Flexible Benefit Account. The BOARD shall make available a flexible benefit account to all full-time Employees eligible for participation in the school district insurance program. The annual date of the Flexible Benefit Account shall be from January 1st to the following December 31st.
Flexible Benefit Account. The Board agrees to have a Flexible Benefit Account (IRS 125 Plan) and to pay all fees except the monthly maintenance fee for each employee.
Flexible Benefit Account. The District has established a flexible benefit plan based on IRC Section 125 to pay eligible non-reimbursable health costs, dependent care costs, and premiums. The plan will be administered by a mutually agreeable disinterested third party. Participating employees will pay a fee to administer their individual accounts. Participation will be voluntary. The plan includes provision for employee orientation and education regarding plan use.

Flexible Benefit Account. Metro Bus shall provide a pre-tax benefit whereby eligible medical expenses and dependent care expenses may be deducted from wages on a pre-tax basis and claims made for those accounts are submitted for reimbursement.
Flexible Benefit Account. Paraeducator’s shall be eligible to participate in Anoka-Hennepin Flexible Benefit Plan in accordance with Anoka-Hennepin plan description and IRS Rules.
Flexible Benefit Account. The Board will provide four hundred dollars ($400.00) per year for the employee, spouse or dependents to use for dental, vision, or prescription cost reimbursement. The employee is to pay the first twenty percent (20%) of the cost of the service provided. All claims are to be submitted to the Third Party Administrator who will process the payments. This amount does not carryover from one year to the next.
Flexible Benefit Account. Town agrees to provide an IRC Section 125 Plan that provides for the payment of unreimbursed medical premiums, costs for medical and dental care not covered by current insurance plans, and dependent care costs with pre-tax dollars allowed by Federal tax law. If the employee leaves Town employment before reimbursing the Town for the amount borrowed, the employee agrees to repay the Town in a timely and responsible manner.
Flexible Benefit Account. Subject to the provisions in this section, each eligible employee shall be credited each month with an amount equal to the single premium amount for the Option 1 health insurance plan with pre- authorization. Employees contracted to work at least thirty (30) hours per week for fifty-two (52) weeks shall also be credited each month with an additional fifty dollars ($50). Each eligible employee shall execute an annual election form for the benefits and/or additional salary from the following options subject to the provisions, terms and conditions of the District’s salary reduction plan and the provisions, terms and conditions of the insurance policies and plans.
1. Option 1 health insurance plan ($500/$1,000 deductible) with preauthorization (single or family).
2. Option 2 health insurance plan ($2,500/$5,000 deductible) (single or family).
3. A group term life insurance policy of $10,000.
4. Group dental plan (single or family) provided the carrier's enrollment requirements are maintained.
5. Additional salary with the following conditions met:
a. New eligible employees hired to begin work on or after July 1, 2006, and eligible employees moving from a nonbenefitted to a benefitted position who were not in a benefitted position within the prior 15 months, must elect either Option 1 or Option 2 District group health insurance benefits (single or family). If such an employee does not elect Option 1 or Option 2 District group health insurance benefits, the employee shall not receive any flexible benefit credit.
b. All eligible employees on the District’s group health insurance plan as of June 30, 2009, must continue to elect Option 1 or Option 2 District group health insurance benefits (single or family). If an employee on the District’s group health insurance plan does not elect Option 1 or Option 2 District group health insurance benefits, the employee shall not receive any flexible benefit credit.
c. Every eligible employee who is not participating in the District’s group health insurance plan shall provide satisfactory evidence during the open enrollment period each year that the employee is otherwise covered by a group health insurance plan through (i) the employer of the employee’s spouse, or (ii) a group plan of the employee’s other place of employment, or (iii) a group plan qualified as a creditable plan by the District’s group health insurance carrier.
