Insured Benefits - Full-Time Employees a) The Hospital shall contribute on behalf of each eligible employee, who have elected to be covered, seventy-five percent (75%) of the billed premium under the Extended Health Care Plan consisting of fifteen dollars ($15.00) single and twenty-five dollars ($25.00) family deductibles. The Hospital shall contribute 100% of the billed premium for a semi-private Hospital insurance plan. b) The Extended Health Care plan shall include a Hearing Aid allowance for acquisition of up to $500.00 every 36 months per individual, and Vision Care of $200.00, plus bi-annual eye exams, every twenty- four months per individual. c) Eligible employees, who have elected to be covered, shall be entitled to participate in the group dental plan (Blue Cross #9 Current ODA Schedule, or its equivalent) subject to the terms and conditions of the Plan. Effective December 20, 2001, the plan shall provide for dental recall, including preventative services, to 9 months; Blue Cross rider #2 (or equivalent) [complete and partial dentures] at 50/50 co-insurance to $1000 annual maximum; and Blue Cross rider #4 (or equivalent) [crowns, bridgework, and repairs to same] at 50/50 co-insurance to $1000 annual maximum. The Hospital shall contribute seventy-five percent (75%) of the billed premium towards coverage of participating employees in the Plan. d) The Hospital shall contribute one hundred percent (100%) of the billed premium for group life insurance coverage up to twice annual salary for each eligible full-time employee in the active employ of the Hospital and in this bargaining unit. e) All eligible employees must enroll in the Hospitals of Ontario Pension Plan (HOOPP) in accordance with the regulations of the Plan.

Insured Benefits - Full-Time Employees. The Hospital agrees, during the term of the Collective Agreement, to contribute towards the premium coverage of participating eligible employees in the active employ of the Hospital under the insurance plans set out below subject to their respective terms and conditions including any enrolment requirements: (a) The Hospital agrees to pay 100% of the billed premium towards coverage of eligible employees in the active employ of the Hospital under the Blue Cross Semi-Private Plan in effect as of September 28, 1993 or comparable coverage with another carrier. (b) The Hospital agrees to contribute 75% of the billed premium towards coverage of eligible employees in the active employ of the Hospital under the existing Blue Cross Extended Health Care Benefits Plan in effect as of September 28, 1993 (as amended below) or comparable coverage with another carrier providing for $22.50 (single) and $35.00 (family) deductible, providing the balance of monthly premiums is paid by the employee through payroll deductions. Reimbursement for prescribed drugs covered by the Plan will be based on the cost of the lowest priced therapeutically equivalent generic version of the drug, unless there is a documented adverse reaction to the generic drug. Services of a chiropractor will be covered up to an annual maximum of $300; and services of a licensed registered physiotherapist will be covered up to an annual maximum of $300. Vision care maximum $200.00 every 24 months in addition to eye examinations biennially, and hearing aide acquisition every 36 months. (c) The Hospital agrees to contribute 100% of the billed premium towards coverage of eligible employees in the active employ of the Hospital under HOOGLIP in effect as of September 28, 1993 or such other group life insurance plan currently in effect providing the balance of the monthly premium is paid by the employee through payroll deductions.

Services of a chiropractor will be covered up to an annual maximum of $300.00; and services of a licensed or registered physiotherapist will be covered up to an annual maximum of $300.00. Effective January 1, 2015, the annual maximum for the services of a chiropractor will be increased to $375. Effective January 1, 2015, the annual maximum for the services of a licensed or registered physiotherapist will be increased to $375. In addition to the standard benefits, coverage will include vision care (maximum of $300 every 24 months plus bi-annual eye exams) as well as a hearing aid allowance (cost of acquisition per individual every 36 months). Existing provisions for private duty nursing services contained in present extended health care plans will be amended to reflect that this benefit is limited to a maximum of ninety (90) eight-hour shifts in any calendar year. Related to Insured Benefits - Full-Time Employees

  • Regular Part-Time Employees A regular part-time employee is one who works less than full-time on a regularly scheduled basis. Regular part-time employees accumulate seniority on an hourly basis and are entitled to all benefits outlined in this Collective Agreement. Regular part-time employees shall receive the same perquisites, on a proportionate basis, as granted regular full-time employees.

  • Part-Time Employees Employees who are scheduled to work less than forty (40) hours per workweek.

  • Regular Full-Time Employees A regular full-time employee is one who works full-time on a regularly scheduled basis. Regular full-time employees accumulate seniority and are entitled to all benefits outlined in this Collective Agreement.

  • Regular Part-Time Employee A regular part-time employee is an employee hired to fill a posted part-time position and is regularly pre-scheduled to work.

  • Shift Employees Employees who work rotating shift patterns or those who work qualifying shifts shall be entitled, on completion of 12 months employment on shift work, to up to an additional 5 days annual leave, based on the number of qualifying shifts worked. The entitlement will be calculated on the annual leave anniversary date. Qualifying shifts are defined as a shift which involves at least 2 hours work performed outside the hours of 8.00am to 5.00pm, excluding overtime.   • Part-Time Employee Part-time employee" means an employee who is normally scheduled to work fewer than 80 hours in a biweekly payroll period.

  • Full-Time Employees A full-time employee is one engaged as such and whose ordinary hours of work average 38-hours per week.

  • Overtime-Eligible Employees Employees who are covered by the overtime provisions of state and federal law.

  • Part-Time Employment Monthly compensation for part-time employment will be pro-rated based on the ratio of hours worked to hours required for full-time employment. In the alternative, part-time employees may be paid the appropriate hourly rate for all hours worked.

  • Special Maternity Allowance for Totally Disabled Employees (a) An employee who: (i) fails to satisfy the eligibility requirement specified in subparagraph 17.02(a)(ii) solely because a concurrent entitlement to benefits under the Disability Insurance (DI) Plan, the Long term Disability (LTD) Insurance portion of the Public Service Management Insurance Plan (PSMIP) or the Government Employees Compensation Act prevents her from receiving Employment Insurance or Québec Parental Insurance Plan maternity benefits, and (ii) has satisfied all of the other eligibility criteria specified in paragraph 17.02(a), other than those specified in sections (A) and (B) of subparagraph 17.02(a)(iii), shall be paid, in respect of each week of maternity allowance not received for the reason described in subparagraph (i), the difference between ninety-three per cent (93%) of her weekly rate of pay and the gross amount of her weekly disability benefit under the DI Plan, the LTD Plan or via the Government Employees Compensation Act. (b) An employee shall be paid an allowance under this clause and under clause 17.02 for a combined period of no more than the number of weeks during which she would have been eligible for maternity benefits under the Employment Insurance or Québec Parental Insurance Plan had she not been disqualified from Employment Insurance or Québec Parental Insurance maternity benefits for the reasons described in subparagraph (a)(i).
