WITHDRAWAL FROM PROGRAM. I understand that I must provide a two (2) week written notice of withdrawal from the program. If this notification is not provided, I agree to pay all tuition and fees for two (2) weeks, whether my child attends. I understand that when my child is withdrawn, s/he will only be eligible for re-admission based upon space availability and all other enrollment criteria. If my child is selected for re-enrollment, I will be required to complete an entire new Enrollment Agreement at the current rate and pay a new non-refundable Registration Fee at the current rate. If there is an outstanding balance, (including tuition or fees) when my child is withdrawn, I will be required to bring my account current prior to completing a re-enrollment application. I understand all fees (Tuition, Registration or Activity) are non-refundable.
WITHDRAWAL FROM PROGRAM. If a Partner elects to withdraw from the Program, Partner will notify the Program representative of its withdrawal at the address provided below and at such time as the Program receives such notice, Partner will no longer be engaged in Active Participation as defined above.
WITHDRAWAL FROM PROGRAM. The Resident withdraws from the Program for any reason, whether voluntarily or involuntarily.
WITHDRAWAL FROM PROGRAM. I understand that I must provide one week’s written notice of withdrawal from the program. If written notification is not provided, I agree to pay all fees for the program in which my child is scheduled to attend, if I wish to re-enroll an additional “Registration Fee” will be due. I agree to complete the Sign-In/Sign Out form, including a complete signature on a daily basis. I understand that my child will be released only to those persons whose name I have listed on the “Blue Card” for emergency purposes. I will notify the Director or other designated person in writing, if any person other than those listed may pick-up my child. Creative Center employees require proof of identification from any caller or person arriving to pick-up my child. It is my responsibility to update my employment information and any emergency contacts and phone numbers.
WITHDRAWAL FROM PROGRAM a. The District may, for good cause, terminate the assignment of any College Student to the early fieldwork experience placement with the District as detailed herein, and, upon request of the District, made for good cause, College shall withdraw the assignment of any such College Student participant. “Good cause” may include but is not limited to failure to perform satisfactorily, refusal to follow District administrative policies, procedures, rules and regulations, or violation of any federal or state law. The District reserves the right to ban anyone from District facilities when the District finds, in its sole discretion, that the presence of the person poses a threat or disruption to operations.

In addition, the school will close early 2 days per year for staff trainings. I agree that I will not receive a refund, credit, or any other allowances for holidays. If a holiday falls on a weekend, it will be observed on either the preceding Friday or the following Monday.
WITHDRAWAL FROM PROGRAM. It is assumed that all students who enrol for instrumental tuition will undertake a full year’s tuition. If it is necessary for a student to be withdrawn from lessons this is to be organised through written notification from the parents to the Co-ordinator of the Instrumental Program, Xx Xxxxxx Xxxxx at Secondary or Xx Xxxx Xxxxx at Primary, and the Instrumental teacher concerned by the 8th Friday of the term (Friday Week 8 of the respective term). The student must complete the term's tuition that notice has been provided in. Please note that if notice is not given by the required time, then the following term's fees will be charged and are payable in full. It is assumed that students will continue with instrumental tuition for each year of their schooling while at Nazareth. Please give notice to the Director of Music (Xxxx Xxxxx) at Primary or the Instrumental Coordinator (Xxxxxx Xxxxx) at Secondary by the end of Term 4 if a student will not be continuing the following year. Please note that new contracts will be issued at the start of every year to be signed by all parties to continue with the program.
WITHDRAWAL FROM PROGRAM. Participants who voluntarily withdraw from their course of study will be terminated as a project participant, or designated “on leave” depending on the situation. • Participants may be administratively withdrawn from the project due to poor academic performance, inadequate attendance, false financial claims, or disciplinary reasons.
WITHDRAWAL FROM PROGRAM. As a general courtesy, if you decide to withdraw your child from the Kidz Zone Before and/or After School Program, please provide the DPRD Community Center office with a minimum of two weeks, written notice. Parents are responsible for payment of fees through the end of the two-week period, regardless of whether or not their child participates in the program. Registration will be accepted beginning Wednesday, April 13th, 2022. All registrations will be taken on a first come, first serve, and walk-in or mail-in basis. Phone or online registration will not be accepted at this time. Waiting lists will exist for the current school year only. Families will be notified throughout the school year as spaces become available. DPRD strives to keep program fees low for participants. If your family would like to donate art supplies, craft supplies, board games, books, puzzles, learning toys or other supplies, please contact Xxxxx Xxx at the De Pere Community Center office, 339-4097.
