Request Procedures. A. Employees desiring to engage in outside employment or activity, as defined by Section 2, shall obtain prior approval of the Employer by forwarding a request on CBP Form 3031, Request to Engage in Outside Employment or Business Activities. Provided the Form 3031 contains sufficient and complete information for the Employer to make a decision, the Employer will act on the request within thirty (30) working days after receipt of the Form 3031. B. In the event the employee has not received a response on his or her request to engage in outside employment or activity, as defined in Section 2 above, the Union may take the matter directly to expedited arbitration in accordance with Article 28: Arbitration. If the Arbitrator finds the Agency failed to provide a final C. If the Employer’s response is to disapprove the request, it will provide the employee a written explanation of the basis for the disapproval. D. If the Employer’s response is to approve the request, the employee is responsible for seeking re-approval if his or her duties materially change or he or she assumes a new position.

Request Procedures. Upon Licensor's agreement to license to Licensee Additional Capacity and Licensee's acceptance of Licensor's estimate of the Allocated Cost thereof (which acceptance shall be evidenced by Licensee's execution of the Cost Estimate Schedule and its return of same to such Licensor, not later than twenty-one (21) days after the date specified on such Cost Estimate Schedule; Licensee's failure to timely return any such Cost Estimate Schedule shall render its Request null and void), Licensor shall seek to obtain any required permits or similar authorizations and thereupon commence construction of such Facilities for Licensee. Upon agreement by Licensor and Licensee as to the license of Additional Capacity, and Licensee's acceptance of Additional Capacity pursuant to Section 2(d) below, the parties shall execute a Final Cost Schedule setting forth the final information regarding such Additional Capacity as is required by the form of Exhibit B. On the effective date or dates set forth in such Final Cost Schedule (or if not specified therein, the date on which the Additional Capacity is accepted by Licensee in accordance with Section 2(d) below or, if earlier, the date Licensee commences use of such Additional Capacity), such facilities shall be deemed to be Additional Facilities and the capacity thereof Additional Capacity, for purposes of this Agreement. Licensee's payments for any Additional Capacity shall be calculated as set forth in Annex A attached hereto, and the amount of such payments shall be set forth on the Final Cost Schedule relating thereto.
Request Procedures. Request for benefits shall be submitted in a sealed envelope to the Vice Chancellor for Human Resources and Employee Relations or his/her designee. The Vice Chancellor for Human Resources and Employee Relations or his/her designee shall receive the request and supporting medical documentation and certification to determine whether the unit member meets eligibility requirements. Such requests must include a physician's certification that the unit member is unable, as a result of a catastrophic illness or injury, to perform the material duties of his/her occupation. At its discretion, the District may require additional medical information or require examination by a second physician of the District's choosing and at the District's expense. After reviewing the application and supporting documentation, the Vice Chancellor for Human Resources and Employee Relations or his/her designee shall determine whether the unit member has met the criteria and furnished all documentation and number of days needed to bridge to eligibility for long term disability benefits. The decision of the Vice Chancellor for Human Resources and Employee Relations or his/her designee shall not be subject to the grievance and arbitration provisions of the Local 39-PCCD Agreement.
