Contractor shall provide the following programmatic services to eligible individuals: a) Information and education about injury prevention to older adults and persons with disabilities. This can be done in coordination with the Office on Aging’s Health Education program. b) Referrals and provision of fall and injury prevention resources in eligible individuals’ local communities. c) In-home environmental assessments. d) Instruction on behavioral, physical, and environmental aspects of injury/fall prevention. 2. Contractor shall purchase injury prevention equipment, services, materials and labor costs for homeowners and renters who meet eligibility requirements established by the California Department of Aging and listed in Article III, C. above. Equipment, materials and services covered under the program shall include, but not be limited to the following: a) Grab bars, nonskid surfaces, toilet seat risers, shower seats, and transfer benches. b) Indoor and outdoor handrails and threshold modifications (e.g., ramps). c) Reconfiguration of furniture and other elements of the physical home environment to reduce hazards. d) Improved lighting including light fixtures, lamps and night lights. e) Medication management items (e.g., pill organizers). f) Licensed electrician services (i.e., electrical outlets installed by a licensed electrician when necessary to properly place wires that present a trip hazard). g) Unsafe flooring repair (e.g., fixing broken/uneven steps or replacing work carpet/tread). 3. To carry out the responsibilities of this Contract, the Contractor shall subcontract with local agencies that are experienced in injury prevention and home safety services, including, but not limited to, medical providers and hospital systems, independent living centers, and home modification providers.

Obtain design survey for the inside of existing well houses X0, X0, X0, X00, M17, M21, M27, M28, M33, M35, M37, M38, M39 and M40 to obtain dimensions, piping and well apparatus layouts and create an AutoCAD file containing raw survey point data, dimensions, and survey features for use in designing piping and building modifications necessary to install new meters. Survey shall also include photos of the inside and outside views of the well houses.” 2. Develop Preliminary Alternatives for Well House Structure Modification. Xxxx will develop up to 2 conceptual design alternatives for adding the necessary space requirements to the existing well house structures to include a new water meter and associated piping modifications to be approved by the CITY and Division of Water Resources. The conceptual design alternatives to be evaluated includes a fiberglass “dog-house” type structure integrated with the existing well house and well house expansion with similar materials as the existing structure. ENGINEER will prepare preliminary conceptual costs for each alternative for the CITY’s consideration. A meeting with the CITY will take place to discuss the alternatives and preliminary costs to determine the final selected alternative which will be pursued though design and development of construction documents. 3. Incorporate the design drawings and specifications for the Well House structure and internal piping modifications with the design drawings and specifications prepared for the Spurline Replacement Projects. 4. Prepare a final Bid Package (Bid Package No. 3) to include spurline replacements, well house structural and internal piping modifications for new meters for xxxxx X0, X0, X0, X00, M17, M21, M27, M28, M33, M35, M37, M38, M39 and M40. 5. If requested by the CLIENT, Wood will provide construction staking for the X0, X0, X0, X00, M17, M21, M27, M28, M33, M35, M37, M38, M39, and M40 Wellfield Well Spurlines.

Research has shown the results can include: increase in natural killer cell activity, reduction in stress and burnout rates, improvement in mood states, enhanced creativity and bonding. Xxxxx of Services. Scope of service is chemical delivery to Beaumont Wastewater Treatment Plant at 000 X. 0xx Xxxxxx, Xxxxxxxx XX 00000 Products to be provided: Citric Acid, Sulfuric Acid, Liquid Ammonium Sulfate, Sodium Hypochlorite, Sodium Bisulfite, and Sodium Hydroxide. Chemicals will be shipped with Northstar delivery equipment, or our manufacturers delivery equipment (see section D above). Citric Acid totes will be delivered via a common carrier freight company. If awarded the RFP, Xxxxxxx Xxxxxx, Project Manager, will conduct a site visit on the tank systems at Beaumont in order to assess the current tank system, site specific delivery requirements and to conduct a site safety audit for account setup. Once the account is setup, we can deliver as soon as City of Beaumont is ready.
