Recordkeeping and Reporting Requirements. The Operator shall establish and maintain a system to provide storage and ready retrieval of the Operator’s Facility operating data pertinent to this Agreement, including, but not limited to, all information necessary to verify calculations made pursuant to its fee schedule.

Recordkeeping and Reporting Requirements. 4.12.1 The provider shall submit itemized reports, records, and information at the requestof the Department. 4.12.2 The provider shall maintain auditable records for all activities performed under this agreement. Financial records shall conform to Generally Accepted Accounting Principles (GAAP). Such records shall include the following, as applicable: a. The specific number and type of service units provided; b. The number and type of children served (i.e. private pay or subsidy eligible); c. Any and all records necessary for performing a full audit of the provider's performanceunder the agreement; and d. Other relevant records. 4.12.3 The provider shall allow the Department or its authorized representativeto inspect and examine the provider's premises and/or records which relate to the performance of the agreement at any time during the period of the agreement and thereafter within the period specifiedhereinfor the provider’s retention of records. 4.12.4 The provider shall promptly provide the Departmentwith access to subsidy eligible children and records of subsidy eligible children without limitation. 4.12.5 The provider shall retain all records pursuant to 13 CSR 35-32.130. Records pertaining to the agreement shall be retained for five (5) calendar years after the close of the agreement year unless audit questions have arisen or any legal action is contemplated or filed within the five year (5) limitation and have n ot been resolved. All records shall be retained until all auditquestions and/or legal actions have been resolved. The provider shall safeguard and retain such records for such additional time as directedby the Department. The obligation of the provider to retain and produce records shall continue even after the agreement expires or is otherwise terminated by eitherparty. Retention and production costs shall be the responsibility of the provider.
Recordkeeping and Reporting Requirements. 7.6.1. The Contractor must maintain updated Grievance and Appeals system information and abide by all Grievance and administrative hearing decisions. 7.6.2. The Contractor must be able to submit upon request Individual-level information in a spreadsheet format to Great Rivers that will contain at least the following information regarding each grievance received by the Contractor. 7.6.2.1. Clients full name, date of birth, and P1 or CIS identifier. 7.6.2.2. Date and type of grievance. 7.6.2.3. Outcome of review of grievance process. 7.6.2.4. Plan for improvement, if patterns of grievance are identified. 7.6.3. Reports that do not meet the grievance system reporting requirements shall be returned to the Contractor for correction. Corrected reports must be resubmitted to Great Rivers within thirty (30) calendar days. 7.6.4. The Contractor must submit a report in a format provided by Great Rivers that includes: 7.6.4.1. The number and nature of Grievances. 7.6.4.2. The timeframes within which they were disposed of or resolved. 7.6.4.3. The nature of the decisions. 7.6.4.4. A summary and analysis of the implications of the data, including what measures will be taken to address undesirable patterns. 7.6.5. Using the form provided by Great Rivers, the Monthly Grievance report, is due to Great Rivers within twenty (20) days following the end of the month. 7.6.6. Using the form provided by Great Rivers, the Quarterly Grievance report is due to Great Rivers within twenty (20) days following the end of each calendar quarter.
Recordkeeping and Reporting Requirements. The Subrecipient shall have written policies and procedures to ensure the requirements of the ESG program are met and shall maintain sufficient records to document homeless status, at risk of homelessness status, determination of ineligibility, annual income, program participant records documenting eligible status, evidence of the use of the centralized or coordinated assessment systems and procedures, rental assistance agreements and payments, monthly allowance for utilities used to determine compliance with rent restriction, compliance with shelter and housing standards, records of the emergency shelter facilities assisted, services and assistance provided for each type of service, coordination with Continuum of Care and other programs, participation in HMIS, matching contributions for the matching requirement in §576.201 and all other requirements listed in 24 CFR § 570.500. Such information shall be made available to City for monitoring purposes upon request.

The Grantee must maintain all records documenting its compliance with the requirements of 40 CFR Part 33, including documentation of its and its prime contractors’ compliance with the six good faith efforts described in Subparagraph VI.B, above, and data relied upon in formulating its fair share objectives, if applicable. Any Grantee whose grant award is greater than $250,000 for any single agreement, or whose grant award(s) total over $250,000 in EPA financial assistance in any one fiscal year is required to maintain a bidder’s list in accordance with 2 CFR 33.501. 3. The Grantee shall provide the following information for all procurements entered into under this agreement with a Minority Business Enterprise (MBE) or Women’s Business Enterprise (WBE): (1) Type of Business Enterprise (MBE/WBE); (2) Dollar Value of the Procurement; (3) Date of Procurement; (4) Type of Product or Service Procured (Construction/Supplies/Services/Equipment); and (5) Name/Address/Phone Number of MBE/WBE Contractor or Vendor. The Grantee shall provide this information to the Department’s Contracts and Grant Management Unit, PO Box 420, 000 Xxxx Xxxxx Xxxxxx, Xxxxxxx, XX 00000, no later than the first week of October for all procurements made during the Federal Fiscal Year prior (October – September). Grantee should direct all questions regarding this requirement to the Contracts and Grant Management Unit at (000) 000-0000.

Other relevant records. 4.13.3 The provider shall allow the Department or its authorized representative to inspect and examine the provider's premises and/or records that relate to the performance of the agreement at any time during the period of the agreement and thereafter within the period specified herein for the provider’s retention of records. 4.13.4 The provider shall promptly provide the Department with access to Department clients and records of the Department clients without limitation.

Recordkeeping and Reporting Requirements. 1 0.5.1 The Contractor must maintain records of Grievances and Fair Hearings originating at or handled by the Contractor and keep them in confidential files from the Individual's clinical record. 10. 5.1.1 Grievance logs must be maintained by the Contractor on all Grievances filed directly with the Contractor. The Contractor must submit these logs to GCBH upon request. Grievance logs must demonstrate that all timeframes and notification requirements were fulfilled. Should the Contractor fail to meet the timeframes or notification requirements, the Contractor must notify the GCBH Grievance and Appeals Coordinator immediately.
Recordkeeping and Reporting Requirements. (1) The public utility must maintain a record of the following information for at least two years:(a) The number of complete large generator interconnection applications received; (b) The time required to complete the review process for each application; and (c) The reasons for the approval or denial of each application. (2) For as long as an interconnection customer's large generator facility is interconnected to a public utility's transmission or distribution system, the interconnecting public utility must maintain copies of the interconnection application, interconnection agreement, and certificate of completion for the large generator facility. The public utility must provide a copy of the interconnection customer's records to the interconnection customer within 15 business days after receipt of a written request. (3) The public utility must submit an annual report to the Commission summarizing the public utility's interconnection activities for the previous calendar year. The annual report must be filed by May 30 and must include the following information: (a) The number of complete large generator interconnection applications received;(b) The number of large generator facility interconnections completed;(c) The types of large generator facilities applying for interconnection and the nameplate capacity of the facilities;(d) The interconnection facilities required to accommodate the interconnection of a large generator facility and the estimated costs of those facilities; and
