Unsafe Condition. Where an unsafe condition is alleged to exist, the affected employee shall first notify his / her immediate supervisor who shall take whatever necessary corrective action the supervisor deems appropriate. Where an unsafe condition is alleged to exist by the Union on behalf of affected employees, or the matter referred by the employee to the supervisor is not resolved to the satisfaction of the employee, the matter will be submitted to the next LMC for discussion. Nothing in this Article requires the Employer to take any corrective actions, and matters raised herein are not subject to the Dispute Resolution Procedures.
Unsafe Condition. The product contains design features where experience with other products in service has shown an unsafe condition might occur in that product, even though compliance with the regulations in the VA Certification Basis can be demonstrated. Unsafe is measured with respect to the overall level of safety intended by the product VA Certification Basis.
Unsafe Condition. If an employee has justifiable reason to believe that his safety is in danger due to an alleged unsafe working condition or alleged unsafe equipment, he must inform his supervisor and may inform the appropriate Union official. Serious disputes may necessitate the decision of the division head. Allegations of an unsafe working condition or equipment will not be applicable unless substantiated in writing as to date, time and witnesses involved. The requirement that there be a prompt resolution to safety disputes is of utmost concern to the City of Galesburg.
Unsafe Condition a) Any fixture or jig or moving machine which is in an unsafe or hazardous condition, shall be "red tagged" until it is made whole.
b) The following procedure for correction of unsafe conditions caused by malfunctions will apply plant-wide. It is the intent of the parties to use sound judgment and common sense in its application.
1) When an unsafe condition is observed by any person, the production supervisor having jurisdiction over the equipment in question will be paged over telecontrol to the location and corrective action will be initiated immediately. Examples of unsafe conditions are:
(i) Press drifting
(ii) Presses or fixtures double cycling
(iii) Hazardous conditions caused by machinery malfunction.
2) The production of parts will cease at that point and no part will be loaded into the press until it is made safe, unless it is absolutely necessary in order to cycle the press for testing purposes.
3) The production supervisor may cycle the press in order to observe the hazard. In any event, he/she will contact the maintenance supervisor in charge of the area and advise him/her of the situation. An electrician and millwright will be paged to the location or assigned by the maintenance supervisor.
4) The skilled tradesman/woman will cycle the equipment the necessary amount of times in order to make an evaluation with the maintenance supervisor present. The operator or the person who first observed the hazard will also be present. 95 No unskilled employees will operate such equipment or assist trades in any way until skilled trades gives the equipment back to production.
5) If any one of either the maintenance supervisor, the electrician or the millwright determines that a hazard exists, the machine will be "locked out" and a tag will be dated and signed by one of the tradesmen/women and the maintenance supervisor and placed in position.
6) The tradesmen/women will check and test all devices on the equipment that may be responsible for the existing malfunction. All necessary repairs will be effected. (Check Sheet).
7) In the event the job carries into the next shift, the original danger tag will remain in place and the machine will be "locked out". The next tradesman/woman and maintenance supervisor will date and sign another danger tag and follow the same procedure.
8) If it is found that the hazard must be corrected by a major repair which will take more than two (2) shifts, the electrician will remove the main fuses and deposit them in the maintena...
Unsafe Condition. This AD was prompted by a report of broken and bent attachment bolts of the main rotor (MR) hub scissors assembly and a determination that additional part markings of the washer, scissor branch, and mast ring of the corresponding nut side, and repetitive inspections of those part markings, are necessary to detect any rotation. The FAA is issuing this AD to address broken and bent attachment bolts of the MR xxx scissors assembly, which could lead to detachment of a MR hub scissors attachment bolt, possibly resulting in complete loss of control of the helicopter.
Unsafe Condition the Set-out Site is not safely accessible to Vehicles or Contractor’s employees.
Unsafe Condition. When an unsafe condition exists at a school, a custodial supervisor will be permitted to evacuate with other occupants of the school until the proper authorities arrive.
Unsafe Condition. This AD was prompted by the discovery during testing that the energy absorber (EA) installed on certain B/E Aerospace Xxxxxxx Common Seats 170/260 H160 may not function as intended during emergency landing. The FAA is issuing this AD to prevent malfunction of the EA on the seat. The unsafe condition, if not addressed, could result in injuries to the occupants during an emergency landing.

Unsafe Condition. This AD was prompted by reports of anomalies with TCAS units during a flight test over a high density airport. The TCAS units dropped several reduced surveillance aircraft tracks because of interference limiting. We are issuing this AD to prevent TCAS units from dropping tracks, which could compromise separation of air traffic and lead to subsequent mid–air collisions.
