Responsibilities of Organization. The Organization: 5.1. Shall use the funds solely for program operations including forensic interviews pertaining to Converse County; victim advocacy; therapeutic services; and community education and training. 5.2. Shall submit to the County a copy of its annual financial compilation, review, or audit when completed for the end of each fiscal year. This document must be submitted by the Organization along with the current budget funding request. 5.3. Shall submit invoices and any required supporting documentation on a monthly, quarterly, biannual, or yearly basis by the County’s accounts payable deadline. Original invoices shall be submitted by US Mail or by hand delivery to the County Clerk’s Office at the address as stated herein. 5.4. Acknowledges that any allocation of funds not invoiced by the Organization for the current fiscal year and by the deadline will revert to the County’s General Fund and will not be paid to the Organization. 5.5. Shall pay, in a timely manner, all property taxes and any other debts assessed in its name and payable to Converse County. The County may withhold any debts due and owing from the Organization’s final fiscal year allocation. 5.6. Shall carry a policy of commercial general liability insurance, with liability limits in an amount of not less than $1,000,000.00 per person and $2,000,000.00 per occurrence, unless otherwise authorized by the County, and shall provide the County Clerk with a copy of the certificate of commercial liability insurance for the Organization. Organization shall save and hold harmless the County from any and all liability to persons for harm or injury resulting from the services performed under this Agreement.

Operation of a victim advocate center serving county residents to include the following: a. Staff salary while conducting fund raising campaigns; and b. Provide educational materials for prevention and awareness campaigns; and c. Accounting services; and d. Day to Day operational expenses, as necessary; and e. Training and operation of a 24-hour HOPE line.
Responsibilities of Organization. 5.1. Organization shall provide all of the following services: 5.1.01. Provide coordinated forensic and comprehensive services for alleged victims of child abuse and neglect in order to minimize trauma to children, to break the cycle of abuse and to xxxxxx a more effective and efficient community response to child maltreatment by doing the following: a. Enhance inter-agency coordination in the investigation and treatment of child maltreatment; and b. Minimize further trauma to the child victim and non-offending family members by training professionals and coordinating services in a centralized, child and family- friendly environment; and c. Improve the quality of evidence collection; and d. Increase community awareness of child maltreatment; and e. Serve as a multi-disciplinary resource for the expansion of this program to other communities in the state.
Responsibilities of Organization. 5.1. Organization shall provide all of the following services: 5.1.01. Advocacy of partnership with individuals and communities to deliver professional mental health and substance abuse treatment, education, and consultation designed to enhance the strengths and well-being of the persons served, including, but not limited to: a. Individual and group counseling and family therapy; and b. Substance abuse evaluations for the courts, Department of Family Services, Division of Vocational Rehabilitation, schools, and other as necessary; and c. Pre-employment evaluations; and d. Case management services for seriously and persistently mentally ill adults and seriously emotionally disturbed youth; and e. Successful Families Program; and f. Emergency on-call availability, 24 hours a day, 7 days a week, for all residents of Converse County; and g. Specialized treatment for trauma victims; and h. Alcohol abuse counseling and education; and i. Intensive Outpatient Treatment for substance abusers; and j. Group Home with Crisis Room; and k. Consultation with other not-for-profit organizations; and l. Development, implementation and monitoring of Employee Assistance Programs for business and governmental agencies; and m. Crisis intervention services for local emergency medical, fire and law enforcement personnel; and n. Prevention and education programs for schools, the Wyoming Law Enforcement Academy, nursing/medical professionals, nursing home personnel, and other not-for- profit organizations, including but not limited to staff in-service training on stress management, substance abuse issues, and depression; and o. Specialized counseling, education and monitoring services to the elderly, pregnant females, victims of trauma, and hearing impaired; and p. Purchase of necessary computer equipment and upgrading of Electronic Health Record software; and q. Building and grounds maintenance and upgrades.

Responsibilities of Organization. 4.1. Organization hereby represents and warrants that: 4.1.1. Any Treatment Data provided to ASHA has been collected by Organization and transmitted to ASHA in compliance with all applicable laws, rules, and regulations (including the Health Insurance Portability and Accountability Act of 1996 as amended); 4.1.2. Organization is authorized and has all necessary rights to collect any Treatment Data transmitted to ASHA pursuant to this Agreement and to export, transmit, and provide ASHA with access to such data as required hereunder; 4.1.3. The Treatment Data will be current, accurate and complete as of the date it is transmitted to ASHA; and 4.1.4. Organization will use NOMS and the NOMS Data in strict compliance with all applicable laws, rules, and regulations. 4.2. Organization hereby grants, and agrees to xxxxx, XXXX and all other Users of NOMS, a royalty-free, perpetual, irrevocable, transferable, sublicenseable, worldwide, non-exclusive license under all of Organization’s and Organization’s Subscribers’ rights in and to such data, to copy, publicly display, store and retrieve in a database, extract, create derivative works of, report, publish and otherwise fully exploit all such data, including for the Purpose and for purposes of operating, improving, supplementing, updating, or otherwise modifying or maintaining NOMS or the NOMS Data. 4.3. Organization shall be liable to ASHA for all acts and omissions of Subscriber and each of its Users in connection with or relating to NOMS or the NOMS Data, and the license grant in this Agreement. 4.4. If Organization chooses to transmit Treatment Data via electronic means, Organization may permit a third- party service provider (“Third Party Provider”) to incorporate and configure the NOMS data fields into Organization’s electronic medical record pursuant to the license set forth in Section 3.2, provided that Organization contractually binds such Third Party Provider to confidentiality and security obligations that are at least as stringent as those set forth in Section 10. Such Third Party Provider shall be deemed the agent of Organization, and Organization shall be liable to ASHA for the Third Party Provider’s non-compliance with confidentiality obligations and other material portions of this Agreement and for all acts and omissions of the Third Party Provider.
