Medical and Other Insurance. The Employee will be entitled to participate in the Company medical insurance plan and other insurance plans from time to time subject to the provisions of the company’s insurance plans in effect at the time.
Medical and Other Insurance. The Executive will be entitled to participate in the Company medical insurance plan and other insurance plans from time to time subject to the provisions of the company's insurance plans in effect at the time.
Medical and Other Insurance. 18.2.1 Insurance will be provided to all employees consistent with the City's Administrative Policy 2.7.14 dated January 1, 2008 and as restated and amended effective January 1, 2011, except as specifically modified in this Agreement. During the term of this Agreement, the City will provide employee and dependent medical, dental with orthodontia, vision and prescription insurance with the same level of benefits in place in 2011.
18.2.2 Medical insurance will continue to be provided via City of Puyallup Healthcare Plan 1, which became effective January 1, 2011. As the Employer establishes annual healthcare budget rates, if at any time the budgeted healthcare rates meet or exceed 10% of existing rates, both parties agree to open this agreement under Article 18, Section 18.2.
18.2.3 Options will be pursued to continue to promote wellness.
18.2.4 If the medical benefit plans listed in Section 18.2 are scheduled to be modified, the parties will negotiate those scheduled modifications.
Medical and Other Insurance. You must procure and maintain in force at your own cost, the maximum coverage available under Medicare Parts A and B. We may accept documented equivalent coverage if you are not qualified for Medicare or are insured under other adequate programs. However you must maintain, at your expense, a supplemental (“Medigap”) type insurance. We do not provide supplemental insurance. You must also shall procure and maintain, at your own expense, sufficient renter’s insurance coverage against damage or, loss to, or theft of, your personal property maintained at the Community and coverage for personal liability and medical payments should a claim be made or suit brought against you for damages because of a bodily injury or property damage caused. You must provide evidence of such insurance prior to occupancy or at our request.
Medical and Other Insurance. City shall pay the entirety of the cost of health insurance for Employee, including her dependents. City shall provide Employee with the same disability insurance and life insurance benefits as is provided to other employees of the City. City shall provide Employee with a term life insurance policy in the face amount of $250,000.
Medical and Other Insurance. The Employee is entitled to the global medical insurance, work-related injury and accident insurance and D&O Liability Insurance that is appropriate based on his/her position.
Medical and Other Insurance. Types and conditions of medical insurance and other types of insurance make the fifth essential condition of a labour contract. In practice, however, there are cases when types of insurance are imposed that are not required by the legislation or when disadvantageous and illegal conditions of medical insurance or specific insurance companies are being lobbied. I think that cases like this merit court arbitration in accordance with requirements provided for in Article 55, part 3 of the Constitution of the Russian Federation. Service rights and freedoms of a civil servant may only be limited by the federal legislation, and then only to the extent necessary to protect the constitutional system, good morals, health, rights and legitimate interests of other individuals, and ensuring national defence capabilities and state security. 15
Medical and Other Insurance. SECTION A. The Company shall provide medical, dental, life and temporary disability coverage as described in the Group Insurance booklets, or in the Company’s determination, substantially equivalent benefits, to regular full-time employees effective the first of the month following sixty (60) days from the date of hire.
SECTION B. To be eligible for this benefit plan an employee must have completed the prescribed waiting period. Coverage under the benefit plan will be terminated the last day of the month during which employment terminates, providing that the employee pays their share of the premium for the entire month. Provisions of COBRA will apply to the medical and dental coverage of the insurance for continuous purposes.
SECTION C. The current cost to the employee for particular coverage is outlined in Appendix B.
SECTION D. Any future increase in Plan costs for medical insurance during the term of this Agreement will be shared 60% by the Company and 40% by the employee. Any increase for dental insurance or short-term disability insurance will be paid solely by the employee.
SECTION E. Any employee who is absent and does not receive their normal paycheck will be responsible for making their insurance deduction payments or the Company, at it option, may take the required missed deductions for the employee’s check after their return to work.
Medical and Other Insurance. Health Insurance
a. Contributions: Custodians/ day leads and maintenance workers who choose to participate in the district health insurance will enroll in the current health insurance plans provided by the district. The School Committee contribution rate for district health insurance will be 75% and the employee contribution is 25%. Life Insurance
b. The district agrees to provide life insurance in the amount of $10,000 for all active custodians, day leads, and maintenance workers at 65% contribution of the premium by the district, 35% by the employee.
c. The district will provide a Group Voluntary Life and Accidental Death and Dismemberment Insurance policy at 100% contribution by the employee. Long Term Disability
d. The district will agree to provide long-term disability insurance at 100% contribution by the employee. Dental Insurance
e. The Committee agrees to provide Delta Dental plan or other comprehensive dental plan at 100% contribution by the employee.
f. All premium payments will cease upon termination of employment.
Medical and Other Insurance. The Executive will be entitled to participate in the Company medical, dental, disability, life, accidental death and travel accident insurance plans and other insurance plans on the same terms as the Company's executive employees.
