Identification Badges. Identification badges will be supplied by Advanced Behavioral Health (ABH) to all credentialed individuals who are providing services to children. Badges must be presented to the child/youth and any present adults at the time of service and must be worn for the duration of the service. Badges will be updated every two years during the re- credentialing process. Any individual or agency who fails to submit a photo ID to ABH within the designated timeframe will have their credentialing status terminated.

Identification Badges. Those maintenance and operations unit members choosing not to wear the shirts provided by the District, will be required to wear a District provided ID photo identification (ID) badge while on duty except at those times when wearing the badge would be a safety hazard. The employee may choose to cover his/her last name. Xxxxxx damaged, lost, or destroyed in the course of his/her work will be replaced at District expense.
Identification Badges. Picture identification badges may be issued to employees by the Employer, and if so, will be worn in a sensible but conspicuous place on the employee's person. The Employer will pay the replacement fee for a badge that is lost no more frequently than once in any eighteen (18) month period of time. Otherwise, if the badge is lost or mutilated by the employee, there will be a replacement fee of three dollars ($3.00) to be borne by the employee. The cost of replacing the badge damaged due to normal wear and tear will be borne by the Employer and will not be the responsibility of the employee.

Identification Badges. SONIFI will provide photo identification badges to be worn by Contractor personnel, including subcontractors, engaged in the performance of Contractor’s duties under this Agreement, including all SOWs. The identification badges must be displayed by all Contractor personnel at all times while performing Services at Customer Facilities. Contractor will immediately notify SONIFI if any identification badge is lost or stolen or is damaged and needs to be replaced. Contractor will be responsible for all costs associated with lost, stolen, or damaged identification badges.
Identification Badges. City agrees to provide flat badges for sale by Department to employees working in the Criminal and Community Justice Divisions. The flat badge remains property of the City, and at the time of employee termination, the badge must be returned to the Department without reimbursement to the employee. However, if the employee transfers to the Civil division or ends their employment at the City Attorney’s office, the employee may choose to sell the badge to another eligible attorney in the Criminal or Community Justice Divisions or have the flat badge encased at the employee’s expense.
