Dental Care Program. 1. It is agreed that the State shall continue the Dental Care Program during the period of this Agreement to the extent it is established and/or modified by the State Health Benefits Design Committee, in accordance with P.L. 2011, c. 78, effective January 1, 2012 (and each year thereafter). Through June 30, 2023, active eligible employees are able to participate in the Dental Care Program as described in the parties’ July 1, 2007 – June 30, 2011 collective negotiations agreement. Pursuant to P.L. 2011, c. 78, the State Health Benefits Plan Design Committee has the sole discretion to set the amounts for maximums, co-pays, deductibles, and other such participant costs for all plans in the program and has the sole discretion to determine the plan design, plan offerings and coverage levels under the program. 2. Participation in the Program shall be voluntary with a condition of participation being that each participating employee authorize a biweekly salary deduction not to exceed 50% of the cost of the type of coverage elected; e.g., individual employee only, husband and wife, parent and child or family coverage. 3. Each employee shall be provided with a brochure describing the details of the Program and enrollment information and the required forms. 4. Participating employees shall be provided with an identification card to be utilized when covered dental care is required.

Dental Care Program. The City will contribute 100% of the premium for the employee-only dental care coverage as provided by the City of Pasadena. For employees who cover a dependent, the City will contribute up to an additional $75.84 per month for the dental plan premium.

For employees who cover one or more dependents, the City shall contribute up to an additional $85.40 per month above the employee only premium. Dental Care Program. The State administered Dental Care Program shall be the provider of dental benefits during the period of this Agreement. Such benefits shall be provided to all eligible employees and their eligible dependents.

The Program shall be administered by the State and shall provide benefits to all eligible bargaining unit members and their eligible dependents. Participation in the Program shall be voluntary with a condition of participation being that each participating employee authorize a biweekly salary deduction not to exceed fifty (50) percent of the cost of the type of coverage elected: e.g., individual employee only, husband and wife, parent and child or family coverage. There shall be only one opportunity for each eligible bargaining unit member to enroll each year and elect the type of coverage desired and, once enrolled, continued participation shall be mandatory for that year. An optional Group Dental Program which will provide services through specific dental clinics will be made available to employees in this unit when legally and administratively feasible. Participation in this program shall be voluntary with a condition that each participating employee authorize a biweekly salary deduction not to exceed fifty (50%) percent of the cost of the coverage for a one-year period. When the new program is available, the employees will be able to enroll in only one of the two programs or in no program at all.
Dental Care Program. During the term of this Agreement, full time employees and their eligible dependents shall continue to be eligible to participate in the Dental Care Program established by the State. 1. Full-time employees and eligible dependents shall be eligible for the State administered Dental Care Program which shall be continued during the life of this Agreement. 2.
