Responsibilities of Grantee. The Grantee agrees:
A. The Grantee shall request reimbursement only for expenses described in the application attached to and incorporated into this Agreement as Attachment A, and approved by the SLIB and the Attorney General’s Office.
B. The Grantee shall submit a request for reimbursement accompanied by invoices and supported by adequate proof that such obligations are due and owing and have
C. The Grantee shall establish and maintain sufficient internal controls to ensure that Relief Funds are spent in accordance with this Agreement, SLIB rules, and all State and federal laws.
D. If OSLI or the Inspector General for the U.S. Department of the Treasury determines that any of the Relief Funds were not utilized for an eligible expense under the CARES Act, the Grantee shall repay such funds immediately to the SLIB. In the event the Grantee does not repay the grant funds, the obligation shall be booked as a debt of the Grantee owed to the State of Wyoming. The Grantee further agrees to provide OSLI, upon request, a full and complete accounting as to the use of the Relief Funds; said accounting to be done in accordance with generally accepted accounting principles and shall be provided to the OSLI within a reasonable time.
E. OSLI, or another approved designee of the SLIB, may perform an audit or examination of the books and records of the Grantee at any time and without notice, and that the SLIB or its designee may at any time without notice perform on-site visits and inspections of the project being funded.
F. The Grantee shall comply with all applicable state and federal laws, rules, and regulations, including compliance with the provisions of Wyo. Stat. § 16-6-1001 and all applicable state procurement laws.
G. The Grantee understands that the Legislature approved the distribution of Relief Funds for the purpose of “combating COVID-19 in Wyoming[.]” 2020 Spec. Session 1, SEA No. 001, Section 1(b)(i). The Grantee agrees that it will not remove any equipment or other items purchased pursuant to this Agreement from the State of Wyoming. This provision shall survive the expiration of the term of this agreement identified in Section 3.
Responsibilities of Grantee. Xxxxxxx agrees to:
A. Use the funds for retrofits identified in the WyLite Energy Engineering Study at the facility located 000 Xxxxxxx Xxxxx, Xxxxxxx, Xxxxxxx 00000. Eligible retrofits are described in Attachment A.
B. Return this signed Contract to the Wyoming Energy Authority within thirty (30) days of receipt or forfeit all claims to monies.
C. Provide the Wyoming Energy Authority with one (1) year of required energy savings data once retrofits are completed per Department of Energy requirements.
Responsibilities of Grantee. A. The Grantee shall:
1. Identify, investigate, and respond to cases of acute HAV, HBV, and HCV according to the disease plan:
a) Hepatitis A (xxxxx://xxx.xxxxxx.xxxx.xxx/wp-content/uploads/Hepatitis-A-Disease- Plan-Updates-to-Required-Fields.pdf);
b) Hepatitis B (xxxxx://xxx.xxxxxx.xxxx.xxx/wp-content/uploads/Hepatitis-B-and-D- Disease-Plan.pdf); and
c) Hepatitis C (xxxxx://xxx.xxxxxx.xxxx.xxx/wp-content/uploads/Hepatitis-C-Disease- Plan.docx-5.pdf).
2. Follow project activity guidance documents provided by DHHS during acute HBV and HCV case and case-contact investigations.
3. Follow project activity guidance to provide disease prevention education, harm reduction services and/or referrals, and other referrals to care as needed to acute HBV and HCV cases and case-contacts.
4. Attend >50% of Viral Hepatitis Elimination Committee meetings and support viral hepatitis elimination activities.
5. Identify and provide project activity improvements to DHHS.
6. Upon request, the Grantee will provide to DHHS a detailed budget within 30 days of the request.
Responsibilities of Grantee. 3.1.1 The Grantee shall build and sustain an MRC unit with capabilities which may include the following:
(1) Expand workforce capacity as necessary to support volunteer management;
(2) Build and sustain partnerships with public and private organizations to prepare for and respond to disasters and public health emergencies;
(3) Recruit and retain volunteers;
(4) Update volunteer management plans;
(5) Provide training opportunities to volunteers;
(6) Incorporate volunteers into response exercises; and
(7) Develop leadership sustainability and continuity.
Responsibilities of Grantee. 4.1 Foreign-Trade Zone Forms - COMPANY is authorized to execute in its own name CBP Forms 214 and 216 in its operation of the Usage Driven Site. Without limiting GRANTEE's rights to inspect and audit as provided herein, the parties specifically recognize that GRANTEE is not obligated to, and does not intend to, monitor the day-to-day activity of COMPANY at the Usage Driven Site and there shall be no representation that the GRANTEE has any knowledge, actual or constructive, of the quantity, character, status, designation, identification, or time of admission, transfer or release of goods into or from the Usage Driven Site and that any information is the representation solely of COMPANY and not of GRANTEE.
Responsibilities of Grantee. 4.1 Foreign-Trade Zone Forms - COMPANY is authorized to execute in its own name CBP Forms 214 and 216 in its operation of the Zone Site. Without limiting GRANTEE's rights to inspect and audit as provided herein, the parties specifically recognize that GRANTEE is not obligated to, and does not intend to, monitor the day-to-day activity of COMPANY at the Zone Site and there shall be no representation that the GRANTEE has any knowledge, actual or constructive, of the quantity, character, status, designation, identification, or time of admission, transfer or release of goods into or from the Zone Site and that any information is the representation solely of COMPANY and not of GRANTEE.
Responsibilities of Grantee. A. Ensure the Financial Assistance subject to this Grant Agreement are used in accordance with the conditions, requirements and restrictions of federal, state and local laws, as well as the federal terms and conditions of the grant award and as consistent with the Scope of Work (SoW).
B. Ensure the timely transmission of quarterly reports to the Grantor for the duration of the period of performance necessary to accomplish the SoW. Quarterly reports shall accurately describe all activities for which the Grantee seeks reimbursement and outlay figures reported for the quarter.
C. Promptly reimburse Grantor for any funds Grantee pays to any entity because of an adverse audit finding, adverse quality control finding, final disallowance of federal financial participation, or other sanction or penalty for which Grantee is responsible.
D. Establish an official file for the project. The file shall contain documentation of all actions taken regarding this Grant.
E. Make records available to Grantor, the FRA, and other authorized governmental agencies for review, copy, audit and investigation. Grantee shall also provide access to and allow the interview of any employees who might reasonably have information related to such records. Such access to employees and records shall be provided during normal business hours throughout the Grant term and for at least three years after the final payment is disbursed pursuant to this Agreement, or until completion of any action and resolution of all issues which may arise as a result of any audit or review of such records, whichever is later.
F. Immediately notify the Grantor in writing of any change in local law, conditions, or any other event that may affect its ability to perform the Project in accordance with the terms of this Agreement.
G. Immediately notify the Grantor of any decision pertaining to the Grantee's conduct of litigation that may affect the Grantor ’s interests in the Project. Before the Grantee may name the Grantor as a party to litigation for any reason, the Grantee agrees first to inform the Grantor in writing; this proviso applies to any type of litigation whatsoever, in any forum.
Responsibilities of Grantee. A. For the Immunization Action Plan, the Grantee shall;
1. Develop a one-year Immunization Action Plan with a minimum of three measurable objectives that identify the priority, achievable and affordable activities that the best available evidence indicates, improvise and sustains vaccination coverage level for all residents of the jurisdiction among: infants/children, adolescents, adults and special population.
a) Include the rationale for choosing the objectives and activities;
b) Implement, conduct and monitor outcomes for each objective and activity;
c) Activities shall focus on low and lagging vaccination coverage levels for the population identified;
d) Develop objectives using the SMART format;
e) New objectives shall not be repeated from the previous five- year grant project period and from the past year;
f) New objectives shall correlate with the immunization’s goals in the Utah Health Improvement Plan 2017 – 2020 (xxxxx://xxxx.xxxxxx.xxxx.xxx/pdf/xxxx/publication/UHIP.pdf);
g) Designate and identify a staff member to act as the local Immunization Coordinator who is the primary contact with DHHS.
B. For Perinatal Hepatitis B, the Grantee shall;
1. Designate and identify a staff member to act at the local Hepatitis B Coordinator who conducts case management, follow-up and data reporting/entry into EpiTrax; and
2. Comply with Utah Department of Health and Human Services, Perinatal Hepatitis B Prevention Program Guidelines (xxxxx://xxxxxxxx.xxxx.xxx/wp-content/uploads/PHBPP-Guidelines- 2023-PDF-1.pdf).
C. For Media, the Grantee shall;
1. Develop an immunization media plan in coordination with the local Nursing Director, Immunization Coordinator, and local immunization coalitions;
2. Include the National public awareness immunization campaigns such as the National Infant/Toddler Immunization (April); Utah Adolescent Immunization Awareness (May); Back to School/College (July/August); and Utah Adult Immunization Awareness Month (October). The media plan for adult immunization awareness shall be comprehensive and shall not focus solely on influenza vaccine awareness;
3. Target the following mixed audience demographic;
a. Parents of infants and young children (women 18-35) (National Infant Immunization Week and Back to School);
b. Parents of adolescents (adults 25-54) (Adolescent Week and Back to School);
c. Older adults 50+ (Utah Adult Immunization Month, October).
4. The PIO shall secure media buys based on the media plan that may include TV, radio, web ...
Responsibilities of Grantee. Grantee will perform, or cause to be performed, all Work in accordance with applicable local and state regulations, including obtaining any requisite approvals from regulatory authorities for the installation and use of the Temporary Easement.
