Tobacco Surcharge. Employees who use tobacco products will be required to pay an additional monthly surcharge of $50 for their medical insurance. An employee who has used a tobacco product(s) anytime within the last 60 days will be considered to be a user of tobacco products. The tobacco surcharge ($50 a month) will be enforced throughout the entire plan year unless the employee meets the requirements of the Affordable Care Act for a change in his/her status. Employees are required to complete an affidavit that indicates their status within 30 days of their hire date. Employees will be able to update their tobacco status between January 1, and October 15 of each year. Changes made during this period will apply for the entire next plan year. By choosing not to disclose tobacco status or by not completing the form, employees will be assessed the $50 monthly default charge, the same as a tobacco user.

Tobacco Surcharge. If an employee hired on or after January 1, 2018 who participates in the City’s insurance program and uses tobacco, the employee will be charged a twenty-five dollar ($25.00) per month surcharge effective January 1, 2018.

By failing to disclose tobacco status or by not completing the form, an employee will be assessed the $50 per pay default charge, the same as a self-identified tobacco user.
Tobacco Surcharge. If the employee and or his/her dependents covered under the Cuyahoga DD benefit plan have used any tobacco products more than 4 times in the last 6 months an additional surcharge will be assessed. The surcharge will be $15/pay, aside from the regular premium share. The surcharge may be waived if the employee or eligible dependent enrolls in and completes a smoking cessation program. An employee’s tobacco use will be based on the employee signing an affidavit attesting to his/her tobacco use and/or the eligible dependent’s use during the annual open enrollment period. Tobacco use is defined as: • Using any tobacco product other than for religious or ceremonial use • On average four or more times per week • Within no longer than the past 6 months Tobacco products would include, but not limited to: • Bidi • Chew • Cigarettes • Cigars • Dip • E-cigarettes • Hookah • Pipes If an employee submits false information or fails to notify Cuyahoga DD timely of the change in eligibility, the employee is subject to disciplinary action and will be personally liable to Cuyahoga DD for reimbursement of the costs of benefits and expenses associated with falsification.
Tobacco Surcharge. A. All bargaining unit employees (not including their spouses or dependents) who are enrolled in the District’s health benefit plan who are habitual or regular tobacco and/or nicotine users (excluding those who use such substances less than five times per year), including tobacco chewing, e-cigarettes and smokers, will be subject to an Employee premium tobacco surcharge of $100 per month per employee, unless the covered Employee is enrolled in a smoking cessation program approved by the District or Insurance Provider/ Administrator. B. All employees will be required to complete and execute a tobacco declaration form (said form to be drafted by the District in consultation with and the approval by the Association.) Those employees who declare on said form that they do not habitually or regularly use tobacco or nicotine products shall not be subject to the $100 per month surcharge. Such employees are however under a continuing obligation to disclose to the Employer any change in their tobacco use status. Any employee who declares use of such products shall be required to pay the $100 premium surcharge. However, in such cases the surcharge will be waived for any period the employee is enrolled in an approved tobacco-use cessation program.
Tobacco Surcharge. Effective January 1, 2019 and for the term of the Agreement, participating employees and/or their spouses who continue to use tobacco products or fail to submit to a recognized tobacco use treatment program shall be subject to a premium surcharge. The amount of the monthly surcharge shall be added to the employee and/or spouse’s share of the monthly premium cost. The surcharges presently in effect are as follows:

Failure to abide by this policy may result in civil penalties levied under Chapter 386, Florida Statutes and/or contract enforcement remedies.

  • Transportation Allowance When an employee is required to travel to the Hospital or to return to her home as a result of reporting to or off work between the hours of hours, (other than reporting to or off work for her regular shift) or at any time while on standby, the Hospital will pay transportation costs either by taxi or by her own vehicle at the rate of thirty-five cents ($0.35) per mile (to a maximum of fourteen dollars ($14.00)) or such greater amount as the Hospital may in its discretion determine for each trip between the aforementioned hours. The employee will provide to the Hospital satisfactory proof of payment of such taxi fare.

  • Transportation Reimbursement Employees who, during the course of their normal duties, are required to actually transport clients/consumers/felons in their own personal vehicle on a regular basis, are eligible for reimbursement for the cost of an automobile rider to their existing insurance policy. To be eligible for the reimbursement, the employee must demonstrate the following: 1. That he/she is normally required to transport clients/consumers/felons in the course of their duties. 2. That there is no access to or available State vehicles. 3. That public transportation cannot be used. 4. That their insurance company requires a special rider on their existing automobile policy. 5. Proof that such a rider has been purchased. 6. Proof of a valid driver’s license and insurance policy. By receiving such reimbursement, employees acknowledge that they may be required to use their own personal vehicle to transport clients/consumers/felons in the normal course of their duties. The reimbursement to such employee(s) is the actual cost of the rider not to exceed seventy-five dollars ($75) per year whichever is less. This reimbursement will be paid on a yearly basis in the pay period that includes July 1st. Employees who either resign, retire, or have their employment terminated during the year and employees who start during any part of the year will have the reimbursement prorated. In the case of employees who either retire, resign, or have their employment terminated will have that portion of the reimbursement repaid to the State, in the last paycheck.

  • CONTRACT CHARGES The Contract Charges for the Services shall be structured using any of the following pricing mechanisms (as may be agreed by the Parties and set out in an SOW); Capped Time and Materials; Price per Story; Time and Materials; Fixed Price (to be used only for Services that are ancillary to software development services); or using such other pricing mechanism or combination of pricing mechanism thereof as may be agreed by the Parties. In consideration of the Supplier’s performance of its obligations under this Contract and in consideration of the specific services that are set out in an applicable SOW, the Customer shall pay the undisputed Contract Charges in accordance with the relevant SOW for the Release and the payment provisions set out at Clause 14 (Payment and VAT). The Customer shall, in addition to the Contract Charges and following delivery by the Supplier of an Invoice, pay the Supplier a sum equal to the VAT chargeable on the value of the Services supplied in accordance with this Contract. If at any time during this Contract Period the Supplier reduces its framework Prices for any Services which are provided under the framework Agreement (whether or not such Services are offered in a catalogue (if any) which is provided under the framework Agreement) in accordance with the terms of the framework Agreement, the Supplier shall immediately reduce the Contract Charges for such Services under this Contract by the same amount. The Supplier shall in any event ensure that the Contract Charges are at all times compliant and consistent with the charging structure set out in framework Schedule 8 (Charging Structure) and do not exceed the prices set out therein. Contract Charges:

  • Direct Charges To the extent Cash-based Expenses are incurred by the Contractor, the Contractor shall be reimbursed for reasonable and necessary actual direct costs incurred (e.g., equipment, supplies, travel and other costs directly associated with the performance of the Agreement) to the extent required in the performance of the Work and to the extent such costs are anticipated in the Budget. Travel, lodging, meals and incidental expenses shall be reimbursed for reasonable and necessary costs incurred. Costs shall not exceed the daily per diem rates published in the Federal Travel Regulations. Reimbursement for the use of personal vehicles shall be limited to the Internal Revenue Service business standard mileage rate in effect at the time the expense was incurred.
