Insurance Package. The board will provide upon the payment of $1.00 enrollment fee a fully paid insurance package of health/medical, dental and prescriptive plans. The Board shall also provide a fully paid long term disability policy with benefits consistent with the LED plan in effect in the school corporation. The Board will provide life insurance in the amount of 2.5 times the salary of the Superintendent.

Insurance Package. If the certified employee wishes to participate in the group health plan and qualifies for the fringe benefit, the employer will pay a package consisting of: • 75% of a single employee Gold level health insurance policy premium per month. • If applicable, the employer will pay up to $250.00 per month for an employee’s dependent health insurance premium. • $50.00 for every 5 years of full-time employment with the district, paid per month. The employer will pay no more than $25,000.00 per month toward the entire group health plan, including certified and classified staff policies. If the package listed above costs the district more than $25,000.00 per month, the percentage paid of a single employee Gold level health insurance policy premium will be adjusted to fall under the $25,000.00 per month cap. The excess premium amount will be subject to salary reduction under Section 125 or salary reduction. If the certified employees wishes to choose a different policy within the group plan, the excess premium amount will be subject to salary reduction under Section 125 or salary reduction. If the employee is covered with health insurance through another source, the employer will pay $250.00 toward an alternative benefit package of insurance chosen by the employee. Flex benefits (unreimbursed medical and dependent day care) are eligible for the $250 benefit package only if proof of health insurance through another group is given. If the employee has individual coverage or no coverage, the benefits cannot be put toward the flex benefits. Approved: June 14, 2018 Part Time Employees: Part-time employees will be eligible to receive $250 per month toward health insurance or the alternative insurance package. SECTION 125 FLEXIBLE BENEFIT PLAN All employees age 19 or older and work 17.5 hours or more per week are eligible to participate in Section 125 plans. This does not necessarily mean you are eligible for the fringe benefit plan. Open enrollment in the plan is scheduled for August of each year. New employees must enroll within 30 days of employment to become eligible for plan participation. If an employee does not enroll during this period, it will be necessary for the employee to wait until the next open enrollment period prior to the next plan year to enroll in plan benefits.   • Insurance Benefits Borrower shall cooperate with Lender in obtaining for Lender the benefits of any Insurance Proceeds lawfully or equitably payable in connection with the Property, and Lender shall be reimbursed for any expenses incurred in connection therewith (including reasonable attorneys' fees and disbursements, and the payment by Borrower of the expense of an appraisal on behalf of Lender in case of a fire or other casualty affecting the Property or any part thereof) out of such Insurance Proceeds.

Neither the Company nor any Subsidiary has any reason to believe that it will not be able to renew its existing insurance coverage as and when such coverage expires or to obtain similar coverage from similar insurers as may be necessary to continue its business without a significant increase in cost.
