Medical and Dental Care. 7.1 Medical Care - During the term of this Agreement, The City shall provide a Medical Care Program to all eligible employees and their dependents under conditions of the medical care contracts between the City and Group Health Cooperative of Puget Sound which are applicable to employees covered by this Agreement and which are in effect upon the execution of this Agreement by both parties. Also, the HMA Traditional and Aetna Preventative self-insured plans which are applicable to employees covered by this Agreement and which are in effect upon the execution of this Agreement by both parties.
A. The medical care contracts between the City, Aetna and Group Health Cooperative of Puget Sound which is only available to employees covered by this Agreement with modifications to benefit levels and costs from the 2012 plan year as identified herein; the City and the Union agree to split the Local 77 monthly health care premium costs; the City shall pay 90 percent and employees shall pay 10 percent of such costs. Employees who elect Local 77 health care plans will receive coverage enhancements to comply with the requirements of the Patient Protection and Affordable Care Act, and the City will implement that following changes to copay costs: Office visit copay will increase from $5 to $10 Retail Rx copay will increase from $5/10/25 to $10/20/40 Mail Rx copay will increase from $10/20/50 to $20/40/80 Retail Rx copays will increase from $8 to $15 Mail Rx copays will increase from $16 to $30 Office visit copays will increase from $5 to $10 Retail Rx copays will increase from $5 to $10 Mail Rx copays will increase from $15 to $30 and which is in effect upon the execution of this Agreement by both parties; or
B. The medical care contracts between the City, Aetna and Group Health Cooperative of Puget Sound provided to “Most Employees” health care plan, enhanced for Local 77 members to comply with the requirements of the Patient Protection and Affordable Care Act. Employees who elect such plan will have the current (as of 2/18/14) cost sharing arrangement as other participants in the “most employees” health care plan in years 2-4 of this agreement. Employees who elect the “most employees” health care plan may not elect to return to the health care plan contracts identified in Article 7.1.
C. The City will hold an open enrollment period as soon as practicable after signature of this agreement so that union members may elect coverage of health care plans. This shall be consid...
Medical and Dental Care. 1) The Contractor must assure that each youth receives routine and non- routine medical and dental care as required in MDHHS JRM Policy and the DCWL standards for the license specified in Section 2.4 of this contract. The Contractor must provide all medical and dental information to the assigned primary caseworker/agency responsible for placement to facilitate maintenance of the Medical Passport (DHS-221). In addition, the Contractor must assure that specific health care is provided, including:
a) Rehabilitative, physical, or dental procedures by medical personnel as necessary.
b) Utilization of enrolled Medicaid providers or a board-certified physician or dentist volunteering his/her time for health procedures.
c) Provision of medication as prescribed by a treating physician. Agency must have a Standard Operating Procedure for dispensing and storage of medication in accordance with JRM policy.
d) Special diets provided as needed and regularly reassessed utilizing appropriate specialized personnel. Any youth who is determined to be obese/underweight must have a plan to address their weight, health and well-being.
e) Registered or licensed nursing staff on-site and/or available 24/7. The nursing staff must be available within 60 minutes to the Contractor at all times. The Contractor may sub-contract the nursing services required in this Contract.
2) The Contractor must upload the above required examination reports and assessments to MiSACWIS/CCWIS within five working days of completion.
Medical and Dental Care. A) The Contractor shall provide access to medically necessary Medical and Dental care for each child.
B) The Contractor shall provide access to the preventive medical services recommended by the most current version of the Texas Health Steps periodicity schedule found in Section 42.2.2 of the Texas Medicaid Providers Manual at xxxx://xxx.xxxx.xxx/default.aspx. In the event an annual medical exam is not required by the Texas Health Steps periodicity schedule, a well child exam must, at a minimum, be provided for that year.
C) The Contractor shall access Medicaid for covered services available to eligible children.
D) In the event that neither community nor Medicaid resources are available to fund recommended Medical or Dental Care, the Contractor shall notify the Department’s caseworker for assistance.
E) In the event the Contractor has any questions or concerns regarding the prescribed recommendations for follow-up treatment, the Contractor shall raise these concerns/questions with the Department and the Department will assist the Contractor with a resolution.
Medical and Dental Care. 7.1. The Customer will guarantee that an appropriate health insurance is taken out in favour of all VT personnel prior to their arrival to the Czech Republic for the sake of eventual injury, illness, or death on the territory of the Czech Republic.
7.2. The Supplier will arrange the provision of medical or dental care to the VT personnel in HN medical or dental facilities if requested, or in case of need.
7.3. Medical and dental care provided to the VT personnel in the HN medical and dental facilities will be paid in accordance with the HN laws and regulations.
7.4. The Supplier will provide necessary assistance and support to any VT personnel required to stay behind in the HN for medical reasons after the departure of the VT.
Medical and Dental Care. 5.1. The Customer shall guarantee that an appropriate health insurance is taken out in favour of all CBRND Centre personnel prior to their arrival to the Czech Republic for the sake of eventual injury, illness, or demise on the territory of the Czech Republic.
5.2. The Contractor will arrange the provision of medical or dental care to the CBRND Centre personnel in the Czech Republic medical or dental facilities if requested, or in case of need.
5.3. Medical and dental care provided to the Customer personnel in the Czech Republic medical and dental facilities will be paid in accordance with the Czech Republic laws and regulations.
5.4. The Contractor will provide necessary assistance and support to any CBRND Centre person required to stay behind in the Czech Republic for medical reasons after the departure of the CBRND Centre training participants back home.
Medical and Dental Care. 5.1. The Customer shall guarantee that an appropriate health insurance is taken out in favour of all (LTFO) personnel prior to their arrival to the Czech Republic for the sake of eventual injury, illness, or demise on the territory of the Czech Republic.
5.2. The Contractor will arrange the provision of medical or dental care to the (LTFO) personnel in the Czech Republic medical or dental facilities if requested, or in case of need.
5.3. Medical and dental care provided to the Customer personnel in the Czech Republic medical and dental facilities will be paid in accordance with the Czech Republic laws and regulations.
5.4. The Contractor will provide necessary assistance and support to any (LTFO) person required to stay behind in the Czech Republic for medical reasons after the departure of the (LTFO) training participants back home.
Medical and Dental Care. For employees covered by this Agreement who are entitled to medical coverage under State Statute RCW 41.26, the City shall provide a medical and dental care program, as established by the City, for the eligible dependents of eligible employees under conditions of the medical care contracts applicable to employees covered by this Agreement and which were in effect upon the effective date of this Agreement. For employees covered by this Agreement who are not entitled to medical coverage under State Statute RCW 41.26, the City shall provide a medical and dental care program, as established by the City, for eligible employees and their eligible dependents under conditions of the medical care contracts applicable to employees covered by this Agreement and which were in effect upon the effective date of this Agreement. Effective January 1, 2002, the City shall provide medical, dental and vision benefits for all regular employees (and eligible dependents) represented by unions that are a party to the Memorandum of Agreement established to govern the plans. For calendar years 2002, 2003 and 2004, the selection, addition and/or elimination of medical, dental and vision benefit plans , and changes to such plans including, but not limited to, changes in benefit levels, copays and premiums, shall be established through the Labor-Management Health Care Committee in accordance with the provisions of the Memorandum of Agreement established to govern the functioning of said Committee.

In addition, the Contractor must assure that specific health care is provided, including:
a) Rehabilitative, physical, or dental procedures by medical personnel, as necessary.
b) Utilization of enrolled Medicaid providers or a board-certified physician or dentist volunteering their time for health procedures.
c) Provision of medication as prescribed by a treating physician. The contractor must have a Standard Operating Procedure for dispensing and storage of medication.

2) The Contractor must forward the above required examination reports and assessments to the primary caseworker/agency within five working days of completion.
Medical and Dental Care.
13.1 Medical
Medical and Dental Care. Emergency health assistance and consultations provided by the military medical services and evacuations by military transport, where available, will be free of charge. Medical and dental care will be provided under the same conditions as for the HN Armed Forces members in accordance with Paragraph 5 of Article IX of the NATO SOFA. All VF personnel deployed to the HN territory on the basis of this TA will arrange appropriate health insurance prior to their arrival to the territory of the HN for reasons of payment of possible medical treatment costs. Costs incurred from the public healthcare providers for VF personnel will be covered by the relevant VF under the prices applicable in the HN at the time. HN will provide one medical NCO as liaison personnel, who will help with the administrative work in case of any medical assistance. The VF will be responsible for ensuring that their personnel is fit in terms of health for the participation in the Exercise and that they don’t have any physical or personal limitations in this context. The individual participants of the VF will deploy with their individual chronic medication. Where assistance will be required from the HN to transfer injured personnel to a hospital of the VF’s choice using HN assets, the VF will accept responsibility for any reimbursement required for the transportation costs involved. Strategic aeromedical evacuation will be a VF responsibility. HP will provide assistance in order to help with contacts with public healthcare providers of the HN.
