for Check-Off hereinafter set forth, the City agrees to deduct Union membership dues or service fees levied in accordance with the Constitution I and By-laws of the Union from the pay of each employee who executes or has executed the following "Authorization of Check-Off' Form.
Check-Off Form. During the life of this Agreement and in accordance with the terms of the form of authorization of Check-off of Dues/Representation Fees hereinafter set forth, the Employer agrees to deduct only LEA membership dues/Representation Fees from the pay of each employee who executes or who has executed the following Authorization for Check- off of Dues/Representation Fee Form and filed the same with the employer. The employer shall not be required to make any check-off for XXX xxxx/Representation Fees if the employee's pay is not sufficient to cover the dues in any pay period.
Check-Off Form. Teachers must submit the following form of authorization thirty (30) days prior to deduction from a payroll check: Date: AUTHORIZATION FOR DEDUCTION OF XXXXXXXX FEDERATION OF TEACHERS MEMBERSHIP DUES AND SERVICE FEES I hereby authorize the Xxxxxxxx School District to deduct the sum of $ from ten (10) consecutive paychecks, which sum is to be paid to the treasurer of the Xxxxxxxx Federation of Teachers. CHECK ONE: This amount represents my membership dues to the Xxxxxxxx Federation of Teachers. This amount represents a service fee to the Xxxxxxxx Federation of Teachers. This authorization is to continue in full force and effect until such time as my employment is terminated, a change of assignment removes me from the bargaining unit, or until the authorization is revoked by written notice from me. I further authorize the Federation treasurer to annually change the amount and number of deductions. Signed DUES PAID TO THE XXXXXXXX FEDERATION OF TEACHERS MAY NOT BE DEDUCTIBLE FOR FEDERAL INCOME TAX PURPOSES; HOWEVER, UNDER LIMITED CIRCUMSTANCES, DUES MAY QUALIFY AS A BUSINESS EXPENSE.
