COUNTY DUTIESThe COUNTY agrees contingent upon receipt of a timely, specific request as provided in Paragraph 3(C), to provide the following election services:
COUNTY DUTIESDuring the term of this Agreement, the County agrees to: A Collect Samples from City wastewater facilities at regular intervals and in coordination with and under the supervision of the City’s Operator, but not more than twice per week. City shall provide County with the contact information for its Operator for purposes of this Agreement.
COUNTY DUTIES a. County shall provide SNF with as much complete information as possible regarding potential residents. County shall disclose if the potential resident has any history of violence, fire-setting, disregard for smoking rules, special dietary requirements, medical care needs, and/or infectious disease(s). b. County shall seek to place only those potential residents in beds covered by this Agreement who met “Special Needs SNF Admission Criteria” as established by the County and updated from time to time. Such criteria are described below in Special Needs SNF Admission Criteria. c. Community Programs-Placement, Director of Placement or designee shall provide an Authorization Letter at time of resident’s admission. d. County shall ensure that each resident has an assigned case manager and that the SNF Care Plan is reviewed and updated regularly. e. Community Programs-Placement staff shall transport resident to the County of San Mateo whenever necessary for court hearings regarding the continuation of conservatorship. The Community Programs-Placement case manager will maintain regular communication with the assigned conservator.
COUNTY DUTIES. County acknowledges and agrees to the provisions below: a) County shall provide a toll-free number for Arlington residents to call if they wish to be referred to a COVID-19 sample collection site. County will refer symptomatic individuals with a valid prescription from a physical to an available site for sample collection. b) The County, or its contractor or other partners, shall furnish the Site with the following:
COUNTY DUTIESThe County will have the following duties and responsibilities: A. To monitor the services provided by the Liaison. The services of the Liaison shall be monitored by the Xxxxxxx-Xxxxxxxxx Administrator of Human Services (“Project Monitor”). The Liaison shall provide a monthly report including financial, operations and program information to the Project Monitor in a format approved by the Project Monitor. The Project Monitor shall submit quarterly reports to the Mayor of the City of Lincoln, containing her review of the services provided by the Liaison. The Project Monitor shall also report noncompliance to the Mayor of the City of Lincoln. In the event of the Liaison’s failure to perform the services in accordance with the Liaison’s contract, job description, and/or Exhibit A, the City may take action to terminate the Liaison. B. To provide the Liaison with office space in the Lancaster County Human Services office, and use of miscellaneous office supplies, printing, postage and other office equipment.

COUNTY DUTIESThe Board will provide Fire & EMS dispatch areas to The Villages® Development in Lake County for the District in the same manner and procedures as provided in Sumter County with two notations: this includes the use of nurse navigator when the District elects to use this for low acuity calls and includes connection of District’s personnel to the District’s contracted medical direction via telephone or radio.
COUNTY DUTIES. COUNTY agrees to perform the following duties: 2.3.1 Provide legal authorization for autopsy examination to UTMB prior to examination. 2.3.2 Provide necessary information, including related police reports, medical records, names of treating physicians, allied information, and support necessary for an informed pre-examination and review. 2.3.3 Provide the body with clothing and, if all or part of the clothing has previously been removed, provide it with the body for examination, correlation, and documentation. 2.3.4 Provide any items associated with treatment and/or life saving attempts undisturbed and, if necessary, assist in distinguishing features of medical/surgical treatment from those due to injury. When available, obtain and forward for analysis any blood, body fluid or other such specimens obtained during life. 2.3.5 Provide for examination any medications prescribed for, or thought to have been used by, the deceased. 2.3.6 Provide for examination any weapons, ammunition, instruments, or case- related items such as suspected suicide notes, or copies of notes, especially any that are found on/about the person of the deceased. If a weapon is not provided, will provide a detailed description with appropriate measurements. 2.3.7 If the identity of the deceased is unknown or in doubt, help acquire any information, items or records necessary to help establish identification by scientific means. 2.3.8 In the event the next-of-kin should request tissue or organ donation, coordinate procedures with UTMB to promote optimum handling and to ensure that appropriate examination can be completed. 2.3.9 Transport the body to and from the morgue to the autopsy room at the location identified in Section 1.2.3. 2.3.10 If requested, send an authorized representative to take possession of items of value or evidence that may be discovered. 2.3.11 Provide the necessary personnel to perform any investigatory functions and related administrative support for crime scenes and death sites. 2.3.12 Notify UTMB of any severely abusively injured child at time of hospitalization. 2.3.13 Promptly make arrangements to remove the remains of the deceased after the performance of the autopsy if the next-of-kin have not been informed of the death or no next-of-kin has been located or identified.
