MANAGEMENT AND ORGANIZATION. Xxxxx shall have personnel with executive responsibility to oversee its quality system. Xxxxx also shall maintain an organizational structure which ensures the Product(s) are designed, developed and/or manufactured in accordance with this quality agreement.

Yes, the CBHC program is managed through a semi-decentralized system. CBHC policies and guidelines are approved by the CBHC Department in the MoPH. However, sub-national level offices help manage the CBHC program implementation. Support is provided by: • CBHC Department (MoPH) [central level] • Relevant Technical MoPH Departments [central level] • Provincial Liaison Office [MoPH central level] • Provincial Public Health Office (PPHO) [provincial level] 27 What level of responsibility do regional, state, or local governments have for the program, if any? Note responsibility by level of municipality. At the central level, the MoPH’s CBHC Unit is responsible for oversight, support, and further development of implementation of CBHC program at the national level. Additionally, the MoPH Strengthening Mechanism provides management and oversight for MoPH-implemented provinces. At the provincial level, the Provincial MoPH CBHC officers supervise and monitor the program. The CBHC officer is responsible for coordination among stakeholders, planning and monitoring of the CHBC activities, and reporting to the Provincial Public Health Directors. In provinces where CBHC is implemented by the MoPH, the CBHC officers are referred to as CBHC Coordinators, and are responsible for day-to-day management of the CBHC activities and CHSs report to them. At the local or community-level, CHSs provide supportive supervision to CHWs at HPs. 28 What level of responsibility do international and local NGOs have for the program, if any? Yes, the PES is managed through a decentralized system. The levels of the system are: • Central • Department • Communes—Community Health Unit • Section Communal—Referral hospital • Localities—Health Clinics and ASCs. 26 Is the MOH responsible for the program, overall? The MSPP provides overall strategy, policy, and national adaptation of health policies and holds ultimate authority of the PES. 27 What level of responsibility do regional, state, or local governments have for the program, if any? Please note responsibility by level of municipality. At the central level, the MSPP manages all information and implements standards and policies. At the commune level, Community Health Units (CHUs) are responsible for the full implementation of the PES on behalf of the MSPP. The CHUs are responsible for coordinating the PES activities of various actors, particularly ASCs. They ensure coverage for all catchment areas, appropriate allocation of resources, and ensure appropriate use of referrals systems. HAITI COMMUNITY HEALTH PROGRAMS 28 What level of responsibility do international and local NGOs have for the program, if any? In collaboration with the MSPP, INGOs and local NGOs implement the PES and supervise ASCs. INGOs and NGOs follow MSPP service provision guidelines and work in coordinating with the CHUs to ensure the PES is being implemented under the MSPP authority. Day-to-day PES activities and service delivery is implemented predominantly by INGOs and NGOS.

The PHC is semi-decentralized. The MOH is involved in the development of policies, standards, and guidelines, and oversight via the HSAs. Implementation directed at the district and local levels based on the needs of each community. In some areas, other CHWs (including CHBCs and PEs) are managed from the district level due to the high workload of the HSAs. Management levels include: • National ° Ministry of Health, including the Environmental Health, Reproductive Health, and HIV/AIDS sections • District ° District Council ° District Development Committee ° District Health Management Team • Health facilities ° Health Center Management Committee • Village ° Area Development Committee ° HSAs ° VHCs The service delivery system includes: • Central Hospital • District Hospital • Health Center • Health Post/Village Clinic (HSAs) • Community Outreach (all other CHW cadres) 26 Is the MOH responsible for the program overall? Yes. MOH provides leadership and responsibility for the PLC. Some other CHWs (CGs, PEs, and SPs) are implemented by partner NGOs under the policy guidance of the MOH.

The Superintendent has designated the Concession Specialist as the staff person responsible for day-to-day concession operations. Law enforcement/visitor protection matters are delegated to the Chief Ranger. Resource Management and Compliance matters are delegated to the Chief of Natural Resource Management and Chief of Cultural Resource Management. Each of these may designate a specific contact person to handle routine matters. Concessioner – Concession operations will carry out the policies and directives of the Service as well as those of the Concessioner in the operation of the authorized concessions services in Buffalo National River. The Concessioner will manage and, if necessary, employ a staff with the expertise to operate all services authorized under the concessions contract. The primary Concessioner contact for the National Park Service will be contact name(s).

Executive Leadership Figure 2 below illustrates SunPlus Management and Functional Organizational Chart for the State Administrative Support contract along with the responsibility flows, Lines of Authority, and communication with the State’s Contracts, Technical and Program Management personnel. XxxXxxx’s executive leadership will provide hands-on executive leadership to our State project team. This will enable us to promptly resolve all issues and conflicts that might arise during the contract execution, thus mitigating the State’s risks associated with operational disruptions and failure. This also demonstrates our total commitment to the success of this contract. XxxXxxx’s PM will directly report to SunPlus’s CEO. SunPlus has designated an Account/Program Manager (PM), and a Quality Assurance Manager (QAM) at no direct cost to the State. We will also establish a Project Management Office (PMO) located at our corporate headquarters at no direct cost to the State. The PMO will assist the PM and the project team in contract maintenance and in meeting the contract objectives and providing superior Administrative support services to the State without any disruptions. SunPlus has instituted a lean, streamlined and effective State project team organization with clear and direct lines of reporting authority and communication. Each project team members’ responsibilities have been defined clearly. More importantly, all project team members including the employees of our subcontractors, will be oriented on the communication protocol, reporting structure, issue logging, escalation and resolution, and time sheets and reports development and submission. Our PM will manage all SunPlus resources including the Project Management Office (PMO) staff, and our State team staff, including the subcontractor employees who are working as a part of our team. The PM will have ultimate authority, responsibility, and accountability to meet the cost, schedule, and technical objectives of the contract without any disruptions on an ongoing basis.
