(3) Life Insurance: The County will provide group life insurance equal to one times an employee’s annual salary. (minimum $10,000, maximum $50,000). Optional life insurance will be employee paid.

1. For employees choosing coverage, the City will pay up to the median cost of the CalPERS medical premium for single, two-party, and family coverage for employees, spouses, domestic partners and dependents.
2. Employees with duplicate health coverage may choose to cash in five hundred twenty-five dollars ($525.00). An employee must submit a written request for payment and annual evidence of duplicate coverage.
Employee Insurance. If requested in writing by Owner, Manager shall require that all contractors and service companies operating in or on the Project maintain such worker’s compensation, employer’s liability and comprehensive general liability insurance as may be required by Owner, including any special coverage required by law or Owner in connection with hazardous operation.

Employee Insurance. An Employee’s insurance will become effective on the date he or she becomes eligible, on condition that an application has been received by the Administrator before such date, or within the 31 days thereafter; otherwise, coverage becomes effective on the first day of the month following acceptance of Evidence of insurability by the Insurer.
Employee Insurance. The City shall provide all Bargaining Unit Employees with health insurance benefits (including medical ((comprehensive hospitalization, surgical, major medical, physician service coverage)), prescription drug, vision and dental), under the group insurance and benefit plans generally provided to the employees of the City, on the same terms and conditions, and at the same rates on which those benefits are generally provided to employees of the City other than those covered by other labor contracts, except as set forth herein in Article 24. The City, in its sole discretion, may modify such benefits, the City’s and employees’ share of the cost of such benefits, the terms and conditions on which such benefits are provided, and/or the means by which such benefits are provided, so long as any such modifications are also applicable generally to employees of the City other than those covered by other labor contracts.

The City, in its sole discretion, may modify such benefits, the City’s and employees’ share of the cost of such benefits, the terms and conditions on which such benefits are provided, and/or the means by which such benefits are provided, when and as it deems necessary, for any reason so long as any such modifications are also applicable generally to employees of the City other than those covered by other labor contracts.
