Leaves of absence with pay of up to five (5) working days shall be granted to an employee by his or her immediate Supervisor on time card approval, upon satisfactory evidence of reasonable cause such as serious illness in the immediate family. The immediate family shall be defined as Spouse, Parent, Sibling, Child, Grandparents and Grandchildren, Guardian in loco parentis, In-Laws in the above categories, where applicable. No deductions from vacation or sick leave time shall be made in this regard. Upon showing of reasonable continuing cause such leave may be extended to a maximum often (10) days. The excess above five (5) days shall be charged against the employee’s earned vacation time. In the event like circumstances reoccur within a single year, an additional four (4) days, with pay, may be authorized by the Superintendent of Schools or Superintendent’s Representative, upon written request. Unused family illness days outlined in this Article will be added to the accumulated sick leave day’s account at the end of the calendar year.

  • Death in Immediate Family A regularly scheduled employee may be granted up to five days of leave of absence with pay by the Agency/Department Head because of death in the immediate family. An employee shall be allowed to take such leave within a four week period. For purposes of this subsection, "immediate family" means mother, stepmother, father, stepfather, husband, wife, domestic partner (upon submission of an affidavit as defined in the appendices), son, stepson, daughter, stepdaughter, brother, sister, grandparent, grandchild, xxxxxx parent, xxxxxx child, mother-in-law, and father-in-law, or any other person sharing the relationship of in loco parentis; and, when living in the household of the employee, a brother-in-law, sister-in-law. Entitlement to leave of absence under this subsection shall be only for all hours the employee would have been scheduled to work for those days granted, and shall be in addition to any other entitlement for sick leave, emergency leave, or any other leave.

  • Illness in Immediate Family 1. Up to three (3) days of sick leave may be used by a teacher for each serious illness of a member of that teacher's immediate family or birth of a child of a teacher. Serious illness shall mean an illness where death is probable and may occur, surgery is performed requiring hospitalization, or illness requiring treatment by a physician. Two (2) additional days of sick leave may be used where round trip travel is 600 miles or more.

  • Death in the Immediate Family The teacher may take a maximum of five (5) sick days per death at the time of the death. Immediate family shall be interpreted as mother, father, husband, wife, grandparents, grandchild, child, sister, and brother of teacher and/or his/her spouse. One (1) of these days must be the funeral day.

At the City's request the employee will provide satisfactory evidence of the facts justifying such absence.

  • Death in Family At the request of the employee, the Employer may grant a leave of absence for other purposes. The terms and conditions of all leaves of absence shall be set forth in writing. The Employer shall grant an automatic leave of absence, if so desired, not to exceed two (2) weeks, in cases of critical illness or injury or death in the employee's immediate family. Any period in excess of two (2) weeks shall require the written consent of the Employer. When possible, the employee shall request such leaves of absence; but in any event, the Employer shall be notified within twenty-four (24) hours.

Any funds on deposit may be utilized to satisfy any debt or garnishment of any Owner of the Account. It is the responsibility of joint account Owners to determine any legal effects of opening and maintaining a joint account.
