Sick Leave Report. The College may request documentation from a qualified medical practitioner when returning to work following a sick leave absence of more than four (4) working days. The College may also request a report from a qualified medical practitioner when it appears that a pattern of absence is developing.
Sick Leave Report. The County shall provide the nurse with a written account of the sick leave the nurse has earned, has taken, and has remaining.
Sick Leave Report. A record of cumulative sick leave will be compiled and reported with the first check received in August.
Sick Leave Report. At least once each year the University shall notify each Employee of (1) the number of days of sick leave the Employee has accrued, classified as pre- and post- January 1, 1984, days; and (2) if applicable, the number of days of annual leave the Employee has accrued.

Sick Leave Report. (a) An employee absent from work through illness or injury shall, within two days of returning to work from the initial absence submit the required online leave request.
(b) The Employer may request a report from a qualified medical practitioner in one or more of the following circumstances:
(1) where it appears that a pattern of consistent or frequent absence from work is developing;
(2) where the employee has been absent for five consecutive scheduled days of work;
(3) where at least 15 days have elapsed since the last statement was obtained and the employee has been in receipt of plan benefits throughout the period. Benefits will cease to be paid when an employee fails to provide satisfactory evidence of medical disability during the benefit period. Where the Employer requests a report from a medical practitioner the Employer will bear the costs of the report.
Sick Leave Report. The University shall maintain employee leave balances on the University electronic self- service system. A printed copy will be provided upon request.
Sick Leave Report. All sick days that an employee has remaining during the calendar year shall be reported in writing to each employee upon request or by January 31 of each year.
