City Contributions. The City’s contribution for retiree medical is established as the same dollar contribution paid for active employees at the single rate of the lowest cost HMO early retiree plan.
City Contributions. The City agrees to contribute to the State Police Pension and Retirement System at the statutory rate for each officer's salary paid by the City.
City Contributions. 2.3.1 Effective July 1, 2017 through June 30, 2018.
City Contributions. (1) Effective the pay period beginning December 21, 2019, for full-time employees enrolled in a City-sponsored medical plan for employee only, the City contribution shall be $829.46 per month. Effective the pay period beginning December 18, 2021, for full-time employees enrolled in a City-sponsored medical plan for employee only, the City contribution shall be $838.00 per month.
(2) Effective the pay period beginning December 21, 2019, for full-time employees enrolled in a City-sponsored medical plan for employee plus one (1) dependent, the City contribution shall be $1,318.38 per month. Effective the pay period beginning December 18, 2021, for full-time employees enrolled in a City-sponsored medical plan for employee plus one (1) dependent, the City contribution shall be $1,333.00 per month.
(3) Effective the pay period beginning December 21, 2019, for full-time employees enrolled in a City-sponsored medical plan for employee plus two (2) or more dependents, the City contribution shall be $1,758.00 per month. Effective the pay period beginning December 18, 2021, for full-time employees enrolled in a City-sponsored medical plan for employee plus two (2) or more dependents, the City contribution shall be $1,777.00 per month.
(4) Full-time employees not enrolled in a City sponsored medical plan shall receive up to $747.00 per month to purchase City-sponsored dental and vision coverage.
City Contributions. The city shall make such contributions as necessary to meet the requirements of the plan. Effective May 1, 1994, the city shall pick up the employee contributions by a reduction in current earnings. Such picked up contributions shall be treated as employer contributions in determining tax treatment under the Internal Revenue Code; however, the contributions so picked up shall be included in earnings for purposes of the plan. Participant contributions which are picked up shall be treated, for all purposes of this plan, in the same manner as participant contributions made prior to May 1, 1994.
City Contributions. 48.2.3.1 Effective July 1, 2017 through June 30, 2018.
48.2.3.1.1 Self-Insured Medical Plan effective Plan Year July 1, 2017 through June 30, 2018. Effective in Benefit Plan Year July 1, 2017 through June 30, 2018, the City shall contribute ninety-five percent (95%) of the basic medical, vision and dental rates adopted by City Council for the one party, two-party or family enrollees (whichever applies) for each full- time regular employee who elects the Self-Insured Medical Plan (benefits listed in Articles 48.1.4 and 48.1.5). The City reserves the right to expand family tier descriptions if it is in the best interest of the employee enrollee and it has been accepted by the PPA and subsequently approved by City Council.
48.2.3.1.2 Insured Kaiser Medical and Vision and Choice of Dental Plan effective Plan Years July 1, 2017 through June 30, 2018. Effective in Benefit Plan Years July 1, 2017 through June 30, 2018, the City shall contribute for employees enrolled in the Insured Xxxxxx Medical and Vision Plan and choice of dental plan for each full-time regular employee, one hundred percent (100%) of the City Contribution under article 48.2.1 for the one party, two- party or family enrollees (whichever applies).
48.2.3.2 Effective July 1, 2017 and for subsequent plan years.
48.2.3.2.1 High Deductible Health Plan (HDHP) effective Plan Year July 1, 2017. Beginning with Benefit Plan Year July 1, 2017, and effective in subsequent plan years, the City shall contribute one hundred percent (100%) of the medical and vision rates and ninety-five percent (95%) of the dental rates adopted by the City Council and elected by the employee for the one party, two party or family enrollees (whichever applies) or any variation of the tiered rates accepted by the PPA and subsequently approved by City Council, for full-time regular employees who elect the HDHP.
48.2.3.3 Effective July 1, 2018 and for subsequent plan years.
48.2.3.3.1 Beginning with Benefit Plan year July 1, 2018, and effective in subsequent plan years, the City shall contribute ninety-five percent (95%) of the basic medical, vision and dental rates adopted by the City Council and elected by the employee for the one party, two party or family enrollees (whichever applies), or any variation of the tiered rates accepted by the PPA and subsequently approved by City Council, for each full-time regular employee who elects the Self-Insured Medical Plan (benefits listed in Articles 48.1.4 and 48.1.5); provided the employ...
City Contributions. City agrees that the funds committed by County pursuant to this agreement are intended by County to be, in addition to and not in substitution or replacement of, any monies previously committed or agreed to be committed by City to the project. City staff will participate and contribute to the Project planning and design team. In addition, City shall, at its sole and separate expense, maintain and carry forward the work product upon completion of the Project. City may also pursue additional funding for all of the components of the Project through applications for grants.
City Contributions. Subject to approval in its annual budget, City will make an annual operating subsidy to Overture, through 201, subject to adjustments calculated using the Consumer Price Index methodology as described in the State’s Expenditure Restraint Program (“ERP”). The initial subsidy shall be One Million Four Hundred Ten Thousand Dollars ($1,410,000) plus an amount equal to the rent provided in Paragraph 3, above, for calendar year 2012. The subsidy and rent payment will be prorated for the first year to recognize the difference in the City’s fiscal year (calendar) and that of MCAD and 201 (July 1- June 30). The City Contribution will be adjusted annually using ERP beginning July 1, 2012. The City’s subsidy herein is contingent upon 201 generating funds by other than ticket sales (“201 Contributions”) in an amount equal to or greater than the City’s subsidy (net of rent payments) in every fiscal year of 201 commencing on July 2012. The 201 Contributions shall be measured on a rolling 3-year average. The City’s subsidy may be reduced on a dollar for dollar basis if 201 fails to generate sufficient 201 Contributions.
City Contributions. The City for the term of this agreement and only while the City participates in the PERS Plan, shall make the following contributions towards the cost of medical insurance for active employees:

2. Self-Insured Medical Plan or Kaiser Plan effective Plan Year July 1, 2020.
a. Beginning with Benefit Plan year July 1, 2020, and effective in subsequent plan years, the City shall contribute ninety-five percent (95%) of the medical, vision and dental rates adopted by the City Council and elected by the employee for the one party, two party or family enrollees (whichever applies), or any variation of the tiered rates recommended by the LMBC and subsequently approved by City Council, for each full-time regular employee who elects the Self-Insured Medical Plan or the Kaiser Plan; provided that the employee has received a preventive health care examination within the prior two (2) calendar years.
b. Beginning with Benefit Plan year July 1, 2020, and effective in subsequent plan years, the City shall contribute ninety percent (90%) of the medical, vision and dental rates adopted by the City Council for each full-time regular employee who has elected the Self-Insured Medical Plan or the Kaiser Plan and who has not received a preventive health care examination within the prior two (2) calendar years.
c. Beginning with Benefit Plan year July 1, 2020, and effective in subsequent plan years, newly hired full-time regular employees who elect the Self-Insured Medical Plan or the Kaiser Plan will have one
(1) full calendar year to receive a preventive health examination to retain the City’s ninety-five percent (95%) contribution in the subsequent plan year. The City shall contribute ninety percent (90%) of the medical, vision and dental rates adopted by the City Council in the subsequent plan year for each newly hired full-time regular employee who does not receive a preventive health examination within the first calendar year of service.
