EMPLOYEE HEALTH AND DENTAL BENEFITS. The Executive shall be entitled to participate in the Company’s employee health and dental benefits plan during the Term, as such plan may be in effect from time to time.
EMPLOYEE HEALTH AND DENTAL BENEFITS. 2 (1) All employees will be covered by the Milwaukee County Health Insurance 3 Plan, as outlined in the annual adopted Milwaukee County Budget and 4 Chapter 17 of the General Ordinances of the County of Milwaukee.
5 (2) Milwaukee County Health Insurance Plan shall pay a monthly amount 6 toward the monthly cost of health insurance as described below:
(a) Effective the month following the execution date of the 2013 labor 8 agreement employees shall pay one hundred dollars ($100.00) per 9 month toward the monthly cost of an Employee only plan. 10 (b) Effective the month following the execution date of the 2013 labor 11 agreement employees shall pay one hundred twenty-five dollars 12 ($125.00) per month toward the cost of an Employee + 13 Child/Children plan. 14 (c) Effective the month following the execution date of the 2013 labor 15 agreement employees shall pay two hundred dollars ($200.00) per 16 month toward the cost of an Employee + Spouse/Partner plan. 17 (d) Effective the month following the execution date of the 2013 labor 18 agreement employees shall pay two hundred twenty-five dollars 19 ($225.00) per month toward the cost of an Employee + Family plan.
20 (3) In the event an employee who has exhausted accumulated sick leave is 21 placed on leave of absence without pay status on account of illness, the 22 County shall continue to pay the monthly cost or premium for the Health 23 Plan chosen by the employee and in force at the time leave of absence 24 without pay status is requested, if any, less the employee contribution 25 during such leave for a period not to exceed one (1) year. The 1-year 26 period of limitation shall begin to run on the first day of the month 27 following that during which the leave of absence begins. An employee 28 must return to work for a period of sixty (60) calendar days with no 29 absences for illness related to the original illness in order for a new 1-year 30 limitation period to commence.
1 (4) Coverage of enrolled employees shall be in accordance with the monthly 2 enrollment cycle administered by the County.
3 (5) Eligible employees may continue to apply to change their health plan to one 4 of the options available to employees on an annual basis. This open 5 enrollment shall be held at a date to be determined by the County and 6 announced at least forty-five (45) days in advance.

EMPLOYEE HEALTH AND DENTAL BENEFITS. 10 (1) Health and Dental Benefits shall be provided for in accordance with the 11 terms and conditions of the current Plan Document and the Group 12 Administrative Agreement for the Milwaukee County Health Insurance 13 Plan or under the terms and conditions of the insurance contracts of those 14 Managed Care Organizations (Health Maintenance Organizations or HMO) 15 approved by the County. 16 (2) Eligible employees may choose health benefits for themselves and their 17 dependents under a Preferred Provider Organization (County Health Plan or 18 PPO) or HMO approved by the County.
EMPLOYEE HEALTH AND DENTAL BENEFITS. 16 Section 2.06 is effective January 1, 2005 through December 31, 2005. Section 2.061 shall replace 17 Section 2.06 in its entirety on January 1, 2006.
(1) Health and Dental Benefits shall be provided for in accordance with the terms and 19 conditions of the current Plan Document and the Group Administrative Agreement 20 for the Milwaukee County Health Insurance Plan or under the terms and conditions 21 of the insurance contracts of those Health Maintenance Organizations approved by 22 Milwaukee County.
(2) Eligible employees may choose health benefits for themselves and their dependents 24 under a fee-for-service plan or Health Maintenance Organization approved by the 25 County.
EMPLOYEE HEALTH AND DENTAL BENEFITS. 30 (1) Health and Dental Benefits shall be provided for in accordance with the terms and 31 conditions of the current Plan Document and the Group Administrative 32 Agreement for the Milwaukee County Health Insurance Plan or under the terms 1 and conditions of the insurance contracts of those Managed Care Organizations 2 (Health Maintenance Organizations or HMO) approved by the County.
3 (2) Eligible employees may choose health benefits for themselves and their 4 dependents under a Preferred Provider Organization (County Health Plan or PPO) 5 or HMO approved by the County.
6 (3) All eligible employees enrolled in the PPO or HMO shall pay a monthly amount 7 toward the monthly cost of health insurance as described below:
(a) Effective January of 2009, employees enrolled in the PPO shall pay 9 seventy-five dollars ($75.00) per month toward the monthly cost of a 10 single plan and one hundred fifty dollars ($150.00) per month toward the 11 monthly cost of a family plan.
EMPLOYEE HEALTH AND DENTAL BENEFITS. 1 (1) All employees shall be covered by the Milwaukee County Health Insurance Plan.
2 (2) Employees covered by the Milwaukee County Health Insurance Plan shall pay a 3 monthly amount toward the monthly cost of health insurance as described below:
(a) Freeze employee contribution at the 2014 level for 2015, 2016 and 2017.
(b) Employees shall pay one hundred forty dollars ($140.00) per month toward 6 the monthly cost of an Employee Only plan effective January 1, 2018.

Section 3.111 16 shall replace Section 3.11 in its entirety on January 1, 2006. 17 18 (1) Health and Dental Benefits shall be provided for in accordance with the 19 terms and conditions of the current Plan Document and the Group 20 Administrative Agreement for the Milwaukee County Health Insurance 21 Plan or under the terms and conditions of the insurance contracts of those 22 Health Maintenance Organizations approved by Milwaukee County.

Permanent full time employees, permanent part time employees whose regular work schedules are at least 20 hours per week, and temporary employees with 1040 hours of service, within a consecutive 12-month period of time, are eligible to be enrolled in the Health Service System of the City and County of San Francisco.
EMPLOYEE HEALTH AND DENTAL BENEFITS. 7 (1) Effective the month following the implementation date of the successor 8 agreement to the 2012-2014 labor agreement, employees enrolled in the 9 Milwaukee County Health Insurance Plan shall pay a monthly amount toward 10 the monthly cost of health insurance as follows:
(a) Employees shall pay one hundred thirty dollars ($130.00) per month toward 12 the cost of an Employee only plan. If employees successfully participate in 13 the County sponsored Wellness Incentive, employees shall pay ninety one 14 dollars ($91.00) per month toward the cost of an Employee only plan.
