Health and Welfare Contributions a. For each day in which a recording musician performs work under this Agreement, the Employer shall contribute $25.00 (twenty-five dollars) on behalf of each musician so employed. The Employer shall also contribute an amount calculated in accordance with Section IV on behalf of those musicians employed under a page rate (i.e. Orchestrators, Copyists and other Music Preparation).
b. With respect to musicians who are members of Federation Locals 47 (Los Angeles), 802 New York), and any other Local which hereafter establishes a Health and Welfare Plan and notifies Industry, Health and Welfare payments for musicians rendering services under this Agreement shall be sent to the plan designated by the local union in which the individual musician is a member, regardless of the place where the musician performed the services. In such case, such payment shall be expressly contingent on the information set forth on the B Report Form as to the member’s local affiliation.
1. All contributions with respect to musicians who are members of AFM Local 47 (Los Angeles) will be made on the musician’s behalf to the Entertainment Industry Flex Plan (the “Flex Plan”). With respect to those musicians, the Employer agrees to be bound by the provisions of the Agreement and Declaration of Trust establishing the Flex Plan as it may be amended from time to time, which is incorporated by reference into and made a part of this Agreement.
2. All contributions with respect to musicians who are members of AFM Local 802 (New York) will be made on the musician’s behalf to Local 802 Musicians Health Fund. With respect to those musicians, the Employer agrees to be bound by the provisions of the Agreement and Declaration of Trust establishing the Local 802 Musicians Health Fund, as it may be amended from time to time, which is incorporated by reference into and made a part of this Agreement.
c. With respect to all other musicians, Health and Welfare payment for musicians rendering services under this Agreement shall be paid to each musician, regardless of the place where the musician performed the service.
d. No such Health and Welfare Fund contribution, whether paid to any Fund or paid directly to a Musician, shall be the basis for computing the applicable AFM-EP contribution or any other payments under this Agreement such as doubling, overtime, premium time pay, etc.
e. Payments will be made simultaneously with Musicians’ wage scales.
Health and Welfare Contributions. The Fringe contributions contained in Section 12 shall also apply to Travel Day Wages contained herein.
Health and Welfare Contributions. Effective July 1, 2007 the County shall contribute a total of $7,550 per year, effective July 1, 2013 a total of $8050 per year and effective July 1, 2014 a total of $8,550 per year toward employee health and welfare coverage. Employees who work between 4 and 6 hours may choose not to participate in the County provided medical coverage. These employees may choose to receive cash-in-lieu as follows:
a) The health and welfare cap is pro-rated based on hours of employment;
b) The costs of the employee selected vision, dental and life insurance plans are subtracted from “a”’
c) The employee receives 45% of the balance from “b” as cash-in-lieu.
Health and Welfare Contributions. The Company would maintain health and welfare benefits at current levels by maintaining current contributions, and increasing them by up to the following amounts, if and as required by the applicable health and welfare funds: January 1, 2015 up to $.35/hr August 1, 2015 up to $.50/hr August 1, 2016 up to $1.00/hr August 1, 2017 up to $1.00/hr August 1, 2018 up to $1.00/hr The 2010 Restructuring Agreement would be modified so that the August 1, 2014 health and welfare increase will be deferred to January 1, 2015, as reflected above.
Health and Welfare Contributions. 10:01 The Tolko Industries Ltd., Manitoba Solid Wood Division, Health and Welfare Plans shall be administered in accordance with the terms of the current insurance policies. The following is a description of the plans and the insurance plan text that should be consulted for a comprehensive description of the insurance coverage period.
10:02 The Company agrees to provide to its employees the Health and Welfare Plan described in Schedule "A" herein. Employees will be eligible to become members of the plan upon completion of forty-five (45) calendar days of continuous employment with the Company, provided they are at work on that date. If not at work on that date, they will be eligible immediately upon return to work.
a) An employee with more than one (1) year but less than five (5) years of continuous service who is laid off, shall be covered under the Health and Welfare Plan, and Dental Plan, for a period of four (4) months. An employee with more than five (5) years of continuous service shall be covered under the Health and Welfare Plan for a period of six (6) months.
b) An employee with more than forty-five (45) calendar days of continuous service but less than one (1) year shall be covered if laid off due to scheduled vacations.
Health and Welfare Contributions a . Contributions For Employees Hired Before March 13, 1983 and Part-time Employees Hired On Or After That Date: Effective March 13, 1983: Maintain contribution levels in effect as of that date. Effective November 1, 1983: Contributions to be increased to that amount necessary to maintain the current level of health and welfare benefits.
Health and Welfare Contributions. The Employer shall contribute to the Health and Welfare Fund in accordance with Article XII of the main body of this Agreement for employees who work thirty (30) or more hours per week. Health and Welfare contributions shall not be required for employees working less than thirty (30) hours per week unless required by law.
Health and Welfare Contributions. The Employer shall contribute to the Administrative District Council 1 Welfare Fund the amount specified in this Agreement per hour for work performed by all employees and supervisors covered by this Agreement.
Health and Welfare Contributions. Temporary Employees— The Employer shall pay the sum designated below for each day worked by a tem porary employee covered by this Agreement, into the Chicago Truck Drivers, Helpers and Warehouse Workers Union (Independent) Health and W elfare Fund heretofore created by an Agreement and declar ation of trust pursuant to collective bargaining agree ments between certain employers and the Union created for the payment of health and w elfare bene fits to covered employees, and the Union agrees to provide health and welfare benefits to each eligible employee with respect to whom the Employer makes such payment as follow s: Effective 4/1/76: $5.20 per day worked, with a maximum of $26.00 per week. Effective 4/1/77: $6.00 per day worked, with a maximum of $30.00 per week. Effective 4/1/78: $6.60 per day worked, with a maximum o f $33.00 per week, and so forth from here on, in accordance with the agreements signed between the Union and the Central Motor Freight Association. Days off which are paid for under the holiday and vacation provisions of this Agreement shall be con sidered as days worked for the purpose o f this Section. The Employer shall transmit the contributions due hereunder with respect to each month not later than the 10th of the following month. Contributions w ill continue during any period a tem porary employee is absent because of an occupational illness or Injury covered by the Workmen’s Compen sation Daw for which the Employer is liable for a period of twelve (12) months or until the employee returns to work, whichever period Is the shorter. The Employer further agrees to make the above contri butions for temporary employees during periods when a collective bargaining agreement is being negotiated.
