Patient Assistance Related Functions. For each of the Reviewed Policies and Procedures identified in Section II.A.1 above, the report shall include the following items:‌ 1. a description of the documentation (including policies) reviewed and any personnel interviewed;‌ 2. a detailed description of Good Days’ systems, policies, processes, and procedures relating to the items identified in Section II.A.1 above, including a‌ general description of Good Days’ control and accountability systems (e.g., documentation and approval requirements, and tracking mechanisms) and written policies regarding the Reviewed Policies and Procedures;‌‌‌ 3. a description of the manner in which the control and accountability systems and the written policies relating to the items identified in Section II.A.1 above are made known or disseminated within Good Days;‌ 4. findings and supporting rationale regarding any weaknesses in Good Days’ systems, processes, policies, and procedures relating to the Reviewed Policies and Procedures, if any; and‌ 5. recommendations to improve any of the systems, policies, processes, or procedures relating to the Reviewed Policies and Procedures, if any.‌

Patient Assistance Related Functions a. TAF’s systems, policies, processes, and procedures relating to Patient Assistance Related Functions. This review shall include an assessment of the following:‌ i. TAF’s systems, policies, and practices for ensuring that TAF and the officers, directors, employees and agents of TAF are independent from and do not have any conflicts of interest with Donors or potential Donors. ii. TAF’s systems, policies, and practices relating to disease funds, including: (a) the systems, policies, and practices that TAF uses to establish, delineate, and modify any disease fund. This includes the bases upon which TAF makes decisions about funds (as referenced in Section III.F.1.c of the IA), the definition and type of disease funds, the scope of coverage for disease funds (as referenced in Section III.F.1.d), the Notification List practices (as referenced in Section III.F.1.f) and the drug coverage methodology used by TAF in connection with its funds (as outlined in Section III.F.1.g); and (b) TAF’s documentation and recordkeeping practices relating to disease funds as outlined in Section III.F.1. iii. TAF’s systems, policies, and practices as they relate to interactions with Donors and potential Donors, including: (a) interactions relating to seeking or receiving donations for any fund (as referenced in Section III.F.2.c); (b) interactions relating to requests for or the provision of data to any Donor (as referenced in Section III.F.2.b); and‌‌‌ (c) TAF’s documentation and recordkeeping practices relating to interactions with Donors (as outlined in Section III.F.2).‌ iv. TAF’s systems, policies, and practices as they relate to providing financial assistance to patients, including: (a) the systems, policies, and practices that TAF utilizes in connection with its Screening Process for determining patient eligibility for assistance (as referenced in Sections III.F.3.a-c);‌ (b) the systems, policies, and practices relating to TAF’s award of assistance to patients and the communication about decisions to award financial assistance (as referenced in Sections III.F.3.d and e); and‌ (c) TAF’s documentation and recordkeeping practices relating to providing assistance to patients (as reference in Section III.F.3).‌

Pfizer’s organizational structure as it relates to arrangements and interactions with Independent Charity PAPS, including:‌ i. the identification of those individuals, departments, or groups within Pfizer (e.g., the Independent Charity Group, legal, compliance) that have responsibility for, or involvement with, such arrangements and interactions;‌‌ ii. the respective scope and nature of the responsibilities of each individual, department, or group with responsibility for, or involvement with, arrangements and interactions with Independent Charity PAPs; iii. the identification of those individuals, departments, or groups within Pfizer (e.g., the commercial business units) that are precluded from involvement with arrangements and interactions with Independent Charity PAPs; and‌ iv. methods that Pfizer uses to separate Independent Charity PAP- related responsibilities from the commercial business units. b. Pfizer’s written policies and procedures as they relate to arrangements and interactions with Independent Charity PAPs, including:‌ i. the criteria governing whether and under what circumstances Pfizer would donate to an Independent Charity PAP or any specific disease state fund of such a PAP;‌‌ ii. communications (including any limitations on such communications) between any representatives of Pfizer and any Independent Charity PAP (including the identity of individuals authorized to engage in such communications, the circumstances of such communications, and the subject matter of such communications including the exchange of any data); iii. communications (including any limitations on such communications) between those individuals, departments, or groups within Pfizer with responsibility for Independent Charity PAPs and the commercial business units of Pfizer (including the identity of individuals authorized to engage in such communications, the circumstances of such communications, and the subject matter of such communications); and iv. communications (including any limitations on such communications) between representatives of Pfizer and health care providers or patients regarding assistance available through any Independent Charity PAP. c.   • Outpatient Dental Anesthesia Services This plan covers anesthesia services received in connection with a dental service when provided in a hospital or freestanding ambulatory surgical center and: • the use of this is medically necessary; and • the setting in which the service is received is determined to be appropriate. This plan also covers facility fees associated with these services. This plan covers dental care for members until the last day of the month in which they turn nineteen (19). This plan covers services only if they meet all of the following requirements: • listed as a covered dental care service in this section.   • Routing for Operator Services and Directory Assistance Traffic For a Verizon Telecommunications Service dial tone line purchased by VarTec for resale pursuant to the Resale Attachment, upon request by VarTec, Verizon will establish an arrangement that will permit VarTec to route the VarTec Customer’s calls for operator and directory assistance services to a provider of operator and directory assistance services selected by VarTec. Verizon will provide this routing arrangement in accordance with, but only to the extent required by, Applicable Law. Verizon will provide this routing arrangement pursuant to an appropriate written request submitted by VarTec and a mutually agreed-upon schedule. This routing arrangement will be implemented at VarTec's expense, with charges determined on an individual case basis. In addition to charges for initially establishing the routing arrangement, VarTec will be responsible for ongoing monthly and/or usage charges for the routing arrangement. VarTec shall arrange, at its own expense, the trunking and other facilities required to transport traffic to VarTec’s selected provider of operator and directory assistance services.

  • Dependent Care Assistance Program The County offers the option of enrolling in a Dependent Care Assistance Program (DCAP) designed to qualify for tax savings under Section 129 of the Internal Revenue Code, but such savings are not guaranteed. The program allows employees to set aside up to five thousand dollars ($5,000) of annual salary (before taxes) per calendar year to pay for eligible dependent care (child and elder care) expenses. Any unused balance is forfeited and cannot be recovered by the employee.

  • CUSTOMER SERVICE FUNCTIONS The Servicer shall handle all Customer inquiries and other Customer service matters according to the same procedures it uses to service Customers with respect to its own charges.

  • MIXED FUNCTIONS An employee engaged for more than two hours during one day or shift on duties carrying a higher rate than his or her ordinary classification shall be paid the higher rate for such day or shift. If for two hours or less during one day or shift he or she shall be paid the higher rate for the time so worked.

  • Termination Assistance Services Following the termination of this Agreement and/or any Ordering Document, the Parties may agree for Axway to provide transition services pursuant to a duly executed SOW, during which time this Agreement will continue in full force and effect solely to the extent necessary to allow such transition services to be performed. Axway agrees that the costs for any such services shall be comparable to the fees charged to other customers for similar types of services.

  • MANAGEMENT FUNCTIONS B.1 The Association recognizes that the management of the Hospital and the direction of working forces are fixed exclusively in the Hospital and shall remain solely with the Hospital except as specifically limited by the provisions of this Agreement and, without restricting the generality of the foregoing, the Association acknowledges that it is the exclusive function of the Hospital to: (a) maintain order, discipline and efficiency; (b) hire, assign, retire, discharge, direct, promote, demote, classify, transfer, lay- off, recall, and suspend or otherwise discipline nurses, provided that a claim of discharge or discipline without just cause may be the subject of a grievance and dealt with as hereinafter provided; (c) determine, in the interest of efficient operation and high standards of service, job rating and classification, the hours of work, work assignments, methods of doing the work, and the working establishment for the service; (d) generally to manage the operation that the Hospital is engaged in and, without restricting the generality of the foregoing, to determine the number of personnel required, methods, procedures, and equipment in connection therewith; (e) make, enforce, and alter from time to time reasonable rules and regulations to be observed by the nurses which are not inconsistent with the provisions of this Agreement. B.2 These rights shall not be exercised in a manner inconsistent with the provisions of this Agreement.

  • Employee Assistance Program (EAP) Section 1. The Employer agrees to provide to the Union the statistical and program evaluation information provided to management concerning Employee Assistance Program(s). Section 2. No information gathered by an Employee Assistance Program may be used to discipline an employee. Section 3. Employees shall be entitled to use accrued sick leave for participation in an Employee Assistance Program. Section 4. Each university will offer training to local Union Stewards on the Employee Assistance Program available in their university, on university time, where an Employee Assistance Program is available.

  • DEPENDENT PERSONAL SERVICES 1. Subject to the provisions of Articles 16, 18 and 19, salaries, wages and other similar remuneration derived by a resident of a Contracting State in respect of an employment shall be taxable only in that State unless the employment is exercised in the other Contracting State. If the employment is so exercised, such remuneration as is derived therefrom may be taxed in that other State. 2. Notwithstanding the provisions of paragraph 1, remuneration derived by a resident of a Contracting State in respect of an employment exercised in the other Contracting State shall be taxable only in the first-mentioned State if: a) the recipient is present in the other State for a period or periods not exceeding in the aggregate 183 days in any twelve-month period commencing or ending in the tax year concerned, and b) the remuneration is paid by, or on behalf of, an employer who is not a resident of the other State, and c) the remuneration is not borne by a permanent establishment or a fixed base which the employer has in the other State. 3. Notwithstanding the preceding provisions of this Article, remuneration derived in respect of an employment exercised aboard a ship or aircraft operated in international traffic by a resident of a Contracting State, may be taxed in that State.
