Annual Examination. 1. The City shall provide a physical examination conducted by a health professional annually for all unit employees to include a stress EKG reviewed by a cardiologist. The health professional will evaluate all employees’ EKG, fitness, and blood chemistry plan results and will provide a confidential summary to each employee. Employees are responsible for adhering to the recommendations indented by the assessment team and any follow-up recommendations from the cardiologist, for actually participating in the lectures series and for reading the literature provided by the City.

Annual Examination. The City shall provide a stress EKG and appropriate related physical examination annually for all unit employees.
Annual Examination. In light of the unique nature of the professional duties of SUPERINTENDENT, DISTRICT shall, at its option and expense, provide a complete medical examination of SUPERINTENDENT prior to April of each school year. SUPERINTENDENT shall notify the BOARD in writing annually of this requirement no later than the last BOARD meeting in March. The examination shall be conducted by a physician chosen by the SUPERINTENDENT. The DISTRICT shall be advised, in writing, by the physician of the continued physical fitness of SUPERINTENDENT to perform her duties, and such report shall be confidential and placed in the SUPERINTENDENT’S medical file.
Annual Examination. The Board of Directors shall provide an annual physical or eye examination at a cost not to exceed ($400) after submission of claim under the medical insurance coverage provided under this contract. The physician shall be the choice of the administrator. The ($400) may be applied towards eye exams and glasses.
Annual Examination. Practice will offer Patient with a comprehensive annual routine diagnostic executive-level physical examination that is delivered regardless of medical necessity or condition (“Annual Exam”) to identify health conditions requiring care, and to create a baseline of health information and health-related goals to assist Practice with providing Patient annual health goal support and education. For Basic and Premier Plan patients, the Annual Exam will include evaluation of: ECG testing, full lab work with more extensive tests that includes the NMR of the cholesterol particles and evaluation of the size of LDL fat particle, pharmacogenetic testing, hearing test with audiometer, vision test, pap smear or PSA testing, nutrition guidance, and a treadmill stress screening to monitor heart and blood pressure. Testing may require additional fees. The Annual Exam will integrate lifestyle, nutrition, fitness, mental health, and other related information provided by Patient, and create specific health goals for the year based on the Annual Exam. Basic Plan patients will receive acupuncture treatments at a reduced rate. Basic Plan patients who are under 62 years old, and lack a permanent disability, also will have the option to utilize the Specialty Consult service at any time throughout the year for an additional fee. Patients who are 62 years old or older, or, who are permanently disabled, must subscribe to the Premier Plan and will never be charged additionally for Annual Exam-related office visits. For Premier Plan patients, the BPC Program will include everything listed above, but will also include the cost of a basic set of wellness labs and the option of in-depth hormone testing and the option of a yearly Integrative Medicine Examination The Yearly Integrative Medicine Consultation (“Integrative Consult”) will be offered to all Premier Plan patients. It will be delivered regardless of apparent medical necessity on an annual routine basis, fully incorporate integrative health guidance, and will be available by phone or in person and include evaluation and recommendation of nutritional dietary advice, supplements, and vitamins relevant to the Patient’s health conditions. The Integrative Consult will include instruction on mind-body interventions such as meditation, mindfulness and Tai Chi. Annual Examination. For purposes of fostering the total health and well-being of the Employees, each Employee is encouraged to undergo an annual medical examination. However, if an Employee selects a health care provider out of network, the rules of the City’s health care plan governing out-of-network providers shall govern the cost of the medical examination. The benefit is maximized if an Employee obtains his/her annual examination at the Whitehall Employee Health and Wellness Center as described above. No Employee shall be required to share the results of the medical examination with the City.

  • Tax Examinations Abroad 1. A Contracting Party may allow representatives of the competent authority of the other Contracting Party to enter the territory of the first-mentioned Party to interview individuals and examine records with the written consent of the persons concerned.   • Physical Examinations Where the Employer requires an employee to take a physical examination, doctor's fees for such examination shall be paid by the Employer.
