Description of Services to be Performed by Contractor. In full consideration of the payments herein provided for, Contractor shall provide Mental Health Services authorized by the San Mateo County Division of Mental Health, and as meet medical necessity. These services shall be provided in manner prescribed by the laws of California and in accord with the applicable laws, titles, rules, and regulations, including quality improvement requirements of the Xxxxx-Xxxxx/Medi-Cal Program. All payments under this Agreement must directly support services specified in this Agreement. The San Mateo County Mental Services Documentation Manual (“County Documentation Manual”) is included herein by reference. To the extent that there is inconsistency between a provision in the County Documentation Manual and this Agreement, the provisions in the County Documentation Manual shall prevail.
Description of Services to be Performed by Contractor. Contractor shall provide a Multi-Cultural Wellness Program, the Xxxxxx Project, for behavioral health clients/consumers and their family members in the Coastside, serving from Half Moon Bay to Pescadero. The purpose of this program is to provide culturally responsive community-based mental health and substance use services and programming, including: peer support groups, art and wellness activities, capacity building, outreach and linkages to behavioral health services and other resources as needed for marginalized ethnic, linguistic and cultural communities in the Coastside region. Contractor is the Lead Agency to coordinate and manage services and shall work with San Mateo County Behavioral Health and Recovery Services (BHRS) staff (“County”) to implement these services in accordance with the California Mental Health Services Act (MHSA) requirements.
A. Scope of Work - Service Approach
1. Trauma-informed All services provided through this agreement shall incorporate the six key principles of a trauma-informed approach into all programming for youth and adults with mental illness and/or co-occurring substance use challenges and their families. The six key principles are: safety, trustworthiness and transparency, peer support, collaboration, empowerment and cultural, historical and gender issues.
Description of Services to be Performed by Contractor. Funding will be used to replace inadequate perimeter security fencing enclosing outdoor play areas at the Belle Haven Child Development Center (BHCDC) with new perimeter security fencing, locks, and gates; and repaint portions of the building exteriors. The City of Menlo Park runs the BHCDC, a licensed, preschool childcare program serving low-income San Mateo County children between the ages of two to five years of age. The BHCDC serves up to 98 children and their families by providing year-round, full-day licensed preschool child care. Scope of Work: • Remove 110 linear feet of existing 4’ tall perimeter fence • Install 110 linear feet of new 6’ tall black chain link perimeter fence • Replace 7 single swing gates with 7 new security gates • Install locks, exit alarms, and emergency egress push bar door handles on 7 gates • Repaint portions of building exteriors Fence removal and replacement $21,500 Gates, alarms, and hardware $12,200 Exterior paint $11,900

a. Develop, format, mail, receive, and analyze on an ongoing basis Inpatient satisfaction surveys of patients on the Medical/Surgical/ICU services at San Mateo Medical Center (SMMC) and present results of those surveys to SMMC. Where surveys include additional comments, Contractor will input and assess such additional comments.
b. Include in the survey all federally-mandated hospital satisfaction survey items under the Hospital Consumer Assessment of Health Providers and Systems (HCAHPS).
c. The estimated number of surveys to be mailed annually is 2,167 for HCAHPS 1st wave mailing and 1,765 for HCAHPS 2nd wave mailing.
2. Effective January 1, 2014 through December 31, 2016.
a. Contractor will make arrangements for two SMMC staff to attend the annual National Client Conference. In addition, Contractor will mediate meetings with SMMC staff to discover ways to improve the Patient Experience.
3. Effective January 1, 2014 through December 31, 2016.
a. Develop, format, mail, receive, and analyze on an ongoing basis satisfaction surveys of patients on the Emergency Department services at San Mateo Medical Center (SMMC) and present results of those surveys to SMMC. Where surveys include additional comments, Contractor will input and assess such additional comments.

A. Day Treatment (Intensive/Rehabilitative) Services (Full-day) programs, Medication Support Services, Mental Health Services, and Crisis Intervention
1. General Description of Services
a. The Day Treatment (Intensive/Rehabilitative) Services, (Full-day) programs, Medication Support Services, Mental Health Services and Crisis Intervention shall collectively be referred to herein as “Services.” These Services are provided to a distinct group of seriously emotionally disturbed children and adolescents and occur in a therapeutic, organized and structured setting.
b. As of the date of this Agreement Contractor provides Day Treatment (Intensive and Rehabilitative) Services (Full-day) for severely emotionally disturbed children/youth. Such Day Treatment (Intensive and Rehabilitative) Services (Full-day) shall collectively be referred to herein as “Day Treatment Services.”
c. Full-day Day Treatment Services must be available more than four (4) hours and less than twenty-four (24) hours each program day to qualify as a full day program. Half- day Day Treatment Services must be available at least three (3) hours each day the program is open to qualify as a half day program. A. Mental Health Services (authorized by the MHP)
1. General Description of Services Contractor shall provide services for clients under the MHP. These services shall be provided to Medi-Cal eligible beneficiaries, clients who are covered by the Healthy Families and Healthy Kids Programs, client caregivers who are covered by HealthWorx, clients who are covered by the Health Plan of San Mateo CareAdvantage program for Medicare, and clients known to be indigent, for whom the MHP has assumed responsibility.
a. All clients shall be authorized for service by the Mental Health Services Division’s ACCESS Team. Separate authorizations shall be required for assessment and ongoing treatment services.
b. These services shall be provided in a manner prescribed by the laws of California and in accord with all other applicable laws, titles, rules, and regulations, including quality improvement requirements of the Xxxxx- Xxxxx/Medi-Cal Program. All payments under this agreement must directly support services specified in this Contract.
c. Services must be pre-authorized by the MHP.
d. After a clinical assessment is completed, Contractor shall notify the ACCESS Team within five (5) working days of completion of assessment with result of the assessment. If the results include a recommendation that Contractor provide further treatment, additional authorization must be obtained.
e. All services shall be provided by licensed, waivered or registered mental health staff.
