Benefit Information. 14.19.1 For more information in addition to the details on Employee Benefits generally described in this Agreement, employees are directed to the booklets provided by the appropriate benefit carrier(s), which the College will provide to all eligible employees. 14.19.2 The College will ensure that a copy of all benefit insurance contracts applicable to the employees are available for their review at each work site. 14.19.3 Upon request, a copy of any applicable benefit insurance contract will be supplied to employees. 14.19.4 Employees who elect not to participate in College benefit plans because they have coverage elsewhere, will be able to participate at the time their other coverage ends in accordance with the terms of the policies. 14.19.5 The College will notify the appropriate insurance companies or agencies upon the death of employees.

Such information shall be made available in a one-page, two-sided summary format, distinguishing between services available pursuant to the State’s approved 1915(b) and 1915(c) home and community-based waivers; (b) services for which prior authorization or a referral is required, and the method of obtaining both; (c) any restrictions on the Member’s freedom of choice among Network Providers; (d) the CONTRACTOR’s policy on referrals for specialty care, long- term services, and other benefits; (e) information regarding the Member’s right of access to and coverage of emergency services which include: (i) the fact that the Member has a right to use any hospital or other setting for emergency care; and (ii) what constitutes emergency medical condition, emergency services, and post-stabilization services; and (f) information that provides potential Members, upon request, and enrolled Members with a list of all items and services that are available to Members covered directly or through a method of referral and/or prior authorization. This material must be available in an easily understood manner and format. (2) The CONTRACTOR shall send out a questionnaire within thirty (30) calendar days of enrollment to all new Members which must include a question regarding the new Member’s primary language spoken and/or written. The CONTRACTOR shall make a good faith effort to obtain this information. (3) The CONTRACTOR shall provide affected Members and/or legal guardians with written updated information within thirty (30) calendar days of the intended effective date of any material change unless regulation, rule or LOD requires the information to be provided within a shorter timeframe. In addition, the CONTRACTOR must make a good faith effort to give written notice of termination of a Network Provider, within fifteen (15) calendar days after receipt or issuance of a termination notice, to each Member...
