Phase Deliverables. The following outline is an example list of deliverables that Transoma requires of Mednet. Refinement of the deliverables list shall result from detailed discussions between and schedule development by Mednet and Transoma personnel. CONFIDENTIAL MATERIAL HAS BEEN OMITTED AND FILED SEPARATELY WITH THE SECURITIES AND EXCHANGE COMMISSION, ASTERISKS DENOTE SUCH OMISSIONS • On a quarterly basis and at Transoma’s reasonable request for cause (in any case not more frequently than on a monthly basis), copies of all raw and analyzed patient data obtained by Mednet using Transoma-developed technology. Such patient data shall be “de-identified”, as defined in HIPAA and shall not include any of the following identifiers of the patient or of the patient’s relatives, employers or household members: • Names; • All geographic subdivisions smaller than a State, including street address, city, county, precinct, zip code, and their equivalent geocodes, except for the initial three digits of a zip code if, according to the current publicly available data from the Bureau of the Census: • The geographic unit formed by combining all zip codes with the same three initial digits contains more than 20,000 people; and • The initial three digits of a zip code for all such geographic units containing 20,000 or fewer people is changed to 000. • All elements of dates (except year) for dates directly related to an individual, including birth date, admission date, discharge date, date of death; and all ages over 89 and all elements of dates (including year) indicative of such age, except that such ages and elements may be aggregated into a single category of age 90 or older; • Telephone numbers; • Fax numbers; • Electronic mail addresses; • Social security numbers; • Medical record numbers; • Health plan beneficiary numbers; • Account numbers; • Certificate/license numbers; • Vehicle identifiers and serial numbers, including license plate numbers; • Device identifiers and serial numbers; • Web Universal Resource Locators (URLs); • Internet Protocol (IP) address numbers; • Biometric identifiers, including finger and voice prints; • Full face photographic images and any comparable images; • Any other unique identifying number, characteristic, or code; and • Mednet does not have actual knowledge that the information could be used alone or in combination with other information to identify an individual who is a subject of the information. • On a quarterly basis, the results of a quarterly aud...

As part of delivering usable solutions at the end of each Phase, Laserfiche will also deliver the following: • A product backlog with features detailed as user stories accompanied by acceptance criteria. This will be delivered in a project collaboration tool (Xxxxxx) to ensure the backlog is available to all Laserfiche and WSLCB team members and that the backlog remains a living document during the course of the project. • System documentation detailing the custom and Laserfiche components used to deliver the solution, where they are installed, and basic configuration details.
