Health and Hospitalization Coverage. Subd. 1 – Full-Time Employees. The School District will contribute a sum not to exceed $650 for the 2020-2021 benefit plan year, and $675 for the 2021-2022 benefit plan year, per month or the premium (whichever is less) toward the single premium; and not to exceed $1450 for the 2020-2021, and $1480 for the 2021-2022 benefit plan year, per month toward the cost of the family premium for insurance coverage for any employees who are assigned enough hours per week to qualify as a full-time employee as defined in Section 3.4 of this Agreement. Any additional cost of the premium shall be borne by the employee and paid through payroll deduction.

Health and Hospitalization Coverage a. The Blue Cross/Blue Shield Blue Select Plan will be 100% funded by the District, for those supervisors selecting this Plan. Supervisors who choose to remain in the PACE Plan will pay 15% of the premium at each enrollment level through payroll deduction.
Health and Hospitalization Coverage a. The Blue Cross/Blue Shield Blue Card PPO Plan will be 100% funded by the District, for those administrators selecting this Plan. The in-network co-pay maximum shall be $15.00 per visit.
Health and Hospitalization Coverage. Subd. 1 – Full-Time Employees. The School District will contribute a sum not to exceed $690 for the 2022-2023 benefit plan year, and $710 for the 2023-2024 benefit plan year, per month or the premium (whichever is less) toward the premium for single coverage. Employees who are assigned enough hours per week to qualify as a full-time employee as defined in ART I CL E I I I , Section 3.4 of this Agreement and enroll in the School District’s group health and hospitalization insurance plan shall be eligible for said premium contributions. Health and Hospitalization Coverage. The school district shall contribute a sum of not to exceed $3,925 annually for the 2013-2014 school year and $4,125 annually for the 2014-2015 school year towards the premium for individual or family coverage for each regular full-time employee employed by the school district who qualifies for and is enrolled in the school district group health and hospitalization plan. To the extent the employer contribution described in this paragraph together with any Health Reimbursement Arrangement (HRA) distribution in Section 8 and 8.1 are not sufficient, the employee will be responsible for any remaining cost of coverage under the District’s group health and hospitalization plan.

  • Health and Hospitalization Insurance Single Coverage: The School District shall contribute a sum not to exceed $284.00 per month toward the premium for individual coverage for each full-time employee employed by the School District who qualifies for and is enrolled in single coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.

  • Health Care Coverage The Company shall continue to provide Executive with medical, dental, vision and mental health care coverage at or equivalent to the level of coverage that the Executive had at the time of the termination of employment (including coverage for the Executive’s dependents to the extent such dependents were covered immediately prior to such termination of employment) for the remainder of the Term of Employment, provided, however that in the event such coverage may no longer be extended to Executive following termination of Executive’s employment either by the terms of the Company’s health care plans or under then applicable law, the Company shall instead reimburse Executive for the amount equivalent to the Company’s cost of substantially equivalent health care coverage to Executive under ERISA Section 601 and thereafter and Section 4980B of the Internal Revenue Code (i.e., COBRA coverage) for a period not to exceed the lesser of (A) 18 months after the termination of Executive’s employment or (B) the remainder of the Term of Employment, and provided further that (1) any such health care coverage or reimbursement for health care coverage shall cease at such time that Executive becomes eligible for health care coverage through another employer and (2) any such reimbursement shall be made no later than the last day of the calendar year following the end of the calendar year with respect to which such coverage or reimbursement is provided. The Company shall have no further obligations to the Executive as a result of termination of employment described in this Section 8(a) except as set forth in Section 12.

  • Health Plans The health plans offered and benefits provided by those plans shall be those approved by the City's JLMBC and administered by the Personnel Department in accordance with LAAC Section 4.

  • HEALTH CARE PLANS ‌ Notwithstanding the references to the Pacific Blue Cross Plans in this article, the parties agree that Employers, who are not currently providing benefits under the Pacific Blue Cross Plans may continue to provide the benefits through another carrier providing that the overall level of benefits is comparable to the level of benefits under the Pacific Blue Cross Plans.
