REFERRAL ARRANGEMENTS. The parties hereby acknowledge and agree that no benefits to Practice hereunder require or are in any way contingent upon the admission, recommendation, referral or any other arrangement for the provision of any item or service offered by Manager or any of its affiliates, to any patients of Practice, Practice's employees or agents.
REFERRAL ARRANGEMENTS. The primary principle of the referral system is to provide integrated and seamless delivery of services to both job seekers and employers.
1. The partners agree to familiarize themselves with the requirements for participation in each of the required partners programs.
2. To the extent possible, the partners agree to develop materials summarizing their program requirements and to make this accessible to all partners in the comprehensive AJC.
3. To the extent possible, the partners agree to develop and utilize common intake forms.
4. The partners agree to refer clients potentially eligible for each other’s services to one another for those services.
5. The partners agree to evaluate ways to improve the referral process, including the use of customer satisfaction surveys.
6. The partners agree to provide technical assistance to each other in determining how best to respond to the diverse employment and support needs of individuals and businesses participating in AJC services.
REFERRAL ARRANGEMENTS. Except as set forth on Section 3.13(a) of the Company Disclosure Letter, no member of the Company Group is currently a party to any Referral Agreement, and is not receiving or paying, and does not have a right to receive or obligation to pay, any compensation or other remuneration in connection with a Referral Agreement. The Company Group has undertaken reasonable diligence efforts to ensure that all Referral Agreements are constructed, and that the conduct of each of its Company Group members under all Referral Agreements are, in material compliance with all Healthcare Laws.
REFERRAL ARRANGEMENTS. 9.1 The Referral process outlines how Residents will be placed with the Provider under this Contract.
9.2 Referrals will be made via telephone, email or letter but are always followed up in writing by the Council and an Individual Form of Agreement (IFA) will be raised as the final step in the process. When the Provider receives the Individual Form of Agreement, the Referral is made (except in the case of emergency Referrals which is described below).
9.3 The Council will send through the Resident's Care and Support Plan.
9.4 The Care and Support Plan will identify the Resident's assessed needs and the expected Outcomes to be achieved, plus any additional special requirements. This information will be provided by the Authorised Officer and should be provided in advance or at the time of placement but no later than five (5) Working Days from admission.
9.5 The Provider/Care Home Trusted Assessor may agree with the Council to accept a Referral by telephone on an emergency basis.
9.6 The Council shall aim to provide written confirmation of such a Referral within seven (7) calendar days.
9.7 Until a Referral is accepted the Council may withdraw the Referral in writing and place the individual in alternative accommodation.
9.8 Following a period of hospital in-patient care, it is necessary for Assessments to be undertaken prior to discharge of individuals into Home settings to enable a safe discharge. This is typically undertaken by each residential or nursing home in accordance with its own process and Assessment requirements and based on timescales determined by the Home. However, in order to reduce delays in hospital discharge, the Provider should utilise the Care Home Trusted Assessor (CHTA).
9.9 The Provider or Care Home Trusted Assessor (on behalf of the Provider) shall indicate whether they accept a Referral, as soon as possible after the Referral has been made. The room number, or another form of identification, shall be recorded on the Individual Form of Agreement (IFA) and the Care and Support Plan at the time of placement.
9.10 Out of Hours Emergency Referrals shall be made by the Emergency Duty Team (EDT) of the Council and will be made by telephone. All enquiries regarding emergency Referrals out of normal office hours (0900-1700 hours, Monday to Thursday and 0900-1630 hours, Friday) must be referred to the EDT (telephone 00000 000000).

REFERRAL ARRANGEMENTS. The parties hereby acknowledge and agree that no benefits to Metroplex hereunder require nor are in any way contingent upon the admission, recommendation, referral or any other arrangement for the provision of any item or service offered by Texas Sub or any of its affiliates, to any patients of Metroplex, or the Metroplex Providers.
REFERRAL ARRANGEMENTS. The parties hereby acknowledge and agree that no benefits to PDS hereunder require or are in any way contingent upon the admission, recommendation, referral or any other arrangement for the provision of any item or service offered by Manager or any of its affiliates, to any patients of PDS, PDS's employees or agents.
REFERRAL ARRANGEMENTS. We use various channels to market our Products & Services to you, including through various third parties, such as comparison websites. We may enter into referral arrangements with third parties for this purpose. The referral arrangements we enter into may involve us paying referral fees or commissions to third parties for referring you to XXxxxxx.xxx’s website or mobile application. The basis on which we may pay referral fees or commissions varies but may include payment of referral fees or commissions on a "per click" basis and/or a "per installation" basis. By agreeing to these Terms, you consent to XXxxxxx.xxx entering into referral arrangements with third parties that may involve the payment of referral fees or commissions to those third parties.
REFERRAL ARRANGEMENTS. As part of its activities, Desjardins Securities may enter into referral arrangements with business partners, including with the business partners members of the same financial group, the Desjardins Group. The terms of our referral arrangement will be set out in writing and provided to you prior to the account opening, either through the delivery of this Statement of Principles or otherwise. Such disclosures will allow you to make an informed decision with respect to the referral and assess any potential conflicts of interest. We undertake periodic reviews of our referral arrangements in order to ensure that the referral fees received or given do not encourage practices incompatible with our obligations toward our clients.
