Application for Sick Leave. An employee must complete and submit an application for sick leave to their immediate supervisor immediately upon returning to duty, or in advance if the employee is aware of the need for such leave.
Application for Sick Leave. (a) An employee absent from work through illness or injury shall, within seven calendar days from the initial day of absence and thereafter as requested, submit a fully completed leave form.
(b) The Employer may request that a Sick Leave Medical Form accompany the leave form if the absence is seven calendar days or where at least 14 calendar days have elapsed since the last form was submitted and the employee has been in receipt of sick leave benefits throughout that period. The Employer may also request a report from a qualified medical practitioner when it appears that a pattern of consistent absence is developing.
(c) Any costs incurred for reports or information other than the Sick Leave Medical Form(s), shall be borne by the Employer. Such reimbursement will not exceed the fee guidelines of the BC Medical Association.
Application for Sick Leave. 1. Application for sick leave shall be filed on a form prescribed by the Employer or designee, within five (5) working days after return to duty; provided, that in the event such Employee dies before that time or before returning to duty, the Employee's executor or administrator or department head if deemed proper may file such application within six (6) months after the Employee's death. Sick leave shall not be granted unless it is provided to the satisfaction of the department head that the Employee's absence from work was necessary because of sickness.
2. The department head shall require the Employee to submit a licensed physician's certificate for absences of five (5) or more consecutive working days to substantiate the fact that the period of absence was due entirely to sickness and that the Employee is physically and/or mentally able to resume the duties of the position. The department head may require the Employee to be examined by a physician of said department head's choice provided the department assumes the cost of the physician's services.
3. Absence due to sickness lasting less than one (1) hour shall not be charged to sick leave when such sickness occurs during the final hour of work and the Employee is released from work.
4. Upon application by the Employee, sick leave when granted may include all sick leave allowance as of the last full month of service immediately preceding the return to duty from sick leave, or as much thereof as is needed, to permit the Employee to recover from the sickness.

Application for Sick Leave. 32.02.1 Professional Staff Members are to notify his/her building Principal of his/her absence no later than one-and one-half hours (1 1/2 hours before the scheduled time to report. Failure to provide such advanced notice may be cause for denial of sick leave payment. A Professional Staff Member is also required to advise his/her building Principal of the anticipated date of return to work. A Professional Staff Member returning to work earlier than expected is required to call the building Principal at least one-half hour before the end of the school day.
32.02.2 The BOARD shall require each Professional Staff Member to furnish a written, signed statement on forms provided by the BOARD for use of sick leave upon return from sick leave. If medical attention is required, the employee's statement shall list the name of the attending physician and the date (s)he was consulted. The Principal, or another member of the Administration, may require a signed physician's statement confirming the need for sick leave after five
Application for Sick Leave. 16.02.1 The licensed Educational Interpreter is to notify the Interpreter Coordinator and/or the OEC Secretary of their absence. They are to call at least by two hours before his/her report time on the day of their absence.

Abuse of sick leave when substantiated by the Principal or another member of the Administration; may result in the Professional Staff Member being subject to disciplinary action in accordance with the provisions in Article Forty-Eight of the contract.
Application for Sick Leave. 21.02.1 The Certified School Psychologist is to notify the Psychological Services Department of their absence. They are to notify their supervisor before the start of the scheduled work day on the day of the absence.
21.02.2 The BOARD shall require each Certified School Psychologist to furnish a written statement on electronic forms provided by the BOARD for use of sick leave upon return from sick leave. If medical attention is required, the employee's statement shall list the name of the attending physician the date (s) he was consulted. The Administration may require a signed physician's statement confirming the need for sick leave, after four (4) consecutive days of absence or where the Certified School Psychologist has established a pattern of absenteeism.
