Health and Medical Insurance. Executive shall be entitled to participate in any health and medical insurance plan(s) offered to NCRIC's senior executives and/or key management employees as a group in accordance with the terms of such plan(s), as they may be modified in their application to all employees at NCRIC's discretion from time to time.
Health and Medical Insurance. Employer shall pay for and provide Executive with the same types of health, medical, dental and vision insurance, if any, as are provided from time to time to all Employer's executive-level employees.

The Board and Association recognize that the Health Plan Document governs many of the policies and practices of the Wellness and Major Medical program and cannot be fully referenced within this document. The Board agrees to pay 100% of each individual full-time teacher’s premium for Major Medical insurance regardless of plan. Each participating employee or married couple will contribute a $150 per year enrollment fee to the plan. The difference in premium, if any, in family and/or dependent coverage shall be paid at the rate of 75% by the Board and 25% paid by the employee. If married employees are both eligible for Board-paid individual premiums, then the Board agrees to pay 100% of the family premium. Major medical coverage is provided through an agreement between the Board and the insurance carrier. The Board recognizes the importance of collaboration between the Association and the Board to provide appropriate coverage within existing financial parameters. The Board will work in a collegial manner to provide for the needs of the Association members and all insured, where feasible. The Board has final discretion regarding decisions concerning premium levels, coverage, and benefits provided.
Health and Medical Insurance. I certify that I have no medical or health-related problems which would preclude or restrict my participation in this Activity. I acknowledge that I have been advised to consult with a doctor if I have any concerns about my ability to participate in this Activity. I understand that Oregon Wild does not carry any insurance that would cover any injuries or losses I may suffer while participating in this Activity. I acknowledge that I am solely responsible for any and all costs of medical treatment required by me or on my behalf, I agree to pay for such medical treatment, and I assume all risks of such medical treatment. I certify that I have my own health insurance in effect.

The University assumes any and all risks of personal injury and property damage attributable to the negligent acts or omissions of the University and its officers, employees, servants and agents while acting within the scope of their authorized powers and duties of their employment by the University. University, as a state entity, warrants and represents that it is self-funded for liability insurance, both public and property, with said protection being applicable to officers, employees, servants and agents while acting within the scope of their employment by the University. Nothing contained herein shall be construed or interpreted as (1) denying to either party any remedy or defense available to such party under the laws of the State of Florida; (2) the consent of the State of Florida or its agents and agencies to be sued; or (3) a waiver of sovereign immunity of the State of Florida beyond the waiver provided in Section 768.28, Florida Statutes. Facilitator shall provide comprehensive general liability insurance on the Facility stipulating limits of not less than $500,000 per person/$500,000 per occurrence.
Health and Medical Insurance. Executive shall be entitled to elect to (i) continue to participate in the German private health and medical insurance plan in which he is currently enrolled, premiums for which are currently reimbursed to him or paid by EVI, or to (ii) participate in the Company's U.S. health plan according to the terms thereof. In the event Executive elects to continue to participate in the German private health and medical insurance plan, the Company shall make, or cause EVI to make, such private health insurance premium payments to the extent such payments would have been made by EVI or reimbursed to Executive pursuant to his current plan in Germany had he remained employed in Germany by EVI. In the event Executive elects to participate in the Company's U.S. health plan, he shall participate therein according to the terms and conditions applicable to the Company's U.S. employees.
Health and Medical Insurance. The Company shall pay all premiums otherwise due from the Executive for family coverage in the medical and dental insurance programs offered by the Company to its executives from time to time.
