In addition to the cash payment to the employee, the City shall contribute for each full- time employee who opts out of medical coverage an additional amount to the Health Fund as follows: City Contribution One Party $117.26 per payday Two Party $93.59 per payday Family $72.86 per payday 17.3.2.2 Effective July 1, of each year of the Agreement, the City contribution rate provided in the previous year of the Agreement to each employee who opts out of medical coverage shall be adjusted to reflect the full annual percentage increase in the Portland-Salem medical care component in the Consumer Price Index for Urban Wage Earners and Clerical Workers (CPI-W) current base period measured by the reported percentage change between the second half of the most recent calendar year and the second half of the second most recent calendar year as published by the federal Bureau of Labor Statistics. However, in no event shall the contribution rate increase be less than two percent (2%) or greater than ten percent (10.0%). 17.3.2.3 The City shall pro-rate the cash payment and City contribution in 17.3.2 and 17.3.2.1 above for part-time benefits eligible employees based on the standard hours schedule.

Medical Coverage Opt Out. An employee may elect to opt out of medical insurance coverage provided by the Department for the employee’s spouse and/or dependents, if the employee provides credible evidence during the open enrollment period that he/she has qualifying medical insurance coverage from another source. In such event, the employee shall be entitled to receive 50% of the total premiums, FSA, and HRA-VEBA contributions that would have been paid by the Department for the employee’s spouse and/or dependents, contributed to their HRA-VEBA account.
Medical Coverage Opt Out. Those bargaining unit members having and providing proof of health insurance coverage elsewhere may elect to “opt-out” of College provided health insurance (medical, dental and vision). Bargaining unit members who “opt-out” will receive a 33 1/3% (COBRA Actuarial Equivalent) payment of the savings to the College premised upon the current cost to the College for the provision of health insurance for the particular bargaining unit member. The Opt-Out payment willbe paid on a quarterly basis, payable the last pay period of the quarter. Bargaining unit members who elect to “opt-out” and incur a qualifying event (as defined by the IRS) during the opt-out period of coverage will be eligible for re-enrollment at the time of the qualifying event. Bargaining unit members who opt-out are not eligible for transition benefits pay-out the year of opt-out. Bargaining unit members who have “opted-out” and decide to re-enroll during the Fall open enrollment period will incur transition benefits and employment benefit contributions as outlined in Section 11.12.00.

  • Medical Coverage The Executive shall be entitled to such continuation of health care coverage as is required under, and in accordance with, applicable law or otherwise provided in accordance with the Company’s policies. The Executive shall be notified in writing of the Executive’s rights to continue such coverage after the termination of the Executive’s employment pursuant to this Section 3(d)(iv), provided that the Executive timely complies with the conditions to continue such coverage. The Executive understands and acknowledges that the Executive is responsible to make all payments required for any such continued health care coverage that the Executive may choose to receive.

  • Medical and Dental Coverage The County and Union agree that this Memorandum of Understanding shall be reopened at the County's request to meet and confer to discuss and mutually agree upon changes related to the Medical and Dental Plans, benefits, and contribution rates.

  • Insurance or Other Medical Coverage Patient acknowledges and understands that this Agreement is not an insurance plan, and not a substitute for health insurance or other health plan coverage (such as membership in an HMO). It will not cover hospital services, or any services not personally provided by Direct Doctors. Patient acknowledges that Direct Doctors has advised that patient obtain or keep in full force such health insurance policy(s) or plans that will cover Patient for general healthcare costs.   • All Coverages Each insurance policy required in this item shall be endorsed to state that coverage shall not be suspended, voided, cancelled, reduced in coverage or in limits except after thirty (30) days' prior written notice by certified mail, return receipt requested, has been given to the Town. Current certification of such insurance shall be kept on file at all times during the term of this agreement with the Town Clerk.

  • Insurance Term The Consultant shall procure and maintain for the duration of this Agreement, insurance against claims for injuries to persons or damage to property which may arise from or in connection with the performance of the work hereunder by the Consultant, its agents, representatives, or employees.
