DESCRIPTION OF SERVICES AND STAFFING. 4 4.1 CONTRACTOR agrees to provide those services, facilities, 5 equipment, and supplies, as described in the Exhibit(s) A through C to the 6 Agreement between County of Orange and , attached hereto and 7 incorporated herein by reference: Exhibit A (Wraparound Orange County (Wrap 8 OC) Provider Network Program (PNP) Services) which include general 9 requirements for all Service Components; Exhibit B (Wrap OC PNP Sexual 10 Behavior Treatment (SBT) Services); and Exhibit C (Wrap OC PNP Tutoring 11 Services).

DESCRIPTION OF SERVICES AND STAFFING. 8 4.1 CONTRACTOR agrees to provide those services, facilities, 9 equipment, and supplies, as described in the Exhibits to the Agreement between 10 County of Orange and Olive Crest, for the Provision of Wraparound Orange 11 County Direct Services, attached hereto and incorporated herein by reference: 12 Exhibit “A” relating to Direct Services, and Exhibit “B” relating to 13 Definitions. CONTRACTOR shall operate continuously throughout the term of 14 this Agreement with the number and type of staff described and as required for 15 provision of services hereunder.
DESCRIPTION OF SERVICES AND STAFFING. COUNTY agrees to provide those services, facilities, equipment and supplies as described in Exhibit A and budgeted for in Exhibit B, attached hereto and incorporated herein by reference. COUNTY shall operate continuously throughout the term of this Agreement with at least the minimum number and type of staff required for provision of services hereunder. Such personnel shall be qualified in accordance with all applicable statutes and regulations. COUNTY agrees to submit to CITY a list of persons who are to provide such services, by name, title, professional degree and experience, and any changes thereto. COUNTY further agrees to comply with all provisions, to perform all work and provide all services set forth in this Agreement in a professional, timely and diligent manner.

DESCRIPTION OF SERVICES AND STAFFING. CONTRACTOR’S staff will essentially function like COUNTY staff, receiving Human Resources Agency screened and referred xxxxxx children who are assessed by Children’s Mental Health and then assigned to COUNTY or CONTRACTOR staff for reunification mental health services. Five (5.0) full-time contract staff will provide the full array of mental health rehabilitation services including assessment, individual, collateral, group, crisis intervention and case management services. These full-time staff will be required to provide some evening and weekend work to meet the needs of families receiving services. Like COUNTY staff, CONTRACTOR’S staff will provide the majority of services in the field homes, school, community). In addition to the five counselors, a half time clinical supervisor and other needed support staff will be added to the program. Only four of the five clinical staff must be dedicated to providing SIS services. The fifth staff can serve community referrals who have Medi-Cal. Quality Assurance Program: CONTRACTOR’S staff will participate in the COUNTY’S clinical meetings, when appropriate, interagency collaborative efforts with the Human Resources Agency and are part of the Quality Assurance (QA) plan under the Medi-Cal Rehabilitative Option. CONTRACTOR’S staff will be assessed through the Quality Improvement Committee’s guidelines as well as through the Children’s Mental Health evaluation component.

DESCRIPTION OF SERVICES AND STAFFING. CONTRACTOR agrees to provide those services, facilities, equipment, and supplies, as described in the Exhibit A attached to this Agreementto the Agreement between County of Orange and The Regents of the University of California, for the Provision of Elder and Dependent Adult Abuse Consultation Services, attached hereto and incorporated herein by reference. CONTRACTOR shall operate continuously throughout the term of this Agreement with the number and type of staff described and as required for provision of services hereunder. Subject to thirty (30) days advance written notice, ADMINISTRATOR and CONTRACTOR may agree upon changes in staffing allocations to reflect current workload demands or service needs as long as COUNTY’s maximum funding obligation, as set forth in this Agreement, is not exceeded. Upon the request of ADMINISTRATOR, CONTRACTOR shall send appropriate staff to attend an orientation session and subsequent training sessions given by COUNTY.
