Required Health Examinations or Vaccinations. Opportunity for health examinations or vaccinations required by the State of Ohio or required locally must be provided without cost to the Professional Staff Member. If a Professional Staff Member elects to have a private examination or vaccinations, the Professional Staff Member will pay the cost and provide a documented statement of satisfactory completion of the required examination or vaccinations.
