Scope of Contractor Services. The Contractor Services are described in Attachment 1 to this Agreement. The Contractor shall, in Attachment 1, identify specific personnel who will be assigned Contractor Services along with a description of the Contractor Services to be performed or provided by personnel identified by the Contractor. Personnel identified by the Contractor for portions of the Contractor Services shall be subject to the District’s approval. The Contractor shall provide all labor, materials, equipment, tools and other items necessary to complete the Contractor Services and authorized Additional Contractor Services without adjustment of the Contract Price.

Scope of Contractor Services. Contractor shall furnish services as described in Appendix B of this Agreement attached hereto and hereby incorporated into this Agreement. In signing this Agreement, Authority grants Contractor specific authorization to proceed with work described in Appendix B. All work authorized by this Agreement shall be completed in accordance with the established project schedule as set forth in Appendix B. Contractor shall:
a. Perform Contractor's duties to the best of Contractor's ability and in accordance with the generally accepted professional and ethical standards of Contractor's profession and community. Contractor agrees to perform Contractor's duties at all times in strict accordance with currently approved methods and practices in Contractor's field and in accordance with the standards required by the Authority. All duties shall be performed and rendered in a competent, efficient, timely and satisfactory manner.
b. Observe and comply with all applicable laws, ordinances, codes and regulations of governmental agencies, including federal, state, municipal and local governing bodies, including all provisions of the Occupational Safety and Health Act of 1979 and all amendments thereto, and all applicable federal, state, municipal and local safety regulations. All services performed by Contractor must be in accordance with these laws, ordinances, codes and regulations. The Authority recognizes that Contractor's ability to complete the scope of work within the budget and timelines established in this Agreement is necessarily dependent on receiving reasonable, timely and adequate input and review from the Authority. The Agreement also assumes that the Authority's policies, goals, objectives, actions or alternatives will not change in a manner which alters the Scope of Work once it has been initially defined and accepted by the Authority. Should the Scope of Work defined by this Agreement be expanded or contracted or the schedule be changed, Contractor's compensation shall be adjusted to reflect any additional work or costs. Scope of Contractor Services. The Contractor will perform the following tasks for this project. The Contractor shall be expected to work closely with affected bureau personnel to accomplish these goals.
1. Conduct a telephone survey of BDS permit and land use customers during July and August 2006 using a City- supplied, pre-tested survey. The survey will consist of a minimum of 675 interviews stratified approximately as follows: • 200 Residential Building permit customers • 200 Commercial Building permit customers • 125 Trade permit customers • 150 Land use customers
2. Average interview length will be 12 minutes, with numbers to be called at least 5 times.
3. Code one open-ended question for everyone as well as three follow-up explanation questions for people who report problems with City services. Verbatim listing of responses.
4. Prepare an electronic data file.
5. Tabulate data and prepare approximately 5 to 10 cross-tabs and 40 to 50 graphs/charts with significance testing and comparison with data from previous years.
6. Prepare a report showing initial survey results and specific data required for the City Auditor’s Service Efforts and Accomplishments report.
7. Prepare a detailed final survey report (including cross-tabs, graphs, charts, and analysis) with an executive summary.
8. Prepare a separate report identifying: • Key predictors of customer satisfaction; • Areas where customer satisfaction has improved significantly from the previous year; • Areas where customer satisfaction has decreased significantly from the previous year or has been consistently low.
9. Prepare a separate report showing the disposition of calls and attempted calls (i.e., numbers of re-calls, no answers, refusals).
10. Provide project management and quality control services. Deliverables shall be considered those tangible resulting work products that are to be delivered to BDS such as reports, draft documents, data, interim findings, drawings, schematics, training, meeting presentations, final drawings and reports. Deliverables and schedule for this project shall include:

Scope of Contractor Services. The Contractor agrees to provide the following:
a. Storage on premises of Contractor located at 0000 Xxxxxx Xxxx 00, Xxxxxxx, XX 00000 of gravel material excavated by the County from various locations within Ouray County. There will be no charge to the County for such storage.
b. Contractor will crush gravel excavated by the County to County specifications.
c. Contractor may, at the request of the County Road and Bridge Supervisor, haul excavated materials to locations specified within Ouray County, Colorado at an hourly rate not to exceed $65.00.
d. The County will provide a loader for loading the crushed materials. No one shall operate the loader except for designated County personnel. Contractor shall provide space on-site for storage of the loader when it is not in use. All services shall be performed in accordance with the terms and conditions of this Agreement.
Scope of Contractor Services. This bid is for the preventive maintenance and repairs to the HVAC Systems, ice machine equipment and related items located at Maryland State Highway Administration Xxxxxx Xxxxx maintenance facility and satellite facilities. The work includes all labor, materials, parts, service, equipment and transportation to ensure the satisfactory performance and efficient operation of the heating, ventilating, air cooled chiller, air conditioning equipment and ice machine, including any gas, oil, propane or electrical services. The Contractor shall inspect and provide service to all systems equipment in accordance with the manufacturer’s recommendations and the procedures listed below (Attachment IV). The bid for preventive maintenance is for all equipment listed in the Contract including parts to complete the service. Preventative Maintenance at the Xxxxxx Xxxxx maintenance facility will be performed four (4) times per year (quarterly): • Quarterly PM (Attachment IV) • Quarterly and Annually PM • Quarterly and Seasonal Changeover PM in September • Quarterly and Seasonal Changeover PM in April Since the satellite facilities are only heated, the only preventative maintenance will be the September changeover. All other work will be charged at time and materials. The Contractor shall maintain detailed service logs on the performance of the equipment at the site. Service logs shall include the date of inspections, name of the inspector, and a complete listing of all work performed and service furnished. Contractor shall update logs at the conclusion of each site visit and return logs to the SHA authorized representative.
