Optical Coverage Sample Clauses
Optical Coverage. Each employee shall be entitled to $500.00 aggregate 30 reimbursement over the life of the contract for examinations frames, and lenses, for the employee, 31 spouse and dependent children residing in the household of the employee through age 18. This 32 benefit shall remain in effect pending review by the insurance committee which will have authority 33 to determine coverage, benefits, and cost sharing for optical benefits.
Optical Coverage. The Employer agrees to provide Optical coverage for part-time employees as follows:
Optical Coverage. Employees, their spouses and eligible dependents shall be provided vision coverage through Eye-Med Advantage or its satisfactory equivalent. Vision benefits are available to dependents through the end of the calendar year in which he/she turn 19 or until age 25 if he/she are full time students. Proof of student status will be requested annually after the age of 19 to verify eligibility. Plan specifics are outlined in Appendix B.
Optical Coverage. Eye Care coverage shall be offered to all Employees. Employees will have the option to opt in or out of the City's eye care benefit plan and to pay the appropriate eye care premium for the level of benefit (single, single plus one, or family) they prefer.

After six (6) months of employment, the City of Xxx Arbor shall provide to each member of the bargaining unit a $200 maximum benefit every 24 months through Mutual Eye Claim Audits, Inc. or its satisfactory equivalent for optical expenses for the employee and their spouse. Also provided is a $200 maximum benefit every 12 months for eligible dependents under age 19. Covered expenses include lenses, frames and contact lenses. This benefit includes a vision examination plus $200 towards covered expenses.
Optical Coverage. The Town agrees to enroll all regular full-time employees in the CSEA Employee Benefit Fund Vision Care Plan called Platinum.
Optical Coverage. The District agrees to provide, at no cost to the employee, optical coverage as detailed in Vision Service Plan A - $20 deductible coverage, and pay the costs thereof.

The District shall provide a self-funded optical benefits plan with standard frames benefit at thirty-three ($33) dollars, and with all other provisions in effect June 30, 2005 remaining unchanged, or equivalent coverage optical benefits to all full-time employees. Optical coverage will be through a self- funded plan administered by SET-SEG. All employees working less than full-time, but more than half- time may enroll for optical benefits and pay the pro-rated share of the premium equivalent payment.
