HEALTH AND WELFARE FUND CONTRIBUTION. 1. The Company will contribute to any existing lawful Health and Welfare Fund of any Federation Local and commencing thirty (30) days after notice in writing to any such lawful Fund, as may be established hereafter, by any other Federation Local, the sum of $48.54 per musician for each day, or $48.54 per show, if the Musician works on more than one (1) show in a given day. Contributions will include days on which pre-recordings are used according to Paragraph J(1) of this Agreement but nothing herein shall require more than one (1) payment of $48.54 for such day, if such Musician performs other covered work on that day, other than music preparation but such work is limited to that show.

HEALTH AND WELFARE FUND CONTRIBUTION. (NON-SYMPHONIC) The Company will contribute to the lawful Health and Welfare Fund presently established by any Federation Local and commencing thirty days after notice in writing, the Company will contribute to any other lawful Health and Welfare Fund established by any other Federation Local, the sum of $21.00 effective December 1, 2005 for each original service performed within the jurisdiction of such Federation Local by each musician covered by this agreement. With respect to any such original service performed within the jurisdiction of a Federation Local where no such Fund is established, the Company shall pay to each such musician said sum of $21.00 effective December 1, 2005. In the case or arrangers, orchestrators and copyists, an original service will be considered as a calendar day.
HEALTH AND WELFARE FUND CONTRIBUTION. The Company will contribute to any existing lawful Health and Welfare Fund of any Federation Local and commencing thirty (30) days after notice in writing to any such lawful Fund as may be established hereafter by any other Federation Local, the sum of $22.00 per day effective January 27, 2013; $24.00 per day effective January 26, 2014; and $25.00 per day effective January 25, 2015, for work performed within the jurisdiction of such Federation local by each Arranger, Orchestrator, and Copyist covered by this Agreement.
HEALTH AND WELFARE FUND CONTRIBUTION. For the period commencing with the effective date of this Agreement, Employer shall contribute to any existing lawful Health and Welfare Fund of any Federation Local and commencing thirty (30) days after notice in writing to any such lawful Fund as may be established hereafter by any other Federation Local, the sum of $17.50 per Program if the Musician works on more than one Program in a given day, with a maximum of $87.50 per week. Contributions shall include days on which pre-recordings are used according to paragraph J of this Exhibit, but nothing herein shall require more than one payment of $17.50 for such day if such Musician performs other covered work on that day but such work is limited to that Program. With respect to each musical service performed within the jurisdiction of a Federation local where no such Fund is established, Employer shall pay said amount to each Musician. Health and Welfare payments for Musician rendering services under this Agreement shall be sent to the plan of the local union in which the individual Musician is a member (if that local union has such a plan), regardless of the place where the Musician performed the services. No such Health and Welfare Fund contribution whether paid to any Fund or paid directly to a Musician shall be the basis for computing the applicable American Federation of Musicians' Employers' Pension Fund contribution or any other payments under this Agreement such as doubling, overtime, premium time pay, etc. Payments shall be made simultaneously with wage scales.

HEALTH AND WELFARE FUND CONTRIBUTION. A. INSTRUMENTALISTS Effective July 29, 2012 the contribution is $4.045 per hour, based on 12-hour minimum for all employees (except those whose work is on a “Page Rate” basis) - per musician total is $48.54 per day.
HEALTH AND WELFARE FUND CONTRIBUTION. The parties hereto are parties to, and bound by the provisions of the "Agreement and Declaration of Trust Establishing the I.A.T.S.E. - Basic Craft-Guilds-Hollywood Producers- Health and Welfare Fund for the Employees of the Motion Picture and Allied Industries" dated as of October 20, 1952 (hereinafter referred to as the "Welfare Fund"), with respect to the musicians classifications of the agreement. In accordance with Article V, Sections 1 and 2, of such Welfare Fund the rate of contribution to the Welfare Fund by Producer for each such employee employed by it hereunder, shall be as follows: During the term of this Agreement, Producer shall pay into the Plan one dollar, five and eight-tenths cents ($1.058) until August 3, 2003 (one dollar, twenty-five cents ($1.258) effective August 3, 2003) for each hour guaranteed a musician by Producer or each hour worked by a musician for Producer on or after November 1, 1992 under the terms of this Agreement, including "straight time" and "overtime" hours on any day worked. Should subsequent increases be negotiated by the I.A.T.S.E. during the term of this Agreement, the same will apply. Minimum calls shall constitute time worked. Weekly schedules shall be considered as a minimum of forty-three and two-tenths (43.2) hours for a full workweek. With respect only to recording and rehearsal musicians employed for less than twelve (12) hours on a daily basis, whether for a single or double session, the contributions to the Fund shall be based on minimum credit of twelve (12) hours for any such day, at the rate of one dollar, five and eight-tenths cents ($1.058). Contributions for musicians employed by Producer at a page rate shall be computed on the basis of one and one-half hours for each $17.16 earned with Producer at a page rate as a copyist; for each $27.42 earned at a page rate as an orchestrator or an arranger. Exhibit I For a dental plan, Producer shall pay into the Welfare Fund an additional eighteen and seven-tenths cents (18.7c) per hour contribution. For a vision care plan, Producer shall pay into the Welfare Fund a five cent (5c) per hour contribution. The above contributions are in accordance with the present pattern of contributions by Producers required under the Welfare Fund. In the event this present pattern of contributions under the Welfare Fund should be changed during the term of this agreement, then the rate of contributions as provided herein shall automatically be changed to conform to such new pa...
