The following items are NOT covered: A Chromebook/Device that is lost or stolen Damage caused by negligence including but not limited to leaving it outside in an automobile, immersion in liquid, any type of damage caused by food or drink, damage caused by pets, rough/inappropriate handling, etc. Intentional misuse of one’s own or a peer’s device More than one accidental incident, including more than one broken screen or accessories. Coverage and Benefit. The total Chromebook Protection Fee is $20.00 per school year. (ONLY one protection fee purchase per school year). Partial semesters are not refundable. Must be paid before Chromebook is distributed the first week of school. If you purchase the protection fee after September 30th, 2019 a $20 late fee will be billed should you access the protection coverage. • This protection fee covers the chromebook loaned to the student against all damages, loss or theft up to $85.00. (Total cost of a chromebook approx. $220) Any additional costs to repair or replace the chromebook will be paid by the student. The benefit covers the chromebook only. (Charger is NOT covered). • This coverage is effective from the date this required form and optional protection payment are received by the school through the date the chromebook equipment is to be returned in good order to the school. • In the event a chromebook is lost or stolen, a police report must be filed and a copy provided to the school before a replacement is issued. • It will be the responsibility of school administration to determine if damages were due to negligence or accidental. A student with a damaged chromebook will be given a replacement. If the chromebook or its replacement is damaged beyond repair or lost or stolen a second time, the student may not be permitted to take the chromebook outside of the building. Coverage and Benefit. This agreement covers the computer loaned to the student against failure and/or damage over $100.00. The agreement covers the equipment 24 hours per day both inside and outside of school. Total repair/replacement costs will be determined at the time repairs are made. In the event of accidental damage or malfunction, the user will be responsible for 100% of the repair costs up to a maximum of $100.00. Payment for repairs will be due at the time repairs are made. Theft claims for the first occurrence will be subject to $200.00 maximum whereas a second theft second claim will be subject to full replacement cost or $405. Any careless, malicious or intentional damage to the equipment is not covered by this policy and will require the full replacement cost to be borne by the parent on behalf of the student. This policy does not cover lost equipment. Accessories valued less than $50.00 are not covered by this agreement. In no case shall the value be greater than $405.
Coverage and Benefit. This protection plan covers the Chromebook loaned to the student against all accidental damage, theft, vandalism or loss for one (1) school year. This program does NOT cover damage or loss of the power cord, which we ask students to keep at home. This program does NOT cover intentional misuse, abuse or neglect by any household members. If the student did not exercise proper care and/or take appropriate precautions, as described in the student handbook, and this behavior resulted in damage to or loss of the Equipment, the cost of the repair or replacement will be the responsibility of the student/parent. Upon paying for the Chromebook Protection Plan, if the Chromebook is accidentally damaged or vandalized, it will be repaired as shown in the table below. Fraudulent reporting of theft will be turned over to the police for investigation. A student making a false report will also be subject to disciplinary action.
Coverage and Benefit. This agreement covers the iPad loaned to the student for the 2020-2021 school year against incidental damage or loss over $50. Accessories (charger and case) and damages valued at less than $50 are NOT covered and are the sole responsibility of the student. Coverage is 24 hours per day. The Technology Protection Plan will only cover two incidents for the school year. NEGLECT IS NOT COVERED. If paid, the Technology Protection Plan will release the parent/guardian from having to pay for loss or damage for the following reasons: • Damages resulting from normal use and accidents. • Loss or damages resulting from theft. An official copy of a police report is required. • Loss or damages resulting from a fire. An official fire report from the investigating authority is required. • Loss or damages resulting from a natural disaster. • Damages resulting from a power surge. • Damages resulting from vandalism by another individual. If found guilty, the vandal will be liable to pay for damages. Exclusions (parent/guardian will be liable for entire replacement cost of the device): • Intentional damages. • Lying on any damage report. • Not carrying the iPad in a district-provided protective case. • Negligence (including loss from carelessness). • Loss or damages resulting from fraudulent, intentional, or criminal acts. Effective and Expiration This coverage is effective from the date premium payment is received by the school through the date at which the iPad is required to be returned in good working order. Premium The total premium cost for the 2020-2021 school year is $25. The Technology Protection Plan is not refundable. Payments can be made through XxXxxxxxxxXxxx.xxx until August 31 or by check made out to Xxxxxxxx City Schools. If paying by check, please write your student’s ID number and TPP in the memo line on your check and turn it in to your school office. It is agreed and understood that: • The HCSD Technology Protection Plan will offer coverage to all students. • Participation is totally voluntary. If a student does not participate, they will be responsible for any costs associated with repairs, loss, or damage. • A separate premium will be needed for each iPad covered. The Building Administration will determine if damages were due to negligence or accident. The Technology Protection Plan will only cover two incidences per school year. Coverage will be in effect upon payment of the premium. The Technology Protection Plan will follow Board Policy-JN.

The district acknowledges that due to the normal usage of district technology that maintenance and/or repair of district technology may be necessary due to no fault of the student. These routine maintenance issues and malfunctions that are not due to “visible physical damage” of the MacBook Air will be completed at no cost to the student. The following policy applies to accidental damage to the MacBook Air. *The AC adapter is not included in this weighted repair policy. In the event of a lost or damaged adapter, the Xxxxxx Technology staff will replace the adapter and the full replacement cost will be the responsibility of the borrower. Willful or negligent damage is not covered by this agreement and replacement or repair costs will be the responsibility of the student. The district will determine visible physical damage and the value of that damage will be based on repair/replacement costs or the assess loss of resale value. In no case shall the value be greater than $900. For every incident of accidental damage or loss, district coverage will progressively decline as follows:

For a second occurrence the student will be responsible for 50% of the cost of repairs or replacement. After two occurrences, the student will be responsible for 100% of the cost of repairs or replacement. Accessories (charger and case) and damages valued at less the $40.00 are NOT covered and are the sole responsibility of the student. Coverage is 24 hours per day. Students with missing keys will be charged $5 per key that is missing.
Coverage and Benefit. This agreement covers the computer and carrying case loaned to the student against all damage or loss over $100.00. Accessories valued less than $100.00 are not covered. Coverage is 24 hours per day. Total value will be determined at the time of loss or damage. The student and/or family will be responsible for the first $100.00 of damages/repair to the computer.

Accessories (charger and case) and damages valued at less than $50 are NOT covered and are the sole responsibility of the student. Coverage is 24 hours per day. If an iPad is accidentally damaged throughout the coverage period, a replacement iPad will be given. If a student’s iPad is accidentally damaged a second time, the parent must pay an additional $50 to provide coverage for the iPad for the balance of the school year. Negligent damage is the sole responsibility of the student and parent. The Technology Department will determine if the damages were accidental or due to negligence. Effective and Expiration This coverage is effective from the date this required form and premium payment are received by the school through the date when the iPad is required to be returned in good order to the school. The iPad Protection Fund provides protection for only the iPad device. Premium The total premium is $50 per year for each student in grades 9-12. It is agreed and understood that: • The Kalida High School iPad Protection Fund will offer coverage to all students. • Participation is totally voluntary, however, if a student does not participate, he or she will not be allowed to take his or her iPad off of school property. • A separate application will be needed for each iPad covered. A student with a damaged iPad will be given a replacement during the time needed for repair.
