Grievance Contents. All grievances shall be filed in writing on a form provided by the Union and shall contain the following information:
1. Date and time grievance occurred.
2. Description of incident giving rise to the grievance.
3. Articles and sections of the agreement involved.
4. Relief requested.
5. Signature of the employee.
Grievance Contents. All grievances should be filed using the grievance form as presented at Appendix A and provide the following information:
A. grieved employee's name and signature;
B. grieved employee's position;

a. A summary of the problem;
b. A brief statement of the facts involved;
c. The provisions of this Agreement that the grievant alleges were violated, or, in a non- contractual grievance, the policy or consistent practices alleged to have been violated;
d. The remedy or solution sought.
Grievance Contents. All grievances (HR Form-23) must contain the following information to be considered and must be filed using the grievance form mutually agreed upon by both parties. The grievance will stipulate the exact article and section of this agreement to which the grievance pertains and the precise remedy for the grievance.
1. Aggrieved employee’s name and signature.
2. Aggrieved employee’s classification.
3. Date grievance was first discussed with his supervisor.
4. Date grievance was filed in writing.
5. Date and time grievance occurred.
6. The location where the grievance occurred.
7. A description of the incident(s) giving rise to the grievance.
8. Specific articles and sections of the Agreement.
9. Xxxxxxx remedy to resolve the grievance.
Grievance Contents. All grievances shall include the name and position of the grievant; the identity of the provisions of this Agreement involved in the grievance; the time and place where the alleged events or conditions giving rise to grievance took place; the identity of the party responsible for causing the said grievance, if known to the grievant; and a general statement of the nature of the grievance and the redress sought by the grievant.
Grievance Contents. The written grievance will be submitted on the grievance form provided by the Union and will contain the following information:
1. Aggrieved employee’s name;
2. Aggrieved employee’s classification;
3. Name of the employee’s immediate supervisor;
4. Date and time of the incident giving rise to the grievance;
5. Date and Signature of the grievant and the Union Xxxxxxx;
6. Date grievance was filed in writing at Step 1;
7. A statement as to the specific articles and sections of the agreement violated;
8. A brief statement of the facts involved in the grievance; and
9. The remedy requested to resolve the grievance.

5. Signature of the employee/union representative. Any grievance that does not contain the above listed information shall be considered defective and shall not be eligible for processing through the grievance procedure. Within the grievance procedure, the grievant/union representative shall be required to prepare copies of the grievance and submit such documentation at each step of the appeal.

Except at the informal step, all grievances shall include the name and position of the aggrieved party; the identity of the provision(s) of this Agreement involved in the grievance; the time and place where the alleged events or conditions constituting the grievance took place; the identity of the party responsible for causing the said grievance, if known to the aggrieved party; and a general statement of the nature of the grievance and the redress sought by the aggrieved party.
Grievance Contents. The written grievance shall state on the grievance form, attached as Appendix D, the specific article and paragraph of this Agreement alleged to have been violated, an explanation of the facts, and the relief requested.
Grievance Contents. All grievances must be completed on the Union Grievance Form and contain the following information:
A. The aggrieved employee’s name, or names of all grievants if it is a group grievance;
B. The aggrieved employee’s department;
C. The date, time and location the circumstances or incidents giving rise to the grievance occurred;
D. The date the grievance is filed;
E. A description of the circumstances or incidents giving rise to the grievance;
F. The specific provisions of the Agreement alleged to have been violated;
