Fire Fighter Paramedics. Lateral candidates will be permitted to submit applications of interest when there is an insufficient number of qualified applicants for the Fire Fighter Paramedic position from the NEKC Consortium at the conclusion of the testing process, or an unexpected vacancy occurs after paramedic school begins precluding the use of the current established list. The lateral candidate pre-testing qualifications will include Paramedics who have successfully completed the University of Washington paramedic training program and are currently certified as ALS providers in Washington State. The selection process of lateral candidates will include the Assessment Center, Interview #1, and Final Interview components of the testing process previously laid out for Fire Fighter Paramedic in Article 12, Section B. When a Fire Fighter Paramedic lateral hiring list is established it will have an expiration date on January 1st of the following year.

Fire Fighter Paramedics. 18.1 All new entry level employees hired on and after the ratification date of the agreement succeeding the agreement which expired June 30, 1997 must obtain certification by the State of Michigan as Basic-EMTs at the City’s expense before the end of their probationary period and keep this certification current. Employees hired after 8/26/86 but before the above cited ratification date may be required to obtain certification by the State of Michigan as Basic-EMTs. Any employee hired after 8/26/86 may be required to be trained at the City's expense to the Advanced-EMT level (Paramedic) and be certified by the State of Michigan and perform as a paramedic fire fighter for a minimum period of five years subject to the following paragraph. After serving five years as a paramedic, a paramedic may notify the Fire Chief in writing that he or she desires to cease the paramedic assignment. If there is an employee who is already trained as a paramedic and who is available to replace the paramedic, the paramedic may cease the paramedic assignment. If a replacement is not trained and available, the paramedic must remain in the paramedic assignment until a replacement is trained and available or until the paramedic has served an additional year as a paramedic, whichever is sooner. Any employee hired after January 21, 2005, who is not already certified by the State of Michigan as an Advanced-EMT may be required to be trained at the City’s expense to the Advanced-EMT level (Paramedic) and be certified by the State of Michigan and perform as a paramedic fire fighter for a minimum period of ten years. A paramedic fire fighter who promotes to lieutenant before performing ten years as a paramedic must serve the balance of ten years as a paramedic lieutenant. A paramedic fire fighter who becomes a drive-engineer, mechanic or inspector before performing ten years as a paramedic must maintain their paramedic license and serve the balance of ten years as a paramedic if the member returns to the position of fire fighter. Fire Fighter Paramedics. In the event there is an insufficient number of fire fighter paramedic qualified applicants from the NEKC Consortium at the closing date of the application period, lateral candidates will be permitted to submit applications of interest. The lateral candidate qualifications would include Paramedics who have successfully completed the University of Washington paramedic training and are currently certified to practice ALS medicine within King County. For the purpose of hiring non-Redmond Fire Department employees who are selected as a result of the assignment process outlined in this Article XII, the Employer and Union shall agree on a Special Registry for submittal to the Civil Service Commission for final processing and hiring of the individuals selected. In the event there is an immediate need to hire Fire Fighters to be assigned as Paramedic(s) due to unexpected attrition and no current Fire Fighter with Paramedic Qualifications eligibility list exists, the Employer and Union will agree on a Special Registry for submittal to the Civil Service Commission for final processing, and the individuals hired form the Special Registry shall be assigned as Fire Fighter Paramedics. Fire Fighter Paramedics hired from the NEKC Consortium Department (NEKC Department) shall be granted seniority for all purposes based on their date of hire at the NEKC Department.

This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia. Preauthorization may be required for these services.

  • Telemedicine Services This plan covers clinically appropriate telemedicine services when the service is provided via remote access through an on-line service or other interactive audio and video telecommunications system in accordance with R.I. General Law § 27-81-1. Clinically appropriate telemedicine services may be obtained from a network provider, and from our designated telemedicine service provider. When you seek telemedicine services from our designated telemedicine service provider, the amount you pay is listed in the Summary of Medical Benefits. When you receive a covered healthcare service from a network provider via remote access, the amount you pay depends on the covered healthcare service you receive, as indicated in the Summary of Medical Benefits.   • Surgery Services and Mastectomy Related Treatment This plan provides benefits for mastectomy surgery and mastectomy-related services in accordance with the Women’s Health and Cancer Rights Act of 1998 and Rhode Island General Law 27-20-29 et seq. For the member receiving mastectomy-related benefits, coverage will be provided in a manner determined in consultation with the attending physician, physician assistant, or an advance practice registered nurse and the patient, for: • all stages of reconstruction of the breast on which the mastectomy was performed; • surgery and reconstruction of the other breast to produce a symmetrical appearance; • prostheses; and • treatment of physical complications at all stages of the mastectomy, including lymphedema.   • Outpatient Dental Anesthesia Services This plan covers anesthesia services received in connection with a dental service when provided in a hospital or freestanding ambulatory surgical center and: • the use of this is medically necessary; and • the setting in which the service is received is determined to be appropriate. This plan also covers facility fees associated with these services.
