PHASE DELIVERABLES. The following outline is an example list of deliverables that Transoma requires of Mednet. Refinement of the deliverables list shall result from detailed discussions between and schedule development by Mednet and Transoma personnel. • On a quarterly basis and at Transoma’s reasonable request for cause (in any case not more frequently than on a monthly basis), copies of all raw and analyzed patient data obtained by Mednet using Transoma-developed technology. Such patient data shall be “de-identified”, as defined in HIPAA and shall not include any of the following identifiers of the patient or of the patient’s relatives, employers or household members: • Names; • All geographic subdivisions smaller than a State, including street address, city, county, precinct, zip code, and their equivalent geocodes, except for the initial three digits of a zip code if, according to the current publicly available data from the Bureau of the Census: • The geographic unit formed by combining all zip codes with the same three initial digits contains more than 20,000 people; and • The initial three digits of a zip code for all such geographic units containing 20,000 or fewer people is changed to 000. • All elements of dates (except year) for dates directly related to an individual, including birth date, admission date, discharge date, date of death; and all ages over 89 and all elements of dates (including year) indicative of such age, except that such ages and elements may be aggregated into a single category of age 90 or older; • Telephone numbers; • Fax numbers; • Electronic mail addresses; • Social security numbers; • Medical record numbers; • Health plan beneficiary numbers; • Account numbers; • Certificate/license numbers; • Vehicle identifiers and serial numbers, including license plate numbers; • Device identifiers and serial numbers; • Web Universal Resource Locators (URLs); • Internet Protocol (IP) address numbers; • Biometric identifiers, including finger and voice prints; • Full face photographic images and any comparable images; • Any other unique identifying number, characteristic, or code; and • Mednet does not have actual knowledge that the information could be used alone or in combination with other information to identify an individual who is a subject of the information. • On a quarterly basis, the results of a quarterly audit for data interpretation accuracy. • On a quarterly basis, Transoma shall provide to Mednet a forecast for service demand and any other...

PHASE DELIVERABLES. Evaluation Report: Final phase report summarizing the findings, recommendations, and the actionable plan approved by the Project Team. This document will list the prioritized action items that need to be implemented before the next project phase and the newly approved baseline to be used going forward. Initial planning steps will also be documented as a starting point for future working sessions with the Project Team. • Lessons Learned Report: Updated documented from the previous phase to include new feedback or recommendations from the discussions with the Project Team. • Ongoing activities and reporting CohnReznick utilizes a very strong project management office (PMO) model to keep (IT) IV&V projects on track and on budget. Our PMO works alongside key client stakeholders and defines and maintains the optimal allocation of resources, assignment of tasks, methodology, and best practice documentation standards to facilitate and manage the on-time and on-budget completion of each project. Typically, the role of our PMO encompasses: • Resource Management – Ensuring the effective and efficient deployment of staff resources to achieve the project objectives and project plan in a timely and quality-based manner, within established budget guidelines. Utilizes tools such as RACI matrices to match resources with project tasks (see below). • Quality Management – Establishing and communicating quality standards to ensure consistency of approach across all project work effort and deliverables. Validating compliance through second-level review of a representative selection of team deliverables. • Performance Management and Reporting – Ensuring agreed-to project timelines and budgets are adhered to via monitoring and timely reporting of project status across geographic locations. Identifying, escalating, and resolving any issues or concerns that may impact “on time and within budget” performance. • Exception and Issue Management and Reporting – Ensuring all findings are discussed with the appropriate local process owner before being finalized and documented by the IV&V team. Identifying and escalating findings or related issues that require the attention of the Project Teams as appropriate.

PHASE DELIVERABLES. Both the WSLCB and Laserfiche teams will strive to deploy a solution that can be pushed into production and used by WSLCB. Every Phase should result in the solution being released into the Test environment, though it is to WSLCB’s discretion when to release code to users. Changes to solutions of already delivered Phases must be treated as new Phases. New Phases will require a formal scope change. As part of delivering usable solutions at the end of each Phase, Laserfiche will also deliver the following: • A product backlog with features detailed as user stories accompanied by acceptance criteria. This will be delivered in a project collaboration tool (▇▇▇▇▇▇) to ensure the backlog is available to all Laserfiche and WSLCB team members and that the backlog remains a living document during the course of the project. • System documentation detailing the custom and Laserfiche components used to deliver the solution, where they are installed, and basic configuration details.
