Proof of Qualifications. Onus of proof of a teacher's placement on the schedule rests with the teacher. Such proof must be documented.
A) To qualify for a September 1st pay adjustment, a teacher must have completed course requirements prior to September 1st and must submit to Human Resource Services, by January 15th of that school year, a revised QECO evaluation or QECO acknowledgement card. Teachers who provide the required documentation by January 15th and are eligible for a pay adjustment shall receive the adjustment retroactive to September 1st.
B) To qualify for a January 1st pay adjustment, a teacher must have completed course requirements prior to January 1st of the same school year and must submit to Human Resource Services, by April 30th of that school year, a revised QECO evaluation or QECO acknowledgement card. Teachers who provide the required documentation by April 30th and are eligible for a pay adjustment shall receive the adjustment retroactive to January 1st.
C) No adjustment shall be made until the teacher has submitted proof of completion date of the final course taken.
D) No salary adjustments relative to QECO placement shall be considered after April 30th for that school year during the term of this Agreement.
E) All submissions/correspondence in regards to this clause shall be through the Human Resource Services Office.

A) A teacher presenting an Evaluation Statement from Q.E.C.O. by January 15th, based on courses taken prior to September 1st, shall have their salary adjustment, if any, made retroactive to September 1st of that same school year.
B) A teacher presenting an Evaluation Statement from Q.E.C.O. by April 30th for courses completed prior to December 31st of the previous year shall have their salary adjustment, if any, made retroactive to January 1st of the year of submission.

Humanus shall require Independent Contractor to produce a valid license or license eligibility, FBI and state background checks, child abuse clearance, and any other documentation or certification needed by Client to satisfy the requirements of their governing body. Client agrees to notify Humanus of their requirements with each referral order. Humanus agrees to provide a copy of said information to Client within 24 hours from the time of request. If an Independent Contractor subcontracts all or part of the services provided to Client, the Independent Contractor will require all subcontractors to provide the information under paragraph 5 to Humanus.
Proof of Qualifications. City Departments may require proof of such qualifications at any time from either the employee or the Contractor. After a request is made for qualifications, Contractor shall provide proof of such qualifications no later than one (1) business day after a request is made. Failure to submit proof of qualifications will result in the application of Performance Metrics Credits stated in the Master Agreement, in Section IV. Performance Metrics.
