Chronic diseases. The insurance covers the examination and treatment of chronic diseases and disorders that arise during the insurance period, for up to 6 months from the date of diagnosis, if we consider that treatment will cause a significant and lasting improvement to the condition. Chronic diseases and disorders that have occurred and/ or are diagnosed before the insurance period, are not covered. We define chronic diseases as diseases, conditions and disorders that our doctors consider are persistent and cannot be cured, and where there is no curative treat- ment. The healthcare team always offers assistance for all chronic diseases with counselling, guidance on the public health system’s treatment offers, patient rights, waiting times, examination and treatment guarantees and help with booking appointments in the public healthcare system.

Chronic diseases. The Parties will promote awareness and prevention of chronic diseases that disproportionately affect Black Georgians.
Chronic diseases. 30. NCCC shall develop and implement a written chronic care disease management program. As part of this program, NCCC shall maintain an updated chronic disease registry of inmates suffering from chronic illnesses, including, but not limited to, the following conditions: asthma, cardiac disease, elevated lipids, diabetes, HIV infection, hypertension, and seizure disorder.
Chronic diseases. Responding organizations or academic institutions must have a minimum of five years of performing relevant experience evaluating chronic disease programs and/or policies. • Programs or policies must include one of the following health issues: obesity prevention or healthy eating, exercise or active living, tobacco prevention, diabetes prevention, cancer prevention, and/or heart disease prevention. • Relevant activities include: developing and completing process and/or outcome evaluations of federal or state funded chronic disease programs/policies, collecting and analyzing quantitative data related to chronic disease programs/policies, and authoring reports or presenting findings related to chronic disease evaluations. • The principle investigator must have a doctoral level education and possess a minimum of two years of experience working in the field of chronic disease programming/policies. • Respondent must have knowledge of secondary data sources that can support chronic disease evaluation projects. • Respondents must have evaluators who have published cited evaluation studies within the field of chronic disease and should include up to three of these publications in the submission. • The respondent should also include a minimum of two letters of support documenting that the principle investigator has the relevant experience. One of these letters of support can be from the institution where the investigator is employed. • Selected respondents cannot evaluate programs for which their institution receives funds from CDPH to perform.
Chronic diseases. On the condition that the first diagnosis is made after becoming insured, chronic blood pressure disorders, cardiovascular diseases, cancer, diabetes, asthma, migraine, rheumatic diseases, renal diseases that require dialysis, Alzheimer, multiple sclerosis and Xxxxxxxxx disease treatment expenses are paid within the policy special and general conditions without a waiting period.

  • Communicable Diseases (a) The Parties to this Agreement share a desire to prevent acquisition and transmission where employees may come into contact with a person and/or possessions of a person with a communicable disease.

  • Diagnosis For a condition to be considered a covered illness or disorder, copies of laboratory tests results, X-rays, or any other report or result of clinical examinations on which the diagnosis was based, are required as part of the positive diagnosis by a physician.

  • Screening 3.13.1 Refuse containers located outside the building shall be fully screened from adjacent properties and from streets by means of opaque fencing or masonry walls with suitable landscaping.

  • Infectious Diseases The Employer and the Union desire to arrest the spread of infectious diseases in the nursing home. To achieve this objective, the Joint Health and Safety Committee may review and offer input into infection control programs and protocols including surveillance, outbreak control, isolation, precautions, worker education and training, and personal protective equipment. The Employer will provide training and ongoing education in communicable disease recognition, use of personal protective equipment, decontamination of equipment, and disposal of hazardous waste.

  • Communicable Disease Bodily injury" or "property damage" which arises out of the transmission of a communi- cable disease by an "insured";

  • Human Leukocyte Antigen Testing This plan covers human leukocyte antigen testing for A, B, and DR antigens once per member per lifetime to establish a member’s bone marrow transplantation donor suitability in accordance with R.I. General Law §27-20-36. The testing must be performed in a facility that is: • accredited by the American Association of Blood Banks or its successors; and • licensed under the Clinical Laboratory Improvement Act as it may be amended from time to time. At the time of testing, the person being tested must complete and sign an informed consent form that also authorizes the results of the test to be used for participation in the National Marrow Donor program.

  • Infectious Disease Where an employee produces documentary evidence that:

  • Rhytidectomy Scar revision, regardless of symptoms. • Sclerotherapy for spider veins. • Skin tag removal. • Subcutaneous injection of filling material. • Suction assisted Lipectomy. • Tattooing or tattoo removal except tattooing of the nipple/areola related to a mastectomy. • Treatment of vitiligo. • Standby services of an assistant surgeon or anesthesiologist. • Orthodontic services related to orthognathic surgery. • Cosmetic procedures when performed primarily: o to refine or reshape body structures or dental structures that are not functionally impaired; o to improve appearance or self-esteem; or o for other psychological, psychiatric or emotional reasons. • Drugs, biological products, hospital charges, pathology, radiology fees and charges for surgeons, assistant surgeons, attending physicians and any other incidental services, which are related to cosmetic surgery.
